This permit shall become null and void if the work is not commenced within 180 days for the date of issuance, or if
the work is suspended or abandoned for a period of 180 days from the last inspection.
PERMIT CONDITIONS:
1: ***PUBLIC WORKS PERMIT CONDITIONS***
2: Schedule and attend a Preconstruction Meeting with the Public Works Department (Dave Stuckle, Public
Works Inspector), prior to start of work under this permit. To schedule, call Public Works at (206) 433-0179.
3: The applicant or contractor must notify the Public Works Inspector at (206) 433-0179 upon commencement
and completion of work at least 24 hours in advance. All inspection requests for utility work must also be
made 24 hours in advance.
4: Permit is valid between the weekday hours of 7:00 a.m. and 10:00 p.m. only. Coordinate with the Public
Works Inspector for any work after 5:00 p.m. and weekends.
5: Any material spilled onto any street shall be cleaned up immediately.
6: Temporary erosion control measures shall be implemented as the first order of business to prevent
sedimentation off-site or into existing drainage facilities.
7: The site shall have permanent erosion control measures in place as soon as possible after final grading has
been completed and prior to the Final Inspection.
8: From October 1 through April 30, cover any slopes and stockpiles that are 3H:1V or steeper and have a
vertical rise of 10 feet or more and will be unworked for greater than 12 hours. During this time period,
cover or mulch other disturbed areas, if they will be unworked more than 2 days. Covered material must
be stockpiled on site at the beginning of this period. Inspect and maintain this stabilization weekly and
immediately before, during and following storms.
9: From May 1 through September 30, inspect and maintain temporary erosion prevention and sediment at
least monthly. All disturbed areas of the site shall be permanently stabilized prior to final construction
approval.
10: Contractor shall obtain a separate water service capping permit from WD #125 @ 206 242-9547 ans
sanitary sewer capping permit from Valley View Sewer District @ 206 242-3236.
11: ***BUILDING PERMIT CONDITIONS***
12: Work shall be installed in accordance with the approved construction documents, and any changes made
during construction that are not in accordance with the approved construction documents shall be
resubmitted for approval.
13: All permits, inspection record card and approved construction documents shall be kept at the site of work
and shall be open to inspection by the Building Inspector until final inspection approval is granted.
14: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap
the sanitary sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks,
wells, and other excavations. Final inspection approval will be determined by the building inspector based
on satisfactory completion of this requirement.
15: All plumbing and gas piping work shall be inspected and approved under a separate permit issued by the
City of Tukwila Building Department (206-431-3670).
16: All electrical work shall be inspected and approved under a separate permit issued by the City of Tukwila
Permit Center.
17: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an
approval of, any violation of any of the provisions of the building code or of any other ordinances of the City
of Tukwila. Permits presuming to give authority to violate or cancel the provisions of the code or other
ordinances of the City of Tukwila shall not be valid. The issuance of a permit based on construction
documents and other data shall not prevent the Building Official from requiring the correction of errors in
the construction documents and other data.

abatement of the asbestos materials greater than 1% from the main office
building located at 4021 S. 144th St., Tukwila, WA 98168.
These materials have now been removed from this location per Asbesto-Test,
Inc. Asbestos Survey, dated March 31, 2016, and ECO Environmental Services,
Inc. contracted scope of work.
All work was done in accordance with all federal, state, and local regulations and
the materials were disposed of at approved landfills for asbestos waste.

On March 22, 2016, Asbesto-Test personnel conducted an Asbestos survey (per
U.S.E.P.A./A.H.E.R.A. guidelines as designated and specified by Puget Sound Clean Air
Agency and Washington State) of the office, garage, and pole building (a, 4021 S. 144th St,
Tukwila, Kin2 County, Washington 98168. This survey purpose is to identify asbestos
containing materials, prior to demolition.
NARRATIVE OF FINDINGS
BASIC CONSTRUCTION: The office is one story on a concrete slab. The siding is concrete
masonry unit (CMU block). The CMU block and mortar materials were sampled. vapor barrier
materials were sampled. The roofing is torch down. The torch down materials were sampled.
INTERIOR CONSTRUCTION, FINISHES, AN FLOORINGS: The interior
construction is wood and drywall. The drywall/taping materials were sampled. The floor tile and
relating backing and mastic materials were sampled.
INSULATION: The insulation was sampled.

ELECTRICAL SYSTEM: The electrical wiring insulation did not appear to be the kind to be
ACM. The power may have been "on" at the time of the inspection, thus sampling was not safe.
Any suspect electrical wiring insulation or any suspect TSI (Thermal System Insulation) found
in, around, or behind any located fuse or breaker boxes should be considered to be ACM
(Asbestos Containing Material), unless determined otherwise by laboratory analysis.
HEATING AND VENTILATION SYSTEM: Heating is portable. There was no visible
suspect ACM located associated with any heating unit. Note: It was not possible to dismantle the
heating unit to locate and/or evaluate any additional suspect ACM which may be concealed,
inside the unit, or associated in hidden areas. It is our opinion any further handling of the
heating unit should be coordinated by the project manager and/or abatement contractor. If any
additional relating suspect materials are located prior to and/or during any demolition or
renovation, any and all additional related suspect materials should be considered to be ACM
(Asbestos Containing Material), unless determined to be otherwise by laboratory analysis. Any
metal insulated chimney system that may be present is presumed asbestos containing.
MISC.: None.
ADDITIONAL STRUCTURES ON SITE INCLUDED IN SURVEY: There is a wood
frame garage and a metal frame pole barn on the property. The garage has composition roofing.
The composition roofing materials were sampled. The internal construction of the garage is wood
and drywall. The drywall and taping compounds were sampled. The insulation from the garage
was also sampled. There was no suspect ACM located in the pole barn.

to be non -asbestos by laboratory analysis.
Asbesto-Test, Inc. does not guarantee approximations of quantities of ACM, which may be listed with the analyses. It is therefore
recommended professional abatement price and/or disposal quotes be obtained by inquiring as to fees per area of specific ACM
material (Le. square or linear foot etc.), or by on site assessment.
Any and all materials identified as ACM and/or PACM in this report must be abated prior to disturbance in renovation or demolition.
PLM (polarized light microscopy) has been known to miss asbestos in small percentages of some samples, which contain asbestos,

5. THE TESC FACILITIES SHOWN ON THIS PLAN ARE THE MINIMUM REQUIREMENTS FOR
ANTICIPATED SITE CONDITIONS. DURING THE CONSTRUCTION PERIOD, THESE TESC
FACILITIES SHALL BE UPGRADED AS NEEDED FOR UNEXPECTED STORM EVENTS (E.G.,
ADDITIONAL SUMP PUMPS, RELOCATION OF DITCHES AND SILT FENCES, ETC.).
6. THE TESC FACILITIES SHALL BE INSPECTED DAILY BY THE APPLICANT/TESC SUPERVISOR
AND MAINTAINED TO ENSURE CONTINUED PROPER FUNCTIONING. WRITTEN RECORDS
SHALL BE KEPT OF WEEKLY REVIEWS OF THE TESC FACILITIES DURING THE WET SEASON
(OCT. 1 TO MARCH 31) AND OF MONTHLY REVIEWS DURING THE DRY SEASON (APRIL 1
TO SEPTEMBER 30).
7. ANY AREA OF EXPOSED SOILS, INCLUDING ROADWAY EMBANKMENTS, THAT WILL NOT BE
DISTURBED FOR TWO DAYS DURING THE WET SEASON OR SEVEN DAYS DURING THE DRY
SEASON SHALL BE IMMEDIATELY STABILIZED WITH THE APPROVED TESC METHODS (E.G.,
SEEDING, MULCHING, PLASTIC COVERING, ETC.).
8. ANY AREA NEEDING TESC MEASURES, NOT REQUIRING IMMEDIATE ATTENTION, SHALL BE
ADDRESSED WITHIN FIFTEEN (15) DAYS.
9. THE TESC FACILITIES ON INACTIVE SITES SHALL BE INSPECTED AND MAINTAINED A
MINIMUM OF ONCE A MONTH OR WITHIN THE 48 HOURS FOLLOWING A STORM EVENT.
10. AT NO TIME SHALL MORE THAN ONE (1) FOOT OF SEDIMENT BE ALLOWED TO
ACCUMULATE WITHIN A CATCH BASIN. ALL CATCH BASINS AND CONVEYANCE LINES SHALL BE
CLEANED PRIOR TO PAVING. THE CLEANING OPERATION SHALL NOT FLUSH
SEDIMENT -LADEN WATER INTO THE DOWNSTREAM SYSTEM.
11. STABILIZED CONSTRUCTION ENTRANCES AND ROADS SHALL BE INSTALLED AT THE
BEGINNING OF CONSTRUCTION AND MAINTAINED FOR THE DURATION OF THE PROJECT.
ADDITIONAL MEASURES, SUCH AS WASH PADS, MAY BE REQUIRED TO ENSURE THAT ALL
PAVED AREAS ARE KEPT CLEAN FOR THE DURATION OF THE PROJECT.
12. ANY PERMANENT RETENTION/DETENTION FACILITY USED AS A TEMPORARY SETTLING BASIN SHALL
BE MODIFIED WITH THE NECESSARY EROSION CONTROL MEASURES AND SHALL PROVIDE
ADEQUATE STORAGE CAPACITY. IF THE PERMANENT FACILITY IS TO FUNCTION ULTIMATELY AS AN
INFILTRATION SYSTEM, THE TEMPORARY FACILITY MUST BE GRADED SO THAT THE BOTTOM AND
SIDES ARE AT LEAST ONE FOOT ABOVE THE FINAL GRADE OF THE PERMANENT FACILITY.
13. WHERE STRAW MULCH FOR TEMPORARY EROSION CONTROL IS REQUIRED, IT SHALL BE
APPLIED AT A MINIMUM THICKNESS OF 2 INCHES.
SEEDING NOTES
1. SEED MIXTURE SHALL BE EROSION CONTROL SEED MIXTURE, AND SHALL BE APPLIED AT THE RATE OF 120
POUNDS PER ACRE.
2. SEED BEDS PLANTED BETWEEN MAY 1 AND OCTOBER 31 WILL REQUIRE IRRIGATION AND OTHER

THE STANDARD STRENGTH FILTER FABRIC SHALL BE STAPLED OR WIRED TO THE FENCE, AND 20 INCHES
OF THE FABRIC SHALL BE EXTENDED INTO THE TRENCH. THE FABRIC SHALL NOT EXTEND MORE THAN
36 INCHES ABOVE THE ORIGINAL GROUND SURFACE. FILTER FABRIC SHALL NOT BE STAPLED TO
