PERMIT CONDITIONS: 1: ***BUILDING PERMIT CONDITIONS*** 2: Work shall be installed in accordance with the approved construction documents, and any changes made during construction that are not in accordance with the approved construction documents shall be resubmitted for approval. 3: All permits, inspection record card and approved construction documents shall be kept at the site of work and shall be open to inspection by the Building Inspector until final inspection approval is granted. 4: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap the sanitary sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks, wells, and other excavations. Final inspection approval will be determined by the building inspector based on satisfactory completion of this requirement. 5: All plumbing and gas piping work shall be inspected and approved under a separate permit issued by the City of Tukwila Building Department (206-431-3670). 6: All electrical work shall be inspected and approved under a separate permit issued by the City of Tukwila Permit Center. 7: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any of the provisions of the building code or of any other ordinances of the City of Tukwila. Permits presuming to give authority to violate or cancel the provisions of the code or other ordinances of the City of Tukwila shall not be valid. Coordinate with the Public Works Inspector for any work after 5:00 p.m. and weekends. 13: No work under this permit during weekend hours without prior approval by Public Works. Coordinate with the Public Works Inspector. 14: Work affecting traffic flows shall be closely coordinated with the Public Works Inspector. Traffic Control Plans shall be submitted to the Inspector for prior approval. 15: Any material spilled onto any street shall be cleaned up immediately. 16: Temporary erosion control measures shall be implemented as the first order of business to prevent sedimentation off-site or into existing drainage facilities. 17: The site shall have permanent erosion control measures in place as soon as possible after final grading has been completed and prior to the Final Inspection. 18: From October 1 through April 30, cover any slopes and stockpiles that are 3H:1V or steeper and have a vertical rise of 10 feet or more and will be unworked for greater than 12 hours. During this time period, cover or mulch other disturbed areas, if they will be unworked more than 2 days. Covered material must be stockpiled on site at the beginning of this period. Inspect and maintain this stabilization weekly and immediately before, during and following storms. 19: From May 1 through September 30, inspect and maintain temporary erosion prevention and sediment at least monthly. LABS EXECUTIVE SUMMARY The Boeing Company (Boeing) has appointed NVL Laboratories, Inc. (NVL) to conduct a hazardous materials survey of Building 25-070 located at 15470 Nelson PI, Tukwila, WA 98188. The purpose of this hazardous materials survey is to identify asbestos containing building materials (ACM), paint coatings containing the Resource Recovery and Conservation Act list of eight heavy metals (RCRA-8 metals) and Polychlorinated Biphenyl (PCB) containing building components including paint and caulking. NVL collected four 1" concrete core samples from interior and exterior painted concrete surfaces to check for the presence of PCBs in concrete substrates. Additionally, a waste stream characterization for the building was conducted via Toxicity Characteristic Leachate Procedure (TCLP). PCB containing fluorescent light ballasts (FLB), and suspect mercury containing fluorescent light tubes/high intensity discharge lamps (FLT and HID) were inventoried. Boeing EHS reported that prior testing of the roof found it to be asbestos containing, therefore no samples of the torch down roofing were collected. Suspect asbestos containing materials that have been presumed to be asbestos in lieu of sampling are mentioned in the table below. Limited qualitative evaluation of the fire door material materials in the building identified no suspect asbestos containing materials. The building was not occupied at the time of the September 29, 2017 investigation. Destructive sampling methods were utilized to collect samples of suspect building materials. No soft/limited demolition was performed during this inspection. INTRODUCTION NVL Laboratories, Inc. (NVL) is pleased to present this report detailing the findings of the hazardous materials survey conducted at Building 25-070, a Boeing property located in Tukwila, WA. The hazardous materials survey was conducted at the request of Mr. Dustin Thompson of Boeing NORMM- EHS group. This survey included the assessment of hazardous materials for the interior and exterior of the structure. Any exterior buried materials and/or utilities associated with Building 25-070 were not included in the survey. This survey includes sampling and analysis of suspect asbestos containing building materials, suspect RCRA-8 containing paint coatings, and suspect PCB containing building materials including concrete substrate. A TCLP waste stream characterization analysis for RCRA 8 metals was performed in addition to an inventory for PCB -containing FLBs and mercury -containing light tubes/lamps. The purpose of the hazardous materials survey was to identify building materials, fixtures, and components that may be impacted by the planned demolition of Building 25-070. It should be noted that hidden materials may exist within the building and/or buried, and all suspect materials must be treated as hazardous until testing proves otherwise. Fuad Ayesh and Munaf Khan, both Industrial Hygienists, certified Asbestos Hazard Emergency Response Act (AHERA) Building Inspectors, and Washington State Department of Commerce (WSDOC) certified Lead -Based Paint Inspectors, conducted the hazardous materials survey at Building 25-070. Copies of certifications are provided in Appendix F. During the survey, NVL walked through and investigated the interior and of Building 25-070 and performed an inventory of all suspect ACM and paint coatings. Materials and locations for PCB sampling were identified by Boeing EHS in a sample inventory with specified unique sample numbers and reference photos. A copy of this PCB sample inventory with results is included in Appendix B. This survey satisfies the requirements for "Good Faith" inspection outlined in Washington Administrative Code (WAC) 296-62-0722(2), Identification, which requires the owner of a structure to provide contractors with a written report identifying the asbestos -containing materials expected to be disturbed during demolition. The building's net square footage is approximately 17,233 square feet. It was previously a mixed-use property as a storage facility and office space. It is currently unoccupied. HAZARDOUS MATERIALS SURVEY The hazardous materials survey was performed on September 29, 2017. The scope of work conducted to meet the objectives of the hazardous materials survey included the following: > Visual inspection of the all accessible interior and exterior building materials that would be impacted by the planned demolition of the structure for suspect ACM and RCRA8-containing paint. ➢ Collection and analysis of bulk asbestos samples to confirm if suspect building materials are asbestos containing. • Collection and analysis of paint samples to confirm if the associated painted surfaces contained RCRA-8 metals. > Collection and analysis of building material and paint coating samples to test for PCBs. ➢ Inventory of PCB -containing light ballasts and Mercury containing light tubes. • Collection and analysis of TCLP samples for waste stream characterization. > Compiling this hazardous material survey report. LABORATORY INFORMATION Laboratory Analysis: Asbestos In accordance with 40 CFR Chapter 1 (1-1-87 edition) Part 763, Subpart F, Appendix A, asbestos samples are analyzed at NVL Labs using polarized light microscopy (PLM) with dispersion staining. If samples are not homogeneous, then sub -samples of the components are analyzed separately. All bulk samples are analyzed using EPA Method 600/R-93/116 with the following measurement uncertainties for reported % asbestos: 1%=0-3%, 5%>_1-9%, 10%=5-15%, 20%=10-30%, 50%=40-60%. Only materials containing more than 1% total asbestos were classified as "asbestos -containing" based on EPA, state, and local regulations. Findings for samples containing more than one separable layer of materials are reported for each layer. The asbestos concentration in the sample is determined by visual estimation. NVL Labs is accredited by the National Institute of Standards and Technology (NIST) under the National Volunteer Laboratory Accreditation Program (NVLAP) program for bulk asbestos fiber analysis; NVLAP Lab Code 102063-0 Laboratory Analysis: Heavy Metals (RCRA-8) Samples are analyzed for the presence of heavy metals (RCRA-8) using Inductively Coupled Plasma (ICP) in accordance with method EPA 3051/6010C/7471 B. This method reports results in milligrams per kilogram (mg/kg) or its equivalent and parts per million (ppm). Building 25-070 NVL Report 2017-0764 Page 3 of 15 October 5, 2017 iNI. ABS Laboratory Analysis: Polychlorinated Biphenyls (PCBs) Laboratory analysis of these samples for the presence of PCB's was conducted by NVL Labs in accordance with EPA Method 8082. NVL Labs is a Washington State Department of Ecology and American Industrial Hygiene Association accredited laboratory certified to perform bulk PCB analysis. Results are reported in milligrams per kilogram (mg/kg), or the equivalent parts per million (ppm). Laboratory Accreditation Professional accreditations for NVL Laboratories, Inc. include the following: NVL Laboratories, Inc. is currently accredited by the National Institute of Standards and Technology (NIST) under the National Volunteer Laboratory Accreditation Program (NVLAP) program for bulk asbestos fiber analysis. The IHLAP program is designed specifically for laboratories involved in analyzing samples to evaluate workplace exposure. IHLAP Certification Number 563 ,ASBESTOS CONTAINING MATERIALS NVL performed an invasive survey for suspect ACM on Building 25-070. This survey was conducted in accordance with AHERA sampling guidelines. Procedures and Methodology The NVL Labs field inspector is an Asbestos Building Inspector, certified under the requirements of the United States Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA) regulation 40 CFR 763, Subpart E. A copy of his certificate is provided in Appendix F. The AHERA Guidelines dictate the following: The inspector must determine homogenous areas, which are defined as an area of Thermal System Insulation, Surfacing Material, or Miscellaneous Material that is uniform in texture and color. Once homogenous areas have been determined, the inspector must determine whether or not material is friable or non -friable. Friable is defined as a material, that when dry, can be crushed, pulverized, or reduced to dust using hand pressure, and non -friable material is defined as a material, that when dry, cannot be crushed pulverized or reduced to dust using hand pressure. Materials normally defined as non -friable can become friable by definition if sufficiently damaged. Once friability has been determined, the materials suspected of containing asbestos are divided into one of three categories: Thermal System Insulation (TSI), Surfacing Material (SM), or Miscellaneous Building 25-070 NVL Report 2017-0764 Page 4 of 15 October 5, 2017 INIVI�L� LABS Material (MM). Generally speaking, TSI and SM are considered to be friable, with the exception of TSI where the structural integrity of the insulation is intact and the protective out wrap is undamaged. Once materials are divided into one of the categories, samples are collected in the following manner: Friable Thermal System Insulation: 1. Inspector shall collect three (3) randomly distributed samples; 2. Inspector shall collect a minimum of one sample of each TSI materials that appears to have been used as a patch, as long as the patch is less than 6 linear feet or 6 square feet; 3. Inspector shall collect in a manner sufficient, samples from areas of TSI applied to fittings, tees, and joints. Friable Surfacing Material: 1. Inspector shall collect samples in random manner of surfacing materials as follows: a. Collect three bulk samples from an area believed to be homogeneous (defined as a. material that appears to be the same or similar and was installed at the same time) that is 1,000 square feet or less in size; b. Collect five bulk samples from an area believed to be homogeneous that is greater. than 1,000 square feet in size, but less than 5,000 square feet in size; Collect seven bulk samples from an area believed to be homogeneous that is greater than 5,000 square feet. Miscellaneous Materials: 1. Inspector shall collect samples in a manner and number sufficient to determine if the material is asbestos -containing or not. All Materials Determined to Be Non Friable: 1. Inspector shall collect samples in a manner and number sufficient to determine if the material is asbestos containing or not. In addition to these sampling requirements, the AHERA Building Inspector is required to assess the following of each material that is found to be positive for asbestos: 1. The condition of each material; 2. Accessibility; 3. Possibility for air erosion. Once the samples have been collected, they must be analyzed by an accredited laboratory, and they must be analyzed using polarized light microscopy methods, commonly referred to as EPA Method 600/R-93/116. NVL Labs collected samples and obtained analytical data for suspect asbestos -containing materials identified in the building. Once collected, each bulk sample was sealed in an unadulterated plastic bag to eliminate the possibility of cross -contamination. "Chain -of -Custody" tracking was followed to maintain sample integrity during handling and data reporting at NVL Labs. The walk-through inspection included a review of the internal and external aspects of the structures. The locations and types of potential asbestos-containing materials were noted. Homogeneous Materials Homogeneous materials are defined as an area of asbestos -containing material or presumed asbestos - containing material which appears similar throughout in terms of color, texture, and date of material application. RESULTS- ASBESTOS LAB ANALYSIS NVL collected a total of 42 samples of suspect ACM from the areas to be impacted by the scope of work of Building 25-070. An inventory of the samples is provided in Appendix B. Laboratory analytical reports are provided in Appendix C. LABS RCRA-8 PAINT COATINGS NVL performed a survey of suspect RCRA-8 interior and exterior paint coatings of Building 25-070. Procedures and Methodology NVL Labs collected representative samples of paint from the interior and exterior of the building. Once collected, each bulk sample was sealed in an unadulterated plastic bag to eliminate the possibility of cross-contamination.'"Chain-of-Custody" tracking was followed to maintain sample integrity during handling and data reporting at NVL Labs. Sampling was representative of all layers of paint. Copies of laboratory reports and field data forms for RCRA-8 metals are in Appendix C. RESULTS- RCRA-8 PAINT LABORATORY ANALYSIS Six paint chip samples were collected from representative interior and exterior painted surfaces on Building 25-070. All six of the samples were identified as being heavy metals containing. Building 25-070 NVL Report 2017-0764 Page 7 of 15 October 5, 2017 LABS POLYCHLORINATED BYPHENYLS (PCBs) NVL performed sampling for suspect PCB containing paint and caulking on Building 25-070. Bulk samples and 1" core samples were also collected. Materials and locations for PCB sampling were identified by Boeing EHS in a sample inventory with specified unique sample numbers and reference photos. A copy of this PCB sample. inventory is included in Appendix B. Procedures and Methodology Bulk sampling for suspect polychlorinated biphenyl (PCB) containing paint coatings impacted by the planned demolition was also performed at the subject structure. Sampling was performed to meet the requirements of the US EPA Toxic Substances Control Act of 1976 (TSCA, 40 CFR 761.61), which requires representative bulk sampling of suspect PCB containing materials. The TSCA regulatory limit for solid PCB Bulk Product Waste is 50 parts per million (PPM). Disposal of bulk solid building materials with 50 PPM or greater total PCB content is regulated under TSCA. However, any work (including demolition) coming into contact with PCB -containing materials requires worker protections and a negative exposure assessment to assure safety and regulatory compliance. Sampling was performed in accordance with the EPA PCB Bulk Sample Collection Procedure for Bulk Solid Samples and the 2011 EPA Standard Operating Procedure (SOP) for Sampling Porous Surfaces for Polychlorinated Biphenyls. For most material samples, metal hand tools were used to remove bulk product materials and place them in labeled, sealed containers to eliminate thepossibility of cross - contamination. For core samples, a roto hammer with 1" bit was used to remove and capture a 1" deep section of the concrete material. Chain -of -Custody" protocols was followed to maintain sample integrity during transportation/handling and data reporting at NVL Labs. The tools utilized to collect samples were cleaned between collections of each unique sample. As per TSCA requirements for representative sampling described in section 13.1 of the 2011 EPA SOP, at least one sample was collected of each unique homogeneous material to be impacted during the planned scope of work. Duplicate samples were collected in accordance with section 13.4 of the EPA SOP. Copies of laboratory reports and field data forms for PCBs are in Appendix C. TOXIC CHARACTERISTICS LEACHATE PROCUDURE (TCLP) RCRA-8 NVL conducted total waste stream sampling in accordance the requirements of Washington Administrative Code (WAC) 173-303-090 "Dangerous Waste Characteristics". The purpose of this section is to set forth characteristics which a solid waste might exhibit and which would cause the waste to be characterized as "dangerous waste". Procedures and Methodology A representative composite sample of the proportionate components which make up the subject structure was collected and analyzed according to ASTM Standard E 1908-97, as suggested by the Washington State Department of Ecology. Waste Characterization Plan number three of this standard, "Composite Sample and Demolish", was used to access the RCRA-8 metals content of the total waste stream of the building RESULTS- TCLP RCRA-8 ANALYSIS Detectable levels of Barium were found below the regulatory threshold of 100 PPM. No other detectable levels of RCRA-8 metals were seen. An inventory of the samples is provided in Appendix B. Laboratory analytical reports are provided in Appendix C. PCB CONTAINING BALLASTS NVL conducted a limited visual inspection of Building 25-070 to locate, identify, and quantify PCB containing FLBs. Procedures and Methodology According to the EPA, FLBs manufactured between July 1, 1979 and July 1, 1998 that do not contain PCBs must be labeled "No PCBs", otherwise they are presumed to contain PCBs The "No PCB" label also means that there are less than 50 ppm PCBs, however the Washington State Department of Ecology (hereinafter referred to as WA -DOE) regulates PCBs at just 2 ppm. NVL selected a representative number of each type of fixture in use to inspect. The EPA recommends to wear proper protective clothing, ventilate area, and cover the floor with plastic sheeting to prevent exposure if leaking FLBs are discovered. In order to inspect FLBs, light fixtures are required to be opened since they are contained within them. FINDINGS- PCB LIGHT BALLAST INVENTORY NVL identified three different types of light fixtures in the subject structure. All were seen to have "No PCB labels" on the ballasts. Procedures and Methodology FLT, HID lamps, and thermostats may contain mercury. They are classified as universal waste by the EPA and Washington Department of Ecology. Recommendations have been included for compliance with WAC 173-303-573, "The Universal Waste Rule for Dangerous Waste". RESULTS —MERCURY CONTAINING TUBE, LAMP, THERMOSTAT INVENTORY NVL identified different three types of light fixtures (All had a No PCB sticker on the ballast): NVL identified three different types of light fixtures in the subject structure. All were seen to have "No PCB labels" on the ballasts. Table 1.2 below provides in inventory of the fluorescent tubes and lamps in the building. These include 1 hard sink, 11 electrical panels/switches, 2 crane brakes, and vapor barrier below the concrete slab. Additionally, NVL identified 5 types of ACM during the survey. These include 18 roof penetrations/patches, 400 linear feet of caulking between concrete sections, 31 cement asbestos board (CAB) panels, 7 windows with caulking around the exterior, and caulking around the 28 panels of CAB. Regulatory Requirements for Materials Containing Less Than 1% Asbestos There is an unknown quantity of asbestos containing joint compound associated with the interior wall/ceiling system of the building. However, this material is part of the composite wall system, and is considered to be less than 1% asbestos containing for demolition purposes only. • If less than 1% asbestos is found in asbestos containing building material (ACBM), the EPA and Local Clean Air Agencies (LCAAs) do not regulate it as "asbestos containing material", as per 40 CFR Part 763.83 and WRD 23.30. • Abatement of <1% asbestos is considered "non -classified" work by Washington State Department of Labor and Industries covered in WAC 296-62-07712 (2, 4, & 5), WAC 296-62-07725 (5) and WAC 296-62-07728. • Competent Person (with no set training requirements, however the Competent Person must have appropriate knowledge and authority to take necessary action to ensure safe work place) to conduct an exposure assessment/negative exposure assessment (NEA), as per WAC 296-62-07709-3-b. • Wet Methods (WAC 296-62-07712-2) • HEPA Vacuum (WAC 296-62-07712-2) • Prompt clean up and disposal (WAC 296-62-07712-2) • Protective clothing and equipment is required if NEA is not performed (WAC 296-62-07717) • Respiratory Protection (WAC 296-62-07715 and WAC 296-842) Planning and coordination of ACM & PACM removal should begin prior to demolition activities. The ACM & PACM identified in this report should be removed by certified, trained, and protected personnel using appropriate work practices and engineering controls prior to disturbance by demolition, as outlined in WAC 296-2,1-1. Any additional, previously un -sampled, suspect materials encountered during demolition activities should be sampled by a certified AHERA Building Inspector and analyzed by a NVLAP-accredited laboratory to evaluate asbestos content prior to disturbing the material. Building 25-070 NVL Report 2017-0764 Page 12 of 15 October 5, 2017 L ABS.:. Paint Chips - RCRA-8 Metals Detectable levels of heavy metals were identified in all the paint samples that were collected from Building 25-070 in samples: 2017 -0764 -MBP -1 through 2017 -0764 -MBP -6. Boeing had previously requested NVL to collect representative dust wipe samples on interior horizontal surfaces. It has been well documented that many Boeing buildings have heavy metals contaminated settled dust. Therefore, NVL is presuming that settled dust on interior horizontal surfaces are contaminated with heavy metals. NVL is also presuming one vent pipe on the roof to be lead- containing. TCLP RCRA-8 Metals (Toxicity Characteristic Leachate Procedure) Detectable levels of Barium were detected in the TCLP sample collected from building 25-070. No other detectable levels of any other RCRA-8 metals were detected. Barium levels were below the threshold of 100.0 ppm for Barium. Therefore, the solid waste stream of the demolition debris (with respective components composition) is to be considered as general construction debris/waste. Prior to disposing building debris, components of the building should be collected and analyzed for RCRA-8 metals using a toxicity characteristic leaching procedure (TCLP) to characterize the waste for disposal. If the collected demolition waste stream samples with leachable concentrations exceeding the regulatory threshold for any RCRA metal, then those materials are considered hazardous and require special handling according to federal and state regulations, per WAC 173-303-090. Worker exposure responsibilities are applicable for any construction work that impacts the above mentioned paint samples, as per WAC 296-841-100 & WAC 296-841-20005. PCB Building Materials During this survey, NVL identified 10 building materials associated with Building 25-070 which were found to be PCB -containing (detectable level so PCB's but less than 50 PPM). 12 building materials were identified with levels of PCBs above the TSCA threshold of 50 PPM of PCBs. According to 40 CFR 761.62 of TSCA, all PCB waste with concentrations above the 50 ppm threshold must be labeled, manifested, transported, and disposed of according to federal and state regulations. This material must be properly disposed of in accordance with federal, state, and local regulations. It should also be noted that the proper visual clearance testing procedure following PCB paint removal from metal surfaces is required and regulated by the Code of Federal Regulations Section 761.79 (Title 40, Chapter, I, Subchapter, R, Part 761, Subpart D). PCB Light Ballasts A total of 214 ballasts presumed to contain PCBs were identified in Building 25-070. "No PCBs" sticker was seen on all the FLB that were inspected during the hazardous materials survey. During demolition activities, any light ballast without "No PCBs" labeling should be handled and disposed of according to state and federal regulations. According to 40 CFR 761, all PCB waste must be labeled, manifested, transported, and disposed of according to federal and state regulations. Building 25-070 NVL Report 2017-0764 Page 13 of 15 October 5, 2017 LABS The WA -DOE regulates PCBs at 2 ppm and the requirement for a "No PCBs" label is 50 ppm. As a result, NVL is assuming that all FLB marked "No PCBs" are PCB containing and must be handled in accordance with 40 CFR Part 761 and WAC Chapter 173-303. Mercury Light Tubes A total of 378 light tubes presumed to contain mercury vapors were identified in Building 25-070. No suspect mercury containing thermostats were identified. The thermostats, FLT, and HID light tubes identified in this report are assumed to contain mercury vapors. They should be removed without breakage and disposed of properly in accordance with WAC 173-303-573. Building 25-070 NVL Report 2017-0764 Page 14 of 15 October 5, 2017 L ABS LIMITATIONS This survey report does not cover other hazardous building materials that may be present; such as silica in concrete and gypsum, mercury containing composite walkway materials, and any other materials outside the scope of work as identified in this report. The purpose of this hazardous materials survey report is to document asbestos containing building materials, RCRA-8 paint coatings, PCB containing building materials, and PCB/Hg containing FLT & FLB which would be impacted by the planned demolition of Building 25-070. Destructive sampling methods were utilized to collect samples of suspect building materials. No soft/limited demolition was performed during this survey. Hidden materials may exist within this structure, and all suspect materials must be treated as hazardous until testing proves otherwise. This site visit consisted of a thorough visual walk-through of the building for the purpose of viewing and sampling potential asbestos -containing material. As hazardous material surveys are non- comprehensive by nature, NVL Laboratories, lhc. cannot be held liable for materials which require destructive means to access, materials which are hidden from sight (e.g. materials hidden behind walls), materials which cannot be found due to their obscure nature, or which otherwise cannot be discovered with reasonable diligence. As used herein, "Hazardous Materials" means and includes any substance, material, waste, pollutant, or contaminant listed or defined as hazardous or toxic, under any Environmental Requirements, asbestos and petroleum, including crude oil or any fraction thereof, natural gas liquids, liquefied natural gas, or synthetic gas usable for fuel (or mixtures of natural gas and such synthetic gas); and "Environmental Requirements" means and includes all applicable present and future statutes, regulations, ordinances, rules, codes, judgments, orders or other similar enactments of any governmental authority or agency regulating or relating to health, safety, or environmental conditions on, under, or about the Property or the environment, including, without limitation, the following: the Comprehensive Environmental Response, Compensation and Liability Act; the Resource Conservation and Recovery Act; the Model Toxics Control Act, and any regulations or policies promulgated or issued thereunder. c.
