PERMIT CONDITIONS:

2: Work shall be installed in accordance with the approved construction documents, and any changes made
during construction that are not in accordance with the approved construction documents shall be
resubmitted for approval.
3: All permits, inspection record card and approved construction documents shall be kept at the site of work
and shall be open to inspection by the Building Inspector until final inspection approval is granted.
4: All construction shall be done in conformance with the Washington State Building Code and the
Washington State Energy Code.
5: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap
the sanitary sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks,
wells, and other excavations. Final inspection approval will be determined by the building inspector based
on satisfactory completion of this requirement.
6: Prior to final inspection for this building permit, a copy of the roof membrane manufacturer's warranty
certificate shall be provided to the building inspector.
7: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an
approval of, any violation of any of the provisions of the building code or of any other ordinances of the City
of Tukwila. Permits presuming to give authority to violate or cancel the provisions of the code or other
ordinances of the City of Tukwila shall not be valid. The issuance of a permit based on construction
documents and other data shall not prevent the Building Official from requiring the correction of errors in
the construction documents and other data.

Carlisle Roofing Systems, Inc., (Carlisle) warrants to the Building Owner (Owner) of the above described building, that; subject to the terms,
conditions, and limitations stated in this warranty, Carlisle will repair any leak in the Carlisle Golden Seal"Total Roofing System (Carlisle Total
Roofing System) installed by a Carlisle Authorized Roofing applicator for a period of 20 years commencing with the date of Carlisle's acceptance
of the Carlisle Total Roofing System installation. However, in no event shall Carlisle's obligations extend beyond 20.5 years subsequent to the date
of substantial completion of the Carlisle Total Roofing System. See below for exact date of warranty expiration.
The Carlisle Total Roofing System is defined as the following Carlisle brand materials: Membrane, Flashings, Adhesives and Sealants, Insulation,
Cover Boards, Fasteners, Fastener Plates, Fastening Bars, Metal Work, Insulation Adhesives, and any other Carlisle brand products utilized in this
installation.
TERMS, CONDITIONS, LIMITATIONS
1. Owner shall provide Carlisle with written notice via letter, fax or email within thirty (30) days of the discovery of any leak in the Carlisle Total Roofing System. Owner should send written notice of a leak to
Carlisle's Warranty Services Department at the address set forth at the bottom of this warranty. By so notifying Carlisle, the Owner authorizes Carlisle or its designee to investigate the cause of the leak.
Should the investigation reveal the cause of the leak to be outside the scope of this Warranty, investigation and repair costs for this service shall be paid by the Owner.
2. If, upon inspection, Carlisle determines that the leak is caused by a defect in the Carlisle Total Roofing System's materials, or workmanship of the Cad'sle Authorized Roofing Applicator in installing the
same, Owner's remedies and Carlisle's liability shall be limited to Carlisle's repair of the leak.
3. This warranty shall not be applicable if, upon Carlisle's inspection, Carlisle determines that any of the following has occurred:
(a) The Carlisle Total Roofing System is damaged by natural disasters, including, but not limited to, lightning, fire, insect infestations, earthquake, tomado, hail, hurricanes, and winds of (3 second)
peak gust speeds of fifty-five mph or higher measured at 10 meters above ground; or
(b) Loss of integrity of the building envelope and, or structure including, but not limited to partial or complete loss of roof deddng, wall siding, windows, doors or other envelope components or from roof
damage by wind-blown objects, or:
(c) The Carlisle Total Roofing System is damaged by any intentional or negligent acts, accidents, misuse, abuse, vandalism, civil disobedience, or the like.
(d) Deterioration or failure of building components, including, but not limited to, the roof substrate, walls, mortar, HVAC units, non -Carlisle brand metal work, etc., occurs and causes a leak, or
otherwise damages the Carlisle Total Roofing System; or
(e) Acids, oils, harmful chemicals and the like come in contact with the Carlisle Total Roofing System and cause a leak, or otherwise damage the Carlisle Total Roofing System.
(f) The Carlisle Total Roofing System encounters leaks -oris otherwise damaged by condensation resultingfrom any condition-within.the building that may generate.moisture.
4. This Warranty shall be null and void if any of the following shall occur:
(a) If, after installation of the Carlisle Total Roofing System by a Carlisle Authorized Roofing Applicator there are any alterations or repairs made on or through the roof or objects such as, but not
limited to, structures, fixtures, solar panels, wind turbines, roof gardens or utilities are placed upon or, attached to the roof without first obtaining written authorization from Carlisle; or
(b) Failure by the Owner to use reasonable care in maintaining the roof, said rtieintegance to indude, but not be limited to, those items listed on Carlisle's Care 8 Maintenance Information sheet which
accompanies this Warranty ; " J
5. Only Carlisle isle brand insulation products are covered bylhis warranty. Carlislelspecifically disdaims liability, under any theory of law, for amages sustained by or caused by non-Cartisle brand insulation
products.

6. During the term of this Warranty, Carlisle shall have -free access to the roof during regular business hour's. ✓ - )
7. Carlisle shall have no obligation underthis-Warranty while any bilis for' installation, supplies;-service;-and,warranty charges have not been paid tri full to the Carlisle Authorized Roofing Applicator,
Carlisle, or material suppliers.
8. Carlisle's failure at any time to enforce any of the terms or conditions stated herein shall not be construed to be a waiver of such provision. - -
9. Carlisle shall not be responsible for the cleanliness or discoloration of the Carlisle Total Roofing System caused by environmental conditions including, but not limited to, dirt, pollutants, or biological
agents.
10. Carlisle shall have no liability under any theory of law for any daims, repairs, restoration, or other damages including, but not limited to, consequential or incidental damages relating, directly or indirectly,
to the presence of any irritants, contaminants, vapors, fumes, molds, fungi, bacteria, spores, mycotoxins, or the like in the building or in the air, land, or water serving the building.
11. This warranty shall be transferable upon a change in ownership of the building when the owner has completed certain procedures including a transfer fee and an inspection of the Roofing System by a
Carlisle representative.
CARLISLE DOES NOT WARRANT PRODUCTS UTILIZED IN THIS INSTALLATION WHICH IT HAS NOT FURNISHED; AND SPECIFICALLY DISCLAIMS LIABILITY,
UNDER ANY THEORY OF LAW, ARISING OUT OF THE INSTALLATION AND PERFORMANCE OF, OR DAMAGES SUSTAINED BY OR CAUSED BY, PRODUCTS
NOT FURNISHED BY CARLISLE OR THE PRIOR EXISTING ROOFING MATERIAL OVER WHICH THE CARLISLE ROOFING SYSTEM HAS BEEN INSTALLED
THE REMEDIES STATED HEREIN ARE THE SOLE AND EXCLUSIVE REMEDIES FOR FAILURE OF THE CARLISLE TOTAL ROOFING SYSTEM OR ITS
COMPONENTS. THERE ARE NO WARRANTIES EITHER EXPRESSED OR IMPLIED, INCLUDING THE IMPLIED WARRANTIES OF FITNESS FOR A PARTICULAR
PURPOSE AND MERCHANTABILITY, WHICH EXTEND BEYOND THE FACE HEREOF. CARLISLE SHALL NOT BE LIABLE FOR ANY INCIDENTAL,

Reinforced Membrane
Overview
Carlisle's Sure -Weld TPO reinforced membrane is a premium, heat -
weldable, single -ply thermoplastic polyolefin (TP0) sheet designed for new
roof construction and re -roofing applications. Sure -Weld High Slope (HS)
membrane is formulated with additional flame retardant for higher -slope
fire code approvals. Sure -Weld EXTRA is 80 mils thick for significantly
higher strength and weatherability.
Sure -Weld TPO membranes use advanced polymerization technology
that combines the flexibility of ethylene -propylene (EP) rubber with the
heat weldability of polypropylene. All Sure -Weld TPO membranes include
OctaGuard XT' an industry-leading, state-of-the-art weathering package.
OctaGuard XT technology enables Sure -Weld TP0 to withstand the extreme
weatherability testing that is intended to simulate exposure to severe
climates.
Physical properties of the membrane are enhanced by a strong polyester
fabric that is encapsulated between the TPO-based top and bottom plies.
The combination of the fabric and TPO plies provides high breaking and
tearing strength, as well as excellent puncture resistance. The relatively
smooth surface of the membrane produces a total surface fusion weld
that results in a consistent, watertight. monolithic roof assembly. The
membrane is environmentally friendly and safe to install.
Carlisle's standard and HS TPO membranes are available in highly reflective
white, tan, and gray, in both 45 -mil and 60 -mil thicknesses. 80 -mil Sure -
Weld EXTRA (including HS) is also offered in white, gray, and tan colors.
Sixteen special colors are also available (see Carlisle's TPO Color Palette
brochure). Carlisle's TPO is offered in 4-, and 6 -ft perimeter sheets and
8-, 10-. and 12 -ft Sure -Weld sheets. Sure -Weld HS and special color TP
membranes are available in limited sizes.
Carlisle's tan and white TPO membranes are ENERGY STAR'S" -qualified and
California Title 24 compliant and can contribute toward LEED` (Leadership
in Energy and Environmental Design) credits.
Optional APEEL'" Protective Film
Carlisle's Sure -Weld TPO reinforced membrane is available with an optional
APEEL Protective Film, saving time and labor by eliminating the need
for roof cleaning upon project completion. Carlisle's innovative APEEL
Protective Film can be left in place for up to 90 days without affecting the
integrity of the film, guarding the TPO membrane's surface from scuffs and
dirt accumulation during installation. Durable and easy to remove, APEEL
Protective Film improves aesthetics and long-term reflectivity and is ideal
for re -roofing, re-cover, and new construction projects.

Reinforced Membrane
Installation
1. Sure -Weld TPO roofing systems are quick to install, as minimal labor
and few components are required. TPO systems are installed using an
Automatic Heat Welder, making sheet welding fast, clean, consistent,
and easy to learn, while reducing strain on the roofing technician.
2. APEEL Protective Film should be removed from within areas that
are to be heat -welded together. In areas that do not require heat -
welding, the APEEL Protective Film can be left in place for up to
90 days. When the installation of the entire TPO roofing system is
complete, remove and discard the APEEL Protective Film.
3. The Carlisle Mechanically Fastened Roof System installation
starts by fastening the insulation with a minimum of 4 fasteners per
4' by 8' board. The membrane is mechanically fastened to the deck
using HP -X'° Fasteners and Piranha Plates" or HP-XTRA Fasteners
and Piranha XTRA Plates. Adjoining sheets of membrane are
overlapped over the fasteners and plates and joined together with a
minimum 1W -wide (4 cm) hot-air weld.

4. The Carlisle Fully Adhered Roofing System installation begins by
fastening the insulation at the required density necessary to meet
the appropriate warranty or wind load requirement. The substrate
and membrane are then coated with an appropriate Sure -Weld TPO
bonding adhesive and the membrane is rolled into place. •

Sunglasses that filter out ultraviolet light are strongly recommended, as
tan and white surfaces are highly reflective. Roofing technicians should
dress appropriately and wear sunscreen.
Surfaces may become slippery due to frost and ice buildup. Exercise
caution during cold conditions to prevent falls.
• Care must be exercised when working close to a roof edge when
the surrounding area is snow-covered, as the roof edge may not be
clearly visible.

Use proper stacking procedures to ensure sufficient stability of the rolls.
Exercise caution when walking on wet membrane. Membranes may be
slippery when wet.
Store membrane in the original undisturbed plastic wrap in a cool,
shaded area and cover with light-colored, breathable, waterproof
tarpaulins. Membrane that has been exposed to the weather must be
prepared with Weathered Membrane Cleaner prior to hot-air welding.
Take care not to stand or place heavy objects on the edge of folded -
over membrane, as this could cause a hard crease in the membrane.
Maximum sustained temperature not to exceed 160°F (71°C) for TPO
membrane.
» Do not use razor blades or other sharp tools to cut the APEEL Protective
Film while it is still adhered to the TPO membrane as damage to the
underlying membrane may occure. Pull the protective film away from
the membrane prior to cutting.
Remove APEEL Protective Film by pulling towards the center of the
roof. Do not remove the film by pulling towards the roof edge.
» A static electric charge may develop when removing APEEL Protective
Film from the surface of the membrane sheet. To avoid the possibility
of ignition, lids must be closed on any flammable products and a fire
extinguisher should be readily available.
Color membranes will 'fade' over time mainly due to the ultraviolet
portion of sunlight. Since most roof surfaces are exposed to variable
sunlight, some areas will be more susceptible to color changes caused by
UV fading. Warranties for color membranes do not cover fading of colors.
EXTREME Testing for Severe Climates
ASTM Standard D6878 is the material specification for Thermoplastic
Polyolefin -Based Sheet Roofing. It covers material property requirements
for TPO roof sheeting and includes initial and aged properties after heat
and xenon -arc exposure. As stated in the scope of the standard, "the tests
and property limits used to characterize the sheet are values intended to
ensure minimum quality for the intended purpose." Carlisle's goal is to
produce TPO that delivers maximum performance for the intended purpose
of roofing membranes. Maximum performance requires the membrane to
far exceed the requirements of ASTM D6878.
Heat Aging accelerates the oxidation rate that roughly doubles for each
18°F (10°C) increase in roof membrane temperature. Oxidation (reaction
with oxygen) is one of the primary chemical degradation mechanisms of
roofing materials.

Solar Reflectance Index (SRI) is calculated per ASTM 01980. The SRI is a measure of the
roof's ability to reject solar heat, as shown by a small temperature rise. It is defined so that
a standard black (reflectance 0.05. emittance 0.90) is 0 and a standard white (reflectance
0.80, emittance 0.90) is 100. Materials with the highest SRI values are the coolest choices
for roofing. Due to the way SRI is defined. particularly tot materials can even take slightly
negative values and particularly cool materials can even exceed 100.
'ENERGY STAR recommends that using the Roof Savings Calculator (rsc.ornl.gov). which

end joints staggered so they are offset by a minimum of 12" from the end
joints in adjacent rows. R -Tech Fanfold Recover Board should abut tightly
against adjacent boards. Joints greater than ?!" should be filled with the
same insulation that is being used in the field of the roof. If R -Tech Fanfold
Recover Board is being installed over an existing layer of insulation; all
joints must be offset a minimum of 6" between layers.

Set up site specific safety per WSLNI and OSHA requirements.
Remove existing cap metal and recycle at side walls. Newer cap metal on the front will be reused.
Install one layer of 1/2" EPS Fanfold cover board Carlisle HP -X screws with Rhinobond Plates per manufactures specifications for a
modified fully adhered system.
Install TPO clad starter metal at gutter edge.
Install Carlisle 045 TPO single ply membrane per manufactures specifications.(GREY) Heat weld seams. Probe all work when cool.
Run Carlisle 045 TPO single ply membrane per manufactures specifications up and over top of walls on sides and where old cap metal
is located. Terminate with term bar at intersection with newer TPO roof. Fully adhere with Carlisle Low VOC adhesive.
Install Carlisle TPO pipe boots. Caulk and clamp per specifications.
Run TPO up under existing skirt of HVAC units. We recommend that the property manager retain their HVAC contractor to work with us
and provide a tune up and possible repairs of units. This is not included in the base bid.
Electrical Conduits will be sealed by either TPO field wraps and clamped or by TPO pitch pockets and pourable sealant. Any satellite
equipment or penetrations which are in the way of new roof installment may need to be relocated or re -calibrated -not included in base
bid.

1. Relationship of Parties.
The statement of work on the front page together with these Terms and Conditions make up the Contract between Fields Roof
Service, inc. and the property owner or client designated on the front page ("Client"). This contractual relationship is an independent
contractor relationship and not a partnership, joint venture, employee -employer or other type of relationship.
2. Payment.
Payment is due immediately upon completion of the job. A down -payment may be required to secure material and equipment prior to
project initiation. If the work requires more than 30 days to complete, then FRS shall invoice client on monthly basis for work performed
during the prior month and payment shall be due upon receipt of invoice. Payments not received within 30 days of their due date shall
incur interest at 1.5% per month until paid. Any costs incurred by FRS to collect delinquent accounts, including reasonable attorneys
fees, court costs, lien costs and other collection related expenses shall be recoverable from the Client addition to the deliquent payment.
3. Limited Warranty.
FRS warrants that the installation of the roof will be performed in a workmanlike manner consistent with the standard of care of
the roofing industry. For a period of one (I) year [or period of time stated on proposal] following the installation or other work involving
the roof, FRS will, at no cost to the client, repair or replace any roof failure caused by a defective installation. In addition, FRS will provide
Client with any manufacturers' warranites that might apply to the roofing materials that are installed, provided that Client understands
that it must enforce these manufacturers warranties directly against the manufacturer; FRS will assist Client in this process.
4. Warranty Exclusions.
The limited warrany above explicity excludes damages caused by or due to (i) pre-existing conditions, such as defective roof support
systems and defective construction materials; (ii) improper building or residential construction (iii) mold, dry rot, mildew, or other
conditions caused by moisture an dimproper ventilation; (iv) termites and pests; and (v) hazardous substances and materials.
5. Client's Responsibilities.
Client shall be responsible for (i) providing an area for the storage of equipment and materials; (ii) obtaining permission from neighboring
landownwers if FRS's work requires the temporary use of such neighboring landowner's property; and (iii) correcting any existing defects,
such as dry rol and structural and code defects. (FRS may agree to correct these defects pursuant to a Change Order as provided below).
6. Change Orders.
Should the Client request any modifications to the work, or if FRS agrees to repair any existing defective conditions that are discovered
during construction, such as dry rot, code, or structural defects, this work shall be in addition to the statement of work on the front page
of this Contract and shall be documented in a Change Order signed by both parties.
7. Title to Materials.
Title to the roofing materials shall pass to the Client upon installation. Any surplus roofing materials that are not installed or used remain the property
of FRS and are not subject to any credit or refund in the contract price.
8. Clean Up.
Upon completion of the project, FRS will remove any surplus materials and debris and shall leave the exterior of the site in neat, broom
clean appearance. Any materials that fall from the ceiling within the building, including cathedral ceilings and any other materials that fall
within the confines of the interior of the building are the responsibility of the Client.
9. Existing Conditions.
FRS is not responsible for conditions existing before coming on to the site, including (i) ceiling damage due to the installation of the roof
or any repair of dry rot or other defective materials; (ii) existing chimney problems due to poor flashing or waterproofing and (iii) any
damage to driveways or pavement subsidence due to the vehicles required by FRS to transport an dinstall the roofing materials.
10. License, Insurance and Bonding.
FRS is properly licensed as a roofing contractor in the State of Washington (Lic # FEILDRS262L1), and is bonded and insured as
required by law.
11. Limitation of Remedies.
Client understands that its sole remedy for any defective work performed by FRS shall be enforcement of the warranty in parapgraph 3.
In addition, Client agrees that FRS's maximum aggregate liability to Client for any and all claims relating to FRS's work, including
claims for negligence and breach of contract shall be limited to the total amount of compensation paid to FRS under this Contract.
If Client desires an extended warranty or higher limitation of liability, FRS may agree to such for additional consideration paid to FRS,
and provided that such extended warranty or increased limitation of liability is documented in a Change Order signed by both parties.
