II. DECISION
SEPA Determination: The City SEPA Responsible Official has previously determined that this application
does not require a SEPA threshold determination because it is categorically exempt.
Decision on Substantive Permit: The City Community Development Department has determined that the
application for a Type 1 Wireless Communications Facility does comply with applicable City and state code
requirements and has approved that application without conditions.

III. YOUR APPEAL RIGHTS
The Decision on this Permit Application is a Type 1 decision pursuant to Tukwila Municipal Code
§ 18.104.010. Other land use applications related to this project may still be pending.
No administrative appeal of a DNS or an EIS is permitted. One administrative appeal to the City Hearing
Examiner of the Decision on the Permit itself is permitted. If an MDNS was issued, any person wishing to
challenge either the conditions which were imposed by the MDNS decision or the failure of the Department
to impose additional conditions in the MDNS must raise such issues as part of the appeal to the Hearing
Examiner.
A party who is not satisfied with the outcome of the Hearing Examiner appeal process may file an appeal in
King County Superior Court from the Hearing Examiner's decision.
IV. PROCEDURES AND TIME FOR APPEALING
In order to appeal the Community Development Director decision on the Permit Application, a written
notice of appeal must be filed with the Department of Community Development within 14 days of the
issuance of this decision, that is by April 29, 2014.
The requirements for such appeals are set forth in Tukwila Municipal Code ch. 18.116. All appeal
materials shall be submitted to the Department of Community Development. Appeal materials MUST
include:
1. The name of the appealing party.
2. The address and phone number of the appealing party; and if the appealing party is a corporation,
association or other group, the address and phone number of a contact person authorized to receive
notices on the appealing party's behalf.
3. A statement identifying the decision being appealed and the alleged errors in the decision, including
any specific challenge to an MDNS.
4. The Notice of Appeal shall identify (a) the specific errors of fact or errors in application of the law in
the decision being appealed; (b) the harm suffered or anticipated by the appellant, and (c) the relief
sought. The scope of an appeal shall be limited to matters or issues raised in the Notice of Appeal.
5. Appeal fee of $541 plus retainer.
V. APPEAL HEARINGS PROCESS
Any administrative appeal regarding the Permit shall be conducted as a closed record hearing before the
Hearing Examiner based on the information presented to the Community Development Director, who
made the original decision. No new evidence or testimony will be permitted during the appeal hearing.
Parties will be allowed to present oral argument based on the information presented to the Community
Development Director before their decision was issued. The Hearing Examiner's decision on the appeal is
the City's final decision.

Any party wishing to challenge the Hearing Examiner's decision on this application must file an appeal
pursuant to the procedures and time limitations set forth in RCW ch. 36.70C. An appeal challenging a
DNS, an MDNS or an EIS may be included in such an appeal. If no appeal of the Hearing Examiner's
decision is properly filed in Superior Court within such time limit, the Decision on this permit will be
final. The City's decision to issue a DNS, an MDNS or an EIS is final for this permit and any other
pending permit applications for the development of the subject property.
VI. INSPECTION OF INFORMATION ON THE APPLICATION
Project materials including the application, any staff reports, and other studies related to the permits are
available for inspection at the Tukwila Department of Community Development, 6300 Southcenter Blvd.,
Suite 100, Tukwila, Washington 98188 from Monday through Friday between 8:30 a.m. and 5:00 p.m. The
project planner is Max Baker, who may be contacted at 206-431-3662 or via email at
max.baker@tukwilawa.gov for further information.
Property owners affected by this decision may request a change in valuation for their property tax purposes.
Contact the King County Assessor's Office for further information regarding property tax valuation changes.

Project Description
The applicant is requesting approval of a Type I Wireless Telecommunication Facilities [WCF] Permit to add equipment
on an existing tower at 12200 51st Ave Pl. S. The project includes:
• Install 2.5 equipment in existing cabinet
• Add (3) antenna (1 per sector)
• Install (3) RRH units (1 per sector)
• Install (6) Junction Cylinders
• Install (3) Filters
The existing cellular tower is 80 feet high with the proposed Sprint antennas to be mounted at 77 feet adjacent to existing
Sprint antennas.
Existing Development
This site is adjacent to Burlington Northern Santa Fe rail yard. In addition to the tower, the site functions as a storage and
staging area for rail containers and includes at least two buildings.
Surrounding Land Uses
The property and the surrounding parcels to the north are zoned MIC/H. To the immediate east are rail lines and freeway.
To the south and west, the adjacent properties are zoned LDR.
BACKGROUND
In 2006, the City adopted a Wireless Facilities Chapter (TMC 18.71) to the Zoning Code. The chapter provides specific
location and design requirements for the placement of WCF within the City of Tukwila.
The WCF being modified was originally approved by the City in 1997 (L97-0030) through a conditional use permit.
In 2012 the WCF was approved for modifications (L12-032) through a Type 1 Wireless facilities permit.
Location Selection
Tukwila Municipal Code (TMC) provides a hierarchy of appropriate locations within the City. Generally, co -locations
and utilizing existing structures are favored over the development of new standalone WCF within the City.

CONCLUSIONS
1. The proposed modifications to the existing antenna will not significantly alter the conditions that currently exist

vicinity.
2. As demonstrated in Tables 1 and 2, the project meets all applicable development standards for a Type 1 WCF.
3. The proposed modifications will not alter the surrounding land uses in terms of traffic and pedestrian circulation,
building or site design.

4. The proposed project is in keeping with the goals and policies of the Comprehensive Land Use Policy Plan.
RECOMMENDATIONS
The recommendation to the Director of Community Development is to approve this application for a Type I Wireless
Telecommunication Facility permit as applied for by Crown Castle for Sprint without conditions.
INFORMATIONAL ITEMS
A City of Tukwila electrical permit is required for service alteration. Please note that the installation of an emergency
power disconnect switch is required for fire department use. Contact Don Tomaso at 206-971-8723 to get more

My name is Ron Meckler and I am a planning consultant with Crown Castle for zoning and permitting
services. Our client, Sprint PCS, is in the process of a national update and modernization of its wireless
service. The project requires adding 3 antennas to an existing aray, No change to tower height or ground
space.
A summary of the work planned for this site is:
• Install 2.5 equipment in existing cabinet
• Add (3) Antennas(1 per sector)
• Install (3) RRH units (1 per sector)
• Install (6) Junction Cylinders (2 per sector)
• Install (3) Filters
THE HEIGHT OF THE EXISTING TOWER WILL NOT BE INCREASED, NOR WILL THE FOOTPRINT
OF THE EXISTING EQUIPMENT COMPOUND BE CHANGED.
Crown Castle is the owner and operator of the wireless facility at the above address. Crown Castle
believes the existing structure will comply with all FAA regulations and applicable standards with the
improvements and alterations proposed in this permit application.
Furthermore, Crown Castle states that the proposed installation, repair, operation, upgrading,
maintenance and removal of antennas shall be lawful and in compliance with all applicable laws, orders,
ordinances and regulations of the state, and local authorities having jurisdiction.
Crown Castle further agrees to remove the facility within one year after all of the site's tenants discontinue
use of the facility or the facility falls into disrepair, and restore the site to its pre-existing condition.

mentioned tower. This analysis has been performed in accordance with the Crown Castle Structural `Statement
of Work' and the terms of Crown Castle Purchase Order Number 602709, in accordance with application
206598, revision 1.
The purpose of the analysis is to determine acceptability of the tower stress level. Based on our analysis we
have determined the tower stress level for the structure and foundation, under the following load case, to be:

The existing tapered monopole tower has three major sections connected by slip joints. The tower has 12 sides
and is evenly tapered from a 34.5" outer diameter at the base to a 19.5" outer diameter at the top. The structure
is galvanized and has no tower lighting.
The monopole was originally designed by Valmont Industries, Inc. of Valley, Nebraska in March of 1996. The
tower was originally designed for an 85 mph basic wind speed with 1/2" radial ice in accordance with TIA/EIA-
222-E.
2) ANALYSIS CRITERIA
This analysis has been performed in accordance with the 2012 International Building Code based upon an ultimate
3 -second gust wind speed of 110 mph converted to a nominal 3 -second gust wind speed of 85 mph per section
1609.3.1 as required for use in the TIA-222-G Standard per Exception #5 of Section 1609.1.1. Exposure Category
C and Risk Category II were used in this analysis.
Per section 2.6.4 of the TIA-222-G standard, ice loading may be ignored for structures located in regions where
the design ice thickness is less than or equal to 0.25 inches (6 mm). Ice loading was not considered in this
analysis.
Seismic loads were determined from hand calculations. It was concluded from these calculations that the wind
Toads control the maximum loading on the structure. The seismic loading case will not control. Refer to Appendix
C for seismic calculations.

3.1) Analysis Method
tnxTower (version 6.1.3.1), a commercially available analysis software package, was used to create a
three-dimensional model of the tower and calculate member stresses for various loading cases.
Selected output from the analysis is included in Appendix A.

1) Tower and structures were built in accordance with the manufacturer's specifications.
2) The tower and structures have been maintained in accordance with the manufacturer's
specification.
3) The configuration of antennas, transmission cables, mounts and other appurtenances are as
specified in Tables 1 and 2 and the referenced drawings.
4) When applicable, transmission cables are considered as structural components for calculating
wind Toads as allowed by TIA-222-G.
5) Mount sizes, weights, and manufacturers are best estimates based on site photos provided and
were determined without the benefit of a site visit by GPD.
6) All member connections and foundation steel reinforcing are assumed designed to meet or
exceed the load carrying capacity of the connected member and surrounding soils respectively
unless otherwise specified in this report.
7) All equipment model numbers, quantities, and centerline elevations are as provided in the CCI
CAD package dated 12/06/2013 with any adjustments as noted below.
- There should only be (1) proposed 1-1/4" coax to 61 ft. listed on the base level drawing.
This analysis may be affected if any assumptions are not valid or have been made in error. GPD

GPD GROUP has not performed a site visit to the tower to verify the member sizes or antenna/coax loading. If
the existing conditions are not as represented on the tower elevation contained in this report, we should be
contacted immediately to evaluate the significance of the discrepancy. This is not a condition assessment of the
tower or foundation. This report does not replace a full tower inspection. The tower and foundations are
assumed to have been properly fabricated, erected, maintained, in good condition, twist free, and plumb.
The engineering services rendered by GPD GROUP in connection with this Structural Analysis are limited to a
computer analysis of the tower structure and theoretical capacity of its main structural members. All tower
components have been assumed to only resist dead loads when no other loads are applied. No allowance was
made for any damaged, bent, missing, loose, or rusted members (above and below ground). No allowance was
made for loose bolts or cracked welds.
GPD GROUP does not analyze the fabrication of the structure (including welding). It is not possible to have all
the very detailed information needed to perform a thorough analysis of every structural sub -component and
connection of an existing tower. GPD GROUP provides a limited scope of service in that we cannot verify the
adequacy of every weld, plate connection detail, etc. The purpose of this report is to assess the feasibility of
adding appurtenances usually accompanied by transmission lines to the structure.
It is the owner's responsibility to determine the amount of ice accumulation in excess of the code specified
amount, if any, that should be considered in the structural analysis.
The attached sketches are a schematic representation of the analyzed tower. If any material is fabricated from
these sketches, the contractor shall be responsible for field verifying the existing conditions, proper fit, and
clearance in the field. Any mentions of structural modifications are reasonable estimates and should not be
used as a precise construction document. Precise modification drawings are obtainable from GPD GROUP, but
are beyond the scope of this report.
Miscellaneous items such as antenna mounts, etc., have not been designed or detailed as a part of our work.
We recommend that material of adequate size and strength be purchased from a reputable tower manufacturer.
GPD GROUP makes no warranties, expressed and/or implied, in connection with this report and disclaims any
liability arising from material, fabrication, and erection of this tower. GPD GROUP will not be responsible
whatsoever for, or on account of, consequential or incidental damages sustained by any person, firm, or
organization as a result of any data or conclusions contained in this report. The maximum liability of GPD
GROUP pursuant to this report will be limited to the total fee received for preparation of this report.

3. Four sets of plans prepared by a design professional. The plans shall include a vicinity map, site
map, architectural elevations, method of attachment, proposed screening, location of proposed antennas,
and all other information which accurately depicts the proposed project. Minimum size
is 8 1/2" x 11". Plans shall be no greater than 24" x 36".

ii Includes personal wireless antennas, other than a microcell, and is to be attached to an existing
structure (that may be an existing tower) that is not a residence or school and does not contain a
residence or school, and the existing structure to which it is to be attached is located in a
commercial, industrial, manufacturing, forest or agriculture zone; or
iii) Involves constructing a personal wireless tower less than sixty feet in height that is located in a
commercial, industrial, manufacturing, forest, or agricultural zone.

7. A letter from the applicant describing the proposed project and describing compliance with the
City's Code requirements.
8. Sensitive Area studies and proposed mitigation (if required).
9. If an outdoor generator is proposed, a report prepared by an acoustical engineer demonstrating
compliance with TMC Chapter 8.22, "Noise".
If the application requires a Type 2 review these additional items are needed:
10. Four sets of photo simulations that depict the existing and proposed view of the proposed facility;
11. Materials board for the screening material;
12. Landscaping plan;
13. Letter from a radio frequency engineer that demonstrates that the facility meets Federal
requirements for allowed emissions;
14. If the facility is located within a residential zone, a report from a radio frequency engineer
explaining the need for the proposed wireless communication facility at that location. Additionally, the
applicant shall provide detailed discussion on why the wireless communication facility cannot be
located within a commercial or industrial zone.

City of Tukwila governs the placement of wireless telecommunication facilities under Tukwila Municipal Code (TMC)
18.58. The review process for new antennas and facilities is based on the type of facility and the location of the facility.
The City's goal is to streamline the placement of desirable facilities that are located on existing cell towers, within
commercial and industrial zones, and on electrical transmission towers.

7. THE CONTRACTOR SHALL PERFORM WORK DURING OWNER'S PREFERRED

CERTIFICATES NAMING NETWORK CARRIER AS ADDITIONAL INSURED, AND

INSURANCE.
9. THE CONTRACTOR SHALL SUPERVISE AND DIRECT THE PROJECT DESCRIBED

RESPONSIBLE FOR ALL CONSTRUCTION MEANS, METHODS, TECHNIQUES,

12. A COPY OF THE GOVERNING AGENCY ISSUED AND APPROVED PLANS SHALL
BE KEPT IN A PLACE SPECIFIED BY THE GOVERNING AGENCY, AND BY
LAW, SHALL BE AVAILABLE AT THE JOB SITE FOR INSPECTION AT ALL
TIMES. THE ORIGINAL PERMIT SET PIANS ARE NOT TO BE USED BY THE
WORKMEN. ALL CONSTRUCTION SETS SHALL REFLECT THE SAME
INFORMATION AS GOVERNING AGENCY APPROVED PLANS. THE CONTRACTOR
SHALL ALSO MAINTAIN ONE SET OF PLANS, IN GOOD CONDITION,
COMPLETE WITH ALL REVISIONS, ADDENDA. AND CHANGE ORDERS ON THE
PREMISES AT ALL TIMES UNDER THE DIRECT CARE OF THE
SUPERINTENDENT. THE CONTRACTOR SHALL SUPPLY THE NETWORK
CARRIER CONSTRUCTION PROJECT MANAGER WITH A COPY OF ALL
REVISIONS, ADDENDA, AND/OR CHANGE ORDERS AT THE CONCLUSION OF
THE WORK AS A PART OF THE AS -BUILT DRAWING RECORDS.
13. THE STRUCTURAL COMPONENTS OF ADJACENT CONSTRUCTION OR FACIUTIES
ARE NOT TO BE ALTERED BY THIS CONSTRUCTION PROJECT UNLESS
NOTED OTHERWISE.
14. THE CONTRACTOR SHALL STUDY THE STRUCTURAL, ELECTRICAL,
MECHANICAL, AND PLUMBING PLANS AND CROSS CHECK THEIR DETAILS,
NOTES, DIMENSIONS. AND ALL REQUIREMENTS PRIOR TO THE START OF
ANY WORK. NOTIFY THE ARCHITECT OF ANY DISCREPANCIES.
15. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE COMPLETE SECURITY
OF THE JOB SITE WHILE WORK IS IN PROGRESS UNTIL THE JOB IS
COMPLETE.
16. THE CONTRACTOR HAS THE RESPONSIBILITY OF LOCATING ALL EXISTING
UTILITIES SHOWN OR NOT SHOWN ON THE PLANS, ALONG WITH
PROTECTING THEM FROM DAMAGE. THE CONTRACTOR AND SUBCONTRACTOR
SHALL BEAR THE EXPENSES OF REPAIR AND/OR REPLACEMENT OF
UTILITIES OR OTHER PROPERTY DAMAGE RESULTING FROM OPERATIONS IN
CONJUNCTION WITH THE EXECUTION OF THE WORK.
17. ALL EXISTING CONSTRUCTION, EQUIPMENT, AND FINISHES NOTED TO BE
REMOVED SHALL BECOME THE PROPERTY OF THE CONTRACTOR AND WILL
BE REMOVED FROM THE SITE WITH THE FOLLOWING EXCEPTIONS:
A. PROPERTY NOTED TO BE RETURNED TO THE OWNER.
B. PROPERTY NOTED TO BE REMOVED BY THE OWNER.
18. THE GOVERNING AGENCIES, CODE AUTHORITIES, AND BUILDING INSPECTORS
SHALL PROVIDE MINIMUM STANDARDS FOR CONSTRUCTION TECHNIQUES.
MATERIALS, AND FINISHES USED THROUGHOUT THE PROJECT. TRADE
STANDARDS AND/OR PUBLISHED MANUFACTURERS SPECIFICATIONS MEETING
OR EXCEEDING DESIGN REQUIREMENTS SHALL BE USED FOR INSTALLATION.
19. WHEN ROOF TOP OR TOP FLOOR DECK TEMPORARY STAGING IS REQUIRED,
MATERIALS SHALL BE EVENLY DISTRIBUTED OVER ROUGH FRAMED FLOORS OR
ROOFS SO AS NOT TO EXCEED THE DESIGNED LIVE LOADS FOR THE
STRUCTURE. TEMPORARY SHORING AND/OR BRACING IS TO BE PROVIDED
WHERE THE STRUCTURE DOESN'T HAVE THE DESIGN STRENGTH FOR
ADDITIONAL LOADING.
20. SEAL ALL PENETRATIONS WITHIN FIRE -RATED AREAS WITH U.L. LISTED OR
FIRE MARSHALL APPROVED MATERIALS IF APPLICABLE TO THE SUBJECT
FACILITY AND OR PROJECT SITE.
21. BUILDING INSPECTORS AND/OR OTHER BUILDING OFFICIALS ARE TO BE
NOTIFIED PRIOR TO ANY GROUND DISTURBANCE, CONSTRUCTION, AND ANY
OTHER PROJECT EFFORT AS MANDATED BY THE GOVERNING AGENCY.
22. CONTRACTOR TO PROVIDE A PORTABLE FIRE EXTINGUISHER WITH A RATING OF
NOT LESS THAN 2-A OR 2 -Al OBC WITHIN 75 FEET TRAVEL DISTANCE TO
ALL PORTIONS OF PROJECT AREA DURING CONSTRUCTION.
23. CONTRACTOR SHALL MAKE NECESSARY PROVISIONS TO PROTECT EXISTING
IMPROVEMENTS, EASEMENTS, PAVING, CURBING, DURING CONSTRUCTION. UPON
COMPLETION OF WORK, CONTRACTOR SHALL REPAIR ANY DAMAGE THAT MAY
HAVE OCCURRED DUE TO CONSTRUCTION ON OR ADJACENT TO THE
PROPERTY.
24. CONTRACTOR SHALL KEEP GENERAL WORK AREA CLEAN AND HAZARD FREE
DURING CONSTRUCTION DISPOSING OF ALL DIRT, DEBRIS, AND RUBBISH.
CONTRACTOR SHALL REMOVE EQUIPMENT NOT SPECIFIED AS REMAINING ON
THE PROPERTY OR PREMISES. SITE SHALL BE LEFT IN CLEAN CONDITION AND
FREE FROM PAINT SPOTS, DUST, OR SMUDGES OF ANY NATURE.
25. NEW CONSTRUCTION INSTALLED ADJACENT EXISTING BUILDINGS OR
CONSTRUCTION SHALL ARCHITECTURALLY MATCH THE EXISTING IN TERMS OF
COLOR, TEXTURE, FINISH MATERIALS, ETC., EXCEPT AS NOTED IN THE PLANS
AND SPECIFICATIONS.
26. THE CONTRACTOR SHALL PROVIDE ALL NECESSARY BACKING, BLOCKING.
AND/OR OTHER ANCHORAGE DEVICES REQUIRED FOR THE INSTALLATION OF
FIXTURES, MECHANICAL EQUIPMENT, PLUMBING, HARDWARE, AND FINISH ITEMS
TO INSURE A PROPER AND CODE COMPLIANT INSTALLATION.
27. THE CONTRACTOR IS RESPONSIBLE FOR PROVIDING INSTALLATIONS THAT ARE
CONSTRUCTED LEVEL, ERECT, EVENLY ALIGNED, PLUMB AND TRUE BASED ON
THE CONSTRUCTION DRAWINGS. THE CONTRACTOR SHALL COMPARE EXISTING
CONDITIONS WITH THE PROPOSED DESIGN PRIOR TO CONSTRUCTION AND
REPORT ANY DISCREPANCIES OR INCONSISTENCIES TO THE NETWORK
CARRIER'S REPRESENTATIVE AND FURTHER TO THE A&E SUCH THAT THE NEW
INSTALLATION WILL LIKEWISE BE LEVEL, ERECT, EVENLY ALIGNED, PLUMB AND
TRUE. ARCHITECT AND NETWORK CARRIER SHALL BE NOTIFIED OF ANY
ERRORS, OMISSIONS, OR INCONSISTENCIES PRIOR TO ANY CONSTRUCTION.
28. THE CONTRACTOR IS TO PROVIDE PROTECTION FOR ADJOINING PROPERTIES
FROM PHYSICAL HARM, NOISE, DUST, DIRT, AND FIRE AS REQUIRED BY THE
GOVERNING AGENCIES.
29. WHERE SPECIFIED, MATERIALS TESTING SHALL BE TO THE LATEST STANDARDS
AND/OR REVISIONS AVAILABLE AS REQUIRED BY THE GOVERNING AGENCY
RESPONSIBLE FOR RECORDING THE RESULTS.
30. THE CONTRACTOR IS RESPONSIBLE FOR THE STORAGE OF ALL MATERIALS
AND SHALL NOT STORE OR STAGE MATERIALS ON PUBLIC PROPERTY WITHOUT
A PERMIT TO DO SO FROM THE GOVERNING AGENCIES FOR THIS PURPOSE.
31. GENERAL NOTES AND STANDARD DETAILS ARE THE MINIMUM REQUIREMENTS
TO BE USED IN ALL CONDITIONS UNLESS ILLUSTRATED AND NOTED
OTHERWISE.
32. TRADES INVOLVED IN THE PROJECT SHALL BE RESPONSIBLE FOR THEIR OWN
CUTTING, FITTING, PATCHING, ETC., SO AS TO BE PREPARED PROPERLY BY
THE WORK OF OTHER TRADES.
33. ALL DEBRIS AND REFUSE SHALL BE REMOVED FROM THE PROJECT PREMISES
AND LEFT IN A CLEAN SWEPT CONDITION AT ALL TIMES BY EACH TRADE AS
THEY PERFORM THEIR OWN PORTION OF THE WORK.
34. NETWORK CARRIER DOES NOT GUARANTEE ANY PRODUCTS, FIXTURES, AND/OR
ANY EQUIPMENT NAMED BY A TRADE OR MANUFACTURER. GUARANTEE OR
WARRANTY THAT MAY BE IN EFFECT IS DONE THROUGH THE COMPANY OR
MANUFACTURER PROVIDING THE PRODUCT, FIXTURE. AND/OR EQUIPMENT
UNLESS SPECIFIC RESPONSIBIUTY IS ALSO PROVIDED BY THE
CONTRACTOR/SUBCONTRACTOR IN WRITTEN FORM.
35. CAUTION! CALL BEFORE YOU DIG! BURIED UTIUTIES EXIST IN THE AREA
AND UTILITY INFORMATION SHOWN MAY NOT BE COMPLETE. CONTACT THE
ONE -CALL UTILITY LOCATE SERVICE A MINIMUM OF 48 HOURS PRIOR TO
CONSTRUCTION.
36. WHEN APPUCABLE, CONTRACTOR IS RESPONSIBLE TO CALL, COORDINATE AND
MAKE ARRANGEMENTS FOR R.O.W. AND/OR PRIVATE PROPERTY LOCATES
BASED ON SPECIFIC SITE REQUIREMENTS.
37. SEE CIVIL DRAWINGS FOR ADDITIONAL SITE INFORMATION AS APPLICABLE.
38. CONTRACTORS TO DOCUMENT ALL WORK PERFORMED WITH PHOTOGRAPHS
AND SUBMIT TO NETWORK CARRIER'S REPRESENTATIVE ALONG WITH REDUNED
CONSTRUCTION SET.
39. CONTRACTOR SHALL DOCUMENT ALL CHANGES MADE IN THE FIELD BY
MARKING UP (REDLINING) THE APPROVED CONSTRUCTION SET AND
SUBMITTING THE REDUNED ALONG WITH PHOTOGRAPHS PER NETWORK
CARRIER REQUIREMENTS.
40. CONTRACTOR SHALL COORDINATE AND SEEK APPROVAL OF ALL POWER DRAW,
INSTALLATION AND/OR MODIFICATIONS WITH POWER COMPANY, OWNER AND
JURISDICTION AS REQUIRED. CONTRACTOR SHALL REPORT POWER
INSTALLATION SOLUTION(S) TO NETWORK CARRIER REPRESENTATIVE, PROJECT
CONSTRUCTION MANAGER AND ARCHITECT.
41. ANY SUBSTITUTIONS OF MATERIALS AND/OR EQUIPMENT, MUST BE APPROVED
BY NETWORK CARRIER CONSTRUCTION MANAGER.
42. THE CONTRACTOR SHALL BE RESPONSIBLE FOR AND SHALL REMEDY ALL
FAULTY, INFERIOR, AND/OR IMPROPER MATERIALS, DAMAGED GOODS, AND/OR
FAULTY WORKMANSHIP FOR ONE (1) YEAR AFTER THE PROJECT IS COMPLETE
ACCEPTING UNDER THIS CONTRACT BETWEEN THE OWNER AND THE
CONTRACTOR. EXCEPTION: THE ROOFING SUBCONTRACTOR SHALL FURNISH A
MAINTENANCE AGREEMENT FOR ALL WORK DONE, COSIGNED BY THE
CONTRACTOR, TO MAINTAIN THE ROOFING IN A WATERTIGHT CONDITION FOR A
PERIOD OF TWO (2) YEARS STARTING AFTER THE DATE OF SUBSTANTIAL
COMPLETION OF THE PROJECT, UNLESS OTHERWISE WRITTEN IN THE
CONTRACT BETWEEN THE OWNER AND THE CONTRACTOR.
43. THE CONTRACTOR SHALL PROVIDE ADEQUATE PROTECTION FOR THE SAFETY
OF THE OWNER'S EMPLOYEES, WORKMEN, AND ALL TIMES DURING THE
CONSTRUCTION OF THE PROJECT.
44. THE CONTRACTOR SHALL BE REQUIRED TO PAY FOR ALL NECESSARY PERMITS
AND/OR FEES WITH RESPECT TO THE WORK TO COMPLETE THE PROJECT.
BUILDING
PERMIT APPLICATIONS SHALL BE FILED BY THE OWNER OR HIS
REPRESENTATIVE. CONTRACTOR SHALL OBTAIN PERMIT (UNLESS OTHER
ARRANGEMENTS HAVE BEEN MADE) AND MAKE FINAL PAYMENT FOR SAID
DOCUMENT(S).
45. NETWORK CARRIER'S REPRESENTATIVE SHALL REVIEW AND APPROVE SHOP
DRAWINGS AND SAMPLES FOR CONFORMANCE WITH DESIGN CONCEPT.
NETWORK CARRIER'S REPRESENTATIVE PROJECT APPROVAL OF A SEPARATE
ITEM SHALL NOT INCLUDE APPROVAL OF AN ASSEMBLY IN WHICH THE ITEM
FUNCTIONS.
46. ALL ANTENNAS MOUNTED ON ROOF SUPPORT FRAMES TO BE PROVIDED BY
NETWORK CARRIER.
47. CONTRACTOR SHALL PROVIDE HEAVY STEEL PLATES AT OPEN TRENCHES FOR
SAFETY AND TO PROTECT EXISTING GROUND SURFACES FROM HEAVY
EQUIPMENT UTILIZED DURING CONSTRUCTION.
48. CONTRACTOR SHALL PATCH AND REPAIR ALL GROUND SURFACES WITHIN THE
CONSTRUCTION AREA AS NECESSARY TO PROVIDE A UNIFORM SURFACE AND
MAINTAIN EXISTING SURFACE DRAINAGE SLOPES.
49. CONTRACTOR SHALL REPLACE EXISTING LANDSCAPE VEGETATION DAMAGED DUE
TO CONSTRUCTION ACTIVITIES, AND REPAIR, RESTORE AND MODIFY EXISTING
IRRIGATION LINES IF NECESSARY TO OPERATING CONDITION, PROVIDING FULL
COVERAGE TO IMPACTED AREAS.
50. IN THE CASE OF ROOFTOP SOLUTIONS FOR EQUIPMENT AND/OR ANTENNA
FRAMES WHERE PENETRATION OF EXISTING ROOFING MATERIALS OCCUR,
CONTRACTOR SHALL COORDINATE WITH BUILDING OWNER TO OBTAIN CONTACT
INFORMATION AND UTIUZE THE EXISTING ROOFING CONTRACTOR OF RECORD
FOR INSTALLATION, PATCH, REPAIR OR ANY AUGMENTATION TO THE ROOF,
AND HAVING THE WORK GUARANTEED UNDER THE ROOFING CONTRACTOR'S
EXISTING WARRANTY ENSURING 100% MOISTURE PROTECTION.
51. IN THE CASE OF ROOFTOP SOLUTIONS WITH THE INSTALLATION OF ANTENNAS
WITHIN CONCEALED (SHROUDED) SUPPORT FRAMES OR TRIPODS, CONTRACTOR
SHALL COORDINATE WITH THE FRP DESIGNER/FABRICATOR TO ENSURE THAT
THE FINAL FRP SHROUD IS SIMULATING (IN APPEARANCE) EXISTING EXTERIOR
BUILDING FACADE MATERIALS, TEXTURES, AND COLORS. THE CONTRACTOR
SHALL FURTHERMORE ENSURE THE USE OF COUNTERSUNK OR FLATHEAD AN
FASTENERS IN ALL FRP CONSTRUCTION. WHEN PHOTOSIMULATIONS ARE
PROVIDED, THE CONTRACTOR SHALL ENSURE THAT FINAL CONSTRUCTION
REPRESENTS WHAT IS INDICATED IN PHOTOSIMULATION. SHOP DRAWINGS
SHALL BE PROVIDED TO THE CONTRACTOR, CONSTRUCTION COORDINATOR,
AND ARCHITECT PRIOR TO FABRICATION AND CONSTRUCTION.

VERIFY EACH COAXIAL CABLE LENGTH, DIAMETER, ROUTING, COLOR CODING AND ALL
APPURTENANCES WITH NETWORK CARRIER REPRESENTATIVE.
2. THE MAXIMUM COAXIAL CABLE LENGTH AND CORRESPONDING COAXIAL CABLE DLAMETER
HAS
BEEN ESTIMATED. THIS CABLE LENGTH IS APPROXIMATE, AND IS NOT TO BE USED FOR
FABRICATION OR CONSTRUCTION. ACTUAL ANTENNA CABLE LENGTH(S) MAY VARY FROM

FRAMES WHERE ANCHORING TO AN EXISTING CONCRETE ROOF SLAB IS
REQUIRED, CONTRACTORS SHALL CONFIRM (PRIOR TO SUBMITTING BID) WITH
CONSULTING CONSTRUCTION COORDINATOR AND ARCHITECT THE PRESENCE OF
POST TENSION TENDONS WITHIN THE ROOF SLAB RESULTING FROM AN
UNDOCUMENTED DESIGN CHANGE IN THE EXISTING BUILDING "AS -BUILT

55. CONTRACTOR SHALL OBTAIN, REVIEW AND EXECUTE ALL NETWORK CARRIER
CONSTRUCTION STANDARDS (MOST RECENT REVISION) AS A PART OF THIS BID
AND CONSTRUCTION PROJECT.
56. CONTRACTOR SHALL BE RESPONSIBLE TO SET ELECTRONIC TILTS FOR NEWLY
INSTALLED ANTENNAS UNDER THE CONDITION THAT THE CONTRACTOR OBTAIN
THE MOST RECENT COPY OF THE RF TILT INFORMATION SUCH THAT THE
ACCURATE CONTROLLER CAN BE ORDERED AND INSTALLED.
57. A STRUCTURAL ANALYSIS SHALL BE COMPLETED AND SUBMITTED TO THE
NETWORK CARRIER REPRESENTATIVE AND CONTRACTOR DEMONSTRATING
CAPACITY AT THE EXACT LOCATION OF EXISTING CONDITIONS TO SUSTAIN
ADDITIONAL HEAVY BATTERY CABINETS OR OTHER OUT OF SCOPE EQUIPMENT.
58. THE CONTRACTOR SHALL PROVIDE MATERIALS UST (BOM) TO THE NETWORK
CARRIER REPRESENTATIVE PRIOR TO CONSTRUCTION.
TENNA & HYBRID COAX NOTES
VERIFY EACH HYBRID CABLE LENGTH, DIAMETER, ROUTING, COLOR CODING AND ALL
APPURTENANCES WITH NETWORK CARRIER REPRESENTATIVE.
2. THE HYBRID CABLE LENGTH AND CORRESPONDING DIAMETER HAS BEEN ESTIMATED. THIS
