Four units responded( Sunpro, Inc. FireRMS 5.0 Version: 2.0.25 were assigned to this incident. Eleven personnel Firefighter Hayman was on this alarm assigned to E51, but is not in data base at this time. We arrived on scene at 0953 hours and cleared at 1048 hours. The incident occurred at 1165 Andover Pk W, Tukwila in District 006. The general description of this property is warehouse. The primary task(s) performed at the scene by responding personnel was investigation. No mutual/automatic aid was given or received. • Tukwila fire Department Page: 5 Incident Report 2001-0002366-000 Printed: 07/25/2001 13:30:48 Narratives Upon arrival A54 reported active gas leak due to fork lift hitting ceiling heater and breaking gas line. Building was partially evacuated. E51 arrived and established Command. Gas service was shut off by E51. I made contact with Paul Kron (Warehouse Manager) and he advised me that everyone was out of the occcupancy, and he confirmed a forklift operator ran into the gas ceiling heater. A54 packed up and investigated the area and noted an open gas line was broken off from the ceiling heater in the warehouse. Once gas meter was shut off, we began mechanically ventilating the warhouse area using PPV. Adjoining occupancies were also evacuated by A54. E 52 depolyed precautionary 1 3/4 line to warehouse roll up door. PSE arrived and performed several measurements within the warehouse, and advised me that there were no longer any readings of gas inside the building. There was still an odor and we requested a reading with another monitor to confirm this reading of zero. PSE Suppervisor, Rich Williams, soon arrived and using another monitor, confirmed a zero reading of natural gas and he advised it was safe to occupy the building. The warehouse manager was advised he would have to contact a private contractor to make the repair and the gas valve was to remain off.
