FireRMS 5.0 Version: 3.26.43 New Narrative Incident 21:03:19 Thursday, September 27, 2001 1112 - Grisham, Martin LT 1 At 1106 hours on Thursday September 27, 2001 we were dispatched to a semi truck accident, unknow if injuries. Upon arrival, the first unit found a semi tractor -trailer off the road in the median. The median is a wetland area, so after it was determined there were no injuried, the crews did a recon to check for environmental contamination. The cab was upright, but the trailer, which was carrying a Sealand container, was on it's side. The Sealand contariner was placarded with a "Corrosive", & a Flammable, and a Marine Pollutant. The chemical was leaking from the container. First in Officer called for additional units and Chief 52. Upon his arrival (Chief 52), he assumed IC, and called for the Off Duty HazMat Team. The HazMat Team Leader arrived and was assigned Operations. Tukwila Safety Officer arrived and assumed Safety. We had 5 HM Technicians on location. It was determined we would have to make a Leval A entry, so we called for Renton HazMat unit. DOE was notified, and responded to the scene. A small amount of product was seen leaking from the container. We put absorbant material around the area's it was leaking from. We did a Ph test, which showed an "8". We did air monitoring around the container and got no significant readings. With a full entry and back up team in place, pre -vitals taken, decon set up, we had the tow truck upright the container so we could make entry. Foss Company was on scene, and secured by the owner of the trucking company to do the clean up. Foss set up plastic sheeting on the ground and diking around the container as it was uprighted. We had diked the area down stream of the spill area. Upon entering the container, we found a 2X4 accross the back of the drums. This board was up against the plastic drums of material. Our entry team pulled the 2x4 out of the way to get access to the containers. The 2x4 had a 16 penny nail sticking out the drum side of the board. The drums moved due to the accident, and hit up against the nail in the 2x4, punturing the drum. It was dificult to tell which other drums were leaking, so we had to removed all the drums out of the container. DOE on scene estimated 150 gallons spilled out. Fire remained on scene until the last of the drums were removed. After that the scene was turned over to WSP and DOE who were on scene to oversee the clean-up. Eight units were assigned to this incident. Nineteen personnel responded. We arrived on scene at 1112 hours and cleared at 2029 hours. The incident occurred at the 3300 Hundred block of SR 599 SB, Tukwila in District 007. The local station is ST53. The general description of this property is a divided highway. The primary task(s) performed at the scene by responding personnel were to provide first aid and check for injuries on the driver. It was determined that he was not injured. A copy of the shipping papers are attached to this report. What was on the shipping papers and what was in the SeaLand Container did not match. The drums were lables "Flammable" and Corrosive". On the drum it had a lable. It listed the chemical as "ZR -1". The Lot # was "631870". The quantity was "900.0 kg". It has a sign on the drum that read..."This container is Fluxed under nitrogen". The company name of "Shin-Etsu Chemical Co., LTD" was on the label. It had a UN number of 2924. The plastic drum was housed in an alluminum cage. The truck was a white Freightliner, with good tires, and a sign in the front window stating, "SFGS 186". A sign on the door read "Seattle Freight Service, Inc.. The vin number was not visable at the door. It had an ICC MC # of 171191. And also "CHL -8" under the DOT #. The trailer was blue in color, with the letters and numbers of "SCZU 794925 g" on the sides, as well as the name "DSR -Senator". The drivers DL # is Equipment that was used and destroyed at the scened were; 2 Keplar Responder Suits 4 pairs of Kevlar and latex gloves 1 bale of absorbants 1 50' utility rope Chemical test strips & Ph paper.
