Tukwila Fire Department
incident. Four units were assigned to this incident. Eleven personnel responded. We
arrived on scene at 1139 hours and cleared at 1358 hours. The incident occurred at 1073
Andover Park E, Tukwila in District 005. The local station is ST51. The general
description of this property is manufacturing, processing. The primary task(s) performed
at the scene by responding personnel were medical aid, material detection, monitoring,
sampling, and analysis. No mutual/automatic aid was given or received.
Thru elimination of other causes for this emergency, It is believed that a diesel truck left
running outside a bay door may have caused the Co poisoning. The propane forklift at this
address will be checked by an industrial hygenist on 11/20/02 to eliminate this as a
possible source.
E-51 stayed on the scene and monitored the Co levels in all of the building until a safe
level was attained. At this time we released the building to occupants.

At 1136 hours on Tuesday November 19, 2002 we were dispatched to a carbon monoxide

incident. Four units were assigned to this incident. Eleven personnel responded. We
arrived on scene at 1139 hours and cleared at 1358 hours. The incident occurred at 1073
Andover Park E, Tukwila in District 005. The local station is ST51. The general
description of this property is manufacturing, processing. The primary task(s) performed
at the scene by responding personnel were medical aid, material detection, monitoring,
sampling, and analysis. No mutual/automatic aid was given or received.

Enclosed please find a report from Sea -Aire, the HVAC company that I hired to inspect the warehouse freeze
protection heaters at the above reference location.
I spoke with Tony Ellis of Graphic Sciences today and encouraged him to make arrangements to inspect the
propane forklifts being used in the space for proper operation. Tony indicated that their company will have an
industrial hygenist visit the space tomorrow to evaluate the situation and that they will provide me with a report
on the outcome.

Our Technician Phil. performed Visual inspection of unit
heaters for cracks in heat exchangers and also checked With
Carbon monoxide detector. He also checked flues to make
sure they werc not plugged and made sure china caps were
secured properly on roof. All heaters checked out okay.
A possible source of carbon monoxide could be the pro -
Pane forklift in the middle space. Alec, a truck was left'
running near the building and exhaust could have infiltrated
due to the Southwest wind*.
