Incident Report
2003-0002537-000
Tukwila Fire Department
Five units were assigned to this incident. Fourteen personnel responded. We arrived on
scene at 1725 hours and cleared at 2132 hours. The incident occurred at 15451 53 Ave S,
Tukwila in District 003. The local station is 52. The general description of this property is
hotel/motel, commercial. The primary task(s) performed at the scene by responding
personnel was investigation. Other special haz mat actions were also taken. Automatic aid
was received on this incident.
Alarm number 0002537 has been assigned to this incident.
Fire Dept dispatched to a report of a white chemical haze in the air in a room with white
powder on the kitchen table. Upon arrival of E-52, HazMat 52 and B-51 we met the
manager who took us to the room and stated that they had the fire alarm go off in the room.
Manager checked and the occupant stated that they were cooking and had tripped the
alarm. Shortly after the room alarm tripped again and when the manager went to check she
found that the smoke detector had been pulled out of the ceiling and the occupants were
now gone. There was a white haze in the air and the strong smell of glue. At the door we
could smell an odor of glue or solvents. It was decided that two of us would enter in
bunkers, on air and recon. While using a four gass monitor we entered and found white
powder on the kitchen table, counter and in the bathroom. Found glass jars with clear
tlquids and sediment in the bottoms. Found solvents and cold capsules. Glassware and
coffee filters around with some coffee filters containing residue. Pans in cupboard with
powder residue. In the refrigerator found a pan of liquid that was slowly boiling. Backed
out of the room at this time and reported to B-51. Air monitor had a slight reading of a
flammable atmosphere. B-51 decided to call for a HazMat strike team. Evacuated the
bottom and second floor on the west side of the building. Strike team units arrived and
finished setting up for a full Level A entry with all appropriate standards being put into
place. Triple entry team took air, radioactive, PH, ammonia, BIO and four gas samples.
Found several chemical inside and determined that the liquid in the refrigerator was
ammonia in liquid. Discussions held with TPD and determined that scene is to be
processed for possible criminal prosecution. State Patrol drug lab team and DOE advised

with both responding. Stated Patrol team processed scene for evidence and then DOE
picked up the chemicals and contaminated cookware and furniture. All equipment picked
up and stowed and all Tukwila units were phased out at various times of the incident with
E-52 and HazMat 52 being the last to leave the scene.
