At 2128 hours on Monday October 25, 2004 we were dispatched to a gas leak. Two units
were assigned to this incident. Four personnel responded. We arrived on scene at 2131
hours and cleared at 2236 hours. The incident occurred at 350 Treck Dr, Tukwila in
District 005. The local station is 51. The general description of this property is warehouse.
The primary task(s) performed at the scene by responding personnel was to shut down

system. No mutual/automatic aid was given or received. On arrival found strong smell of
gas in warehouse area. Appears leak from a ceiling fan that may have been struck by a fork
lift. Only slight rise in LEL per E51 gas detector. Warehouse area was ventilated with
power fan while waiting for PSE rep to shutdown system. System was shutdown at meter
and responsible was informed by PSE what actions he would need to perform in the am.
Building was turned over to company manager and B51 and E51 returned in service.
Alarm number 0003579 has been assigned to this incident.
