Narrative Text At 0818 hours on Monday January 9, 2006 we were dispatched to a carbon monoxide
incident. Eight units were assigned to this incident. Fourteen personnel responded. We
arrived on scene at 0825 hours and cleared at 0955 hours. The incident occurred at 12622
#B INTERURBAN Ave S, Tukwila in District 001. The local station is 52. The general
description of this property is business office. The primary task(s) performed at the scene
by responding personnel was investigation. Other special haz mat actions were also taken.

Alarm number 0600095 has been assigned to this incident.
E52 responded to the above address for a smell of natural gas within the building. E52
upon arrival made contact with the reporting party and smelled some type of gas within the
structure. E52 evacuated all occupants within the structure and then proceeded to do air
monitoring. Air monitoring of the above address found only slight elevated CO content of
10 to 15 ppm. In occupancy 12624 INTERURBAN Ave S, Western Graphics a high
reading of 70 to 80 ppm of CO with a LEL of 5 to 8%. No other notable were located
within 12624. In 12626 and 12622 slightly elevated CO of 5 to 15 ppm was found with no
other notables. E52 ventilated 12622 #B with ppv E52 focused on 12624 occupancy were
the reading were the highest to locate the source. E24 assisted in ventilation of 12624. L54
went to the roof and investigated. Also they forced entry to 12622 to monitor the
atmosphere. Reef Property Management had previously be notified by one of the
occupants of the gas issue and they had already contacted PSE and a representative arrived
on location soon after E52. This was also the case for a HVAC technician. Both were
consulted in there area of knowledge. E52 contacted people evacuated and evaluated them
for medical aid and none was necessary. E52 was unable to located any source giving off
any CO within 12624, this unit was ventilated and reading were brought down to less that
5 ppm. This unit was monitored two additional times prior to E52 returning inservice with
no change of levels of CO. E52 made contact with Greg a representative of Reef Property
Management (206) 940-0593 about the damage to the door of 12622. He said someone
would be out to repair it. Also E52 contacted Walter Lucas, CTS Designer of 12622 #B
CompView (253) 229-3338 to notify him that the business was unsecured and E52 was
unable to secure the business. He contacted an employee that was about 45 to 60 minutes
from the location and said it was ok for E52 to leave the business unsecured until the
employee arrive. E52 returned inservice.
