Beckman, Kenneth Author Rank LT Author Assignment 1 Narrative Text At 0042 hours on Sunday August 27, 2006 we were dispatched to a gas leak. Five units were assigned to this incident. Eleven personnel responded. We arrived on scene at 0048 hours and cleared at 0139 hours. The incident occurred at 4415 S 150 St, Tukwila in District 012. The local station is 54. The general description of this property is elementary school, including kindergarten. The primary task(s) performed at the scene by responding personnel was investigation. Other special haz mat actions were also taken. Automatic aid was received on this incident. Alarm number 0603064 has been assigned to this incident. Dispatched to odor of natural gas by TPD working school security. He noticed the odor in the school bus turn around. He stated to us there had been a problem Friday, a micro leak at the earthquake valve. PSE scheduled to come out and fnc it the next week Monday. Checked the meter for the school, no odor or movement of the meter showing any flow. While checking out the area, noticed strong odor at the storm drain out on the street located in front of the bus turn loop. The odor by his car had dissipated. We used the air monitors and found the highest concentration of 80 ppm at this drain. Investigated all other drains in the area with no findings. PSE had been called. E52 reported a slight odor at the SE corner of the school, upon returning to that area later, it was gone. When PSE arrived we gave him our information and fmdings, he was summoning more assistance. rtq End of Report Page: 4 Printed: 08/27/2006 08:19:39 Incide'nt History for:#TF06002923 CN: $TF06003064 Xref:#TP06024476 Received 08/27/06 00:42:58 BY PD33 /VC5014 Entered 08/27/06 00:42:58 BY PD33 /VC5014 Dispatched 08/27/06 00:43:05 BY FD23 /VC5903 Enroute 08/27/06 00:44:48 Onscene 08/27/06 00:48:32 Closed 08/27/06 01:39:00 Initial Type:NGASXT TG: Initial Alarm Level:1 Final Type:NGASXT (NATURAL GAS LEAK OUTSIDE (5A))Pri:lDispo: Lev: 1 Police TP2500 Fire TF5421 Grp:T7Beat: Lt/Lg: Police Rms: Fire Rms: Medic Rms: Loc:4415 S 150 ST ,TUK -- THORNDYKE ELEMENT (V) Name:9T24 Addr: Phone: 00:42:58 VC5014 PD33 OP Alrm ENTRY ON LOC, STRONG ODOR OF NAT GAS OUTSIDE, CONTACT PD ON LOC $TF06003064 DISP L54 #TF1135 BECKMAN, KEN #TF1121 HAYMAN, BEN #TF1140 CODENYS, ALAN #TF1178 REES, RICHARD C/F3 ASST E52 #TF1119 MCFARLAND, LOREN #TF1106 JOHNSON, AARON #TF1107 NICHOLS, JEFF C/F3 ASST A14 #RF0069 HARTMAN, GREG #RF0112 SWYNENBURG, KEVIN #RF0153 MCGEE, BOB C/F3 $RF06006505 GRP: T9 ---> T7. 00:42:58 PD33 $ASNCAS 00:43:05 VC5903 FD23 00:43:05 FD23 00:43:05 FD23 00:43:05 00:43:53 FD23 ASNCAS A14 FD23 CONTRL 00:44:48 TF1119 E52 00:45:42 RF0112 A14 00:45:44 VC5903 FD23 00:45:51 TF1135 L54 00:46:27 VC5903 FD23 00:46:49 00:46:56 *ENROUT E52 *ENROUT A14 ASSTER B51 *ENROUT L54 CDGRN A14 FD23 CLEAR A14 FD23 ASST E51 00:48:32 FD23 00:49:49 TF1143 E51 00:49:50 TF1119 E52 ONSCNE L54 *ENROUT E51 *ONSCNE E52 [4415 S 150 ST ,TUK] #TF1122 PETERSON, L AVERN ,A14 IS CODE GREENED.
