Narrative Text At 1158 hours on Saturday September 2, 2006 we were dispatched to an oil or other
combustible liquid spill. One unit was assigned to this incident. Three personnel
responded. We arrived on scene at 1203 hours and cleared at 1212 hours. The incident
occurred at 13900 56 Ave S, TUKWILA in District 002. The local station is 52. The
general description of this property is residential street, road or residential driveway. The

Narratives
primary task(s) performed at the scene by responding personnel were hazardous materials
leak control and containment. No mutual/automatic aid was given or received.
Alarm number 0603164 has been assigned to this incident.
Upon arrival found the vehicle on the right side of the roadway with an oil trail leading
twentyfive feet behind the vehicle. Trail started at a Sewer man hole cover that has a dip
in the pavement adjacent to it of six plus inches. Mrs Simmermon states that she just
drove over the cover and heard a loud bang and then smelled what she thought was
gasoline. She parked the vehicle and got her and her daughter away from the vehicle.
Mrs Simmermon had a chuck of the oil pan with the oil plug in it that had been ripped out
of the oil pan. Approximately four quarts of engine oil on the ground. Applied Amerisorb
and broomed it in. TPD on location collecting information.
2000 VW Beetle, 740 LDP.
Left Mrs Simmermon awaiting the arrival of her husband.
E-52 in service- returning.
