Narrative Name 0703748 Narrative Type Incident Narrative Date 00:17:10 Thursday, October 25, 2007 Author TF1134 - Morales, Mark Author Rank FFI Author Assignment 1 Narrative Text At 1118 hours on Wednesday October 24, 2007 we were dispatched to an oil or other combustible liquid spill. One unit was assigned to this incident. Three personnel responded. We arrived on scene at 1128 hours and cleared at 1211 hours. The incident occurred at 6900 I-405 Fwy S, Tukwila in District 003. The local station is 51. The general description of this property is highway or divided highway. The primary task(s) performed at the scene by responding personnel was investigation. No mutual/automatic aid was given or received. Alarm number 0703748 has been assigned to this incident. E51 dispatched to I-405 S off ramp to West Valley for a reported oil sheen causing vehicles to slide, approximate amount said to be about 5 gallons. The first due station is Station 51. E51 arrived to find a 300 to 400 foot long oil sheen on the exit ramp off I-405 to Interurban Ave S. Washington State Patrol arrived on location with DOT. Washington State Patrol said that a sanding truck was in route with a 20 to 30 minute estimated arrival time. E51 investigated to find the road conditions to hazardous to remain open to motor vehicles. E51 communicated this to Washington State Patrol. Washington State Patrol agreed with E51 assessment. E51 assisted DOT and Washington State Patrol in closing the off ramp to motor vehicle traffic. E51 viewed Interurban Ave S from I-405 and could see a visible oil sheen at that location. E51 communicated with Washington State Patrol if any further assistance was needed. He said there was not. E51 observed a semi that was in the process of being towed at the intersection of Interurban Ave S and the off ramp. E51 interviewed the driver of the semi and found that the semi was the cause of the oil sheen that covered the off ramp and a quarter mile North and South of the off ramp intersection. E51 assessed the semi for additional spill containment however the semi's oil system was empty. E51 notified B51 of the additional hazardous road location. B51 contacted Tukwila City Shops to notify them of the need for a sanding truck at this location. E51 requested City of Tukwila Police to come to this location for any addition documentation for their records. E5lobtained the driver's information in addition to a representative from the trucking company that was on scene. City of Tukwila Police interviewed the driver. E51 returned in-service.
