Narrative Name 0704454 Narrative Type Incident Narrative Date 19:56:06 Friday, December 14, 2007 Author TF 1176 - Crowley, Mark Author Rank FFI Author Assignment 1 Narrative Text On 12/14/2007 at 0919 hrs, Engine 52 was dispatched to a reported SPILL at 301 Baker Blvd. Engine 51 was out of service for training. E52 arrived (0927) to find an oil spill from a mini -van (Wa License 697TZF). The vehicle had been vandalized (See Tukwila PD Case # TP07-008589). The investigating officer (1T5, Craig Gardner) requested our presence due to another, unrelated spill that had sheened over a 50 x 300 area in the immediate vicinity, and primarily concentrated in the construction staging area for 301 Baker Blvd (SE corner of building). FM51 was added to the incident to assist determining the responsible party (for clean-up). FM51 made contact with General Manager Don Malstrom and notified him that he was responsible for clean-up. Malstrom stated he would do so. E52, FM51 then left the scene. -end- End of Report Page: 3 Printed: 12/14/2007 20:02:26 Incident History for:#TF07004202 CN: $TF07004454 Xref:#TP07034096 Deceived Entered Dispatched Enroute Onscene Closed 12/14/07 12/14/07 12/14/07 12/14/07 12/14/07 12/14/07 09:19:45 09:19:45 09:19:49 09:22:30 09:27:56 10:32:09 BY PD33 /GOLREE BY PD33 /GOLREE BY FD23 /VC1140 Initial Type:SPILL TG: Initial Alarm Leve1:1 Final Type:SPILL (FLAMMABLE LIQUID SPILL (2A))Pri:2Dispo: v: 1 Police TP700 Fire TF5122 Grp:T7Beat: Lt/Lg: Police Rms: Fire Rms: Medic Rms: Loc:301 BAKER BL ,TUK -- ACURA OF SEATTLE (V) Add'l Loc Info:BACK PLOT, BETW CIRC CITY/ACURA "Name:1T5 Addr: Phone: a7 -W r Alrm Le 09:19:45 GOLREE PD33 ENTRY REQ FD FOR SPILLED OIL/GAS UNK AMT, POSS FROM DAMAGED VEHS ON LOT 09:19:45 PD33 $ASNCAS $TF07004454 09:19:49 VC1140 FD23 DISP E52 #TF1176 CROWLEY, MARK #TF1157 DENNEY, JODI #TF1163 RYDEEN, STEVE C/F3 09:20:01 GOLREE PD33 CROSS #TP07034096 09:20:31 VC1140 FD23 CONTRL GRP: T9 ---> T7. 09:22:30 TF1176 E52 *ENROUT E52 09:27:56 E52 *ONSCNE E52 09:35:49 VC8395 FD24 ASST B51 [301 BAKER BL ,TUK] #TF1164 SOSS, MIKE 09:36:28 TF1164 B51 *ENROUT B51 09:37:00 VC8395 FD24 NEWLOC E52 [250 APW BEH BBB] 09:38:23 TF1164 B51 MOBINS B51 09:39:10 VC8395 FD24 ASSTER FM51 [250 APW BEH BBB] #TF1170 TOMASO,DON FM5 1 2063968910 09:42:18 TF1170 FM51 *ONSCNE FM51 10:29:56 TF1176 E52 MOBINS E52 10:32:09 TF1170 FM51 MOBINS FM51 10:32:09 FM51 CLOSE E52
