E51 arrived at 1119 and cleared the scene at 1132 hrs. Page: 2 Printed: 02/28/2008 18:49:03 Incident Report 2008-0800804 -000 Tukwila Fire Department Narratives On arrival, E51 was met by the occupant, BRYAN, who stated that when he opened the door to his business he was overpowered by a very strong chemical/fiberglass type odor throughout his tenant space. He opened all the doors and windows to air out the space (with success), but was concerned about the source of the smell. Investigation revealed that 960 Industry Dr (Wholesale Sign) had recently installed a spray booth which had been used the previous night. An open container of paint thinner had been left out on a work bench overnight. The occupant of 960, TAYLOR, stated that one of his employees usually paints things in the evening, and when he came in this morning found a STRONG odor of paint chemicals in his business. As he is used to the odor, he did not think anything of it. He will speak with his employee to see what happened and why the materials had not been put away properly the night before. It should be noted that the spray booth used was a new installation (City Permit approved on 02/26/2008 and on location). A copy of this report was forwarded to the Fire Marshal's Office for appropriate follow-up. -end- End of Report Page: 3 Printed: 02/28/2008 18:49:03 Incident Received Entered Dispatched Enroute Onscene Closed History for:#TF08000755 CN: $TF08000804 02/28/08 02/28/08 02/28/08 02/28/08 02/28/08 02/28/08 11:16:04 11:16:04 11:16:11 11:18:11 11:19:42 11:32:25 BY CR10 /VC8731 BY CR10 /VC8731 BY FD23 /VC0496 -*oA-o KOL/ Initial Type:ODORIN TG: Initial Alarm Level:1 Final Type:ODORIN (ODOR INVESTIGATION INDOORS (2A)Pri:2Dispo: Lev: 1 Police TP1200 Fire TF5132 Grp:T7Beat: Lt/Lg: Police Rms: Fire Rms: Medic Rms: Loc:964 INDUSTRY DR ,TUK near UNK (V) Add'l Loc Info:BRYAN EMERGENCY AUTOMOTIVE RESPONSE Name:BRYAN,ALAN/OWNER Addr:B.E.A.R. 11:16:04 11:16:04 11:16:11 11:16:53 11:18:11 11:19:42 11:32:25 11:32:25 VC8731 CR10 ENTRY CR10 $ASNCAS VC0496 FD23 DISP E51 FD23 TF1176 E51 VC3690 FD24 TF1176 E51 E51 CONTRL Alrm Phone:2063712165 NOW, STRONG CHEMICAL ODOR INS BLDG, UNK SOURCE $TF08000804 #TF1176 CROWLEY, MARK #TF1178 REES, RICHARD #TF1123 GILBERT, MATTHEW C/F3 GRP: T9 ---> T7. *ENROUT E51 ONSCNE E51 , INV MOBINS E51 CLOSE E51
