Page: 5 Printed: 04/09/2008 16:08:18 Incident Report 2008-0801390 -000 Tukwila Fire Department Narratives E51 and multiple units dispatched to gas leak from 3/4" supply pipe to warehouse heater that had been broken by a forklift operator. E51 arrived first to fmd occupancy evacuating. E51 tied in with building manager, confirming that all occupants were out, and determining the likely source of the leak. The manager stated that he had the gas main line shut off on side D of the occupancy. B51 did a 360 and took 180th Command. Ch52 stood by, available. E51 went on air and entered the building to monitor the atmosphere, using the four gas monitor. E51 opened some man doors on sides C & D, enabling natural ventilation to occur. E51 confirmed the location of the broken gas line, and that gas was not flowing. L54 took PPV blowers to the man door on side C, ventilating the warehouse. Puget Sound Energy arrived and also took gas readings. All other units code greened. E51 continued PPV operations when L54 and B51 returned in service. After consulting with Puget Sound Energy, E51 turned over the occupancy to the manager so that business could resume. E51 told the manager that E51 had gotten some high CO readings, and that those readings could relate to the operation of propane powered forklifts in the warehouse. E51 recommended that a CO monitoring program be considered. E51 returned in service.
