Narrative Text At 1004 hours on Monday November 10, 2008 we were dispatched to a fire. Two units
were assigned to this incident. Seven personnel responded. We arrived on scene at 1007
hours and cleared at 1110 hours. The incident occurred at 730 Andover Park E, Tukwila in
District 005. The local station is 51. The general description of this property is warehouse.
The primary task(s) performed at the scene by responding personnel was investigation. No
mutual/automatic aid was given or received.
The estimated property loss on this incident was $2,000. The estimated content loss was
$50,000. The estimated property value was $2,000. The estimated content value was
$50,000.
Alarm number 0804308 has been assigned to this incident.
E51 dispatched to 730 Andover Pk E for a reported forklift on fire. The first due station is
Station 51. While in route E51 was updated by dispatch that employees have removed the
propane tank off the forklift and they believe that the cause of the fire was electrical.
E51 arrived to find a 300 X 100 two story concrete tilt -up with light gray smoke coming
from the role up door on side A with the reporting party in the doorway.
E51 investigated and found a forklift with dry chemical extinguishing agent over it with
fire damage, no fire extension. E51 confirmed extinguishment and locate cause of fire .
E51 found a significant amount of smoke within the warehouse area. E51 requested that
L54 to ventilate the building. L54 used positive pressure ventilation to accomplish this
task. E51 examined businesses on either side of this address and did not fmd any smoke or
other relating issues.

Narratives
L54 upon completion of the ventilation task monitored the atmosphere within the building
and upon leaving the only value on the four gas detector that was not zero was Carbon
Monoxide. This value was 16 parts per million (ppm). Well below OSHA permissible
exposure limit of 50 ppm. E51 was unable to down load the information from the air
monitor.
E51 took three photos of the involved forklift and have attached them to this report. They
are numbered as follows 0804308 #1, 0804308 #2 and 0804308 #3.
E51 was assisted by Inspector 514 and Fire Marshal 51 in estimating amount of property
damage and requiring the reporting party to have the HVAC system to the building
serviced.
E51 returned in-service. E51 provided a copy of this report as requested to Marshal 51
with photos on a CD.
