Narrative Name 0901224 Narrative Type Incident Narrative Date 13:45:09 Friday, April 10, 2009 Author TF 1132 - Lucero, Brian Author Rank FFI Author Assignment 1 Narrative Text At 0540 hours on Friday April 10, 2009 we were dispatched to an oil or other combustible liquid spill. Two units were assigned to this incident. Four personnel responded. We arrived on scene at 0549 hours and cleared at 0610 hours. The incident occurred at 13435 Interurban Ave S, Tukwila in District 001. The local station is 52. The general description of this property is service station, gas station. The primary task(s) performed at the scene by responding personnel were hazardous materials spill control and confinement. No mutual/automatic aid was given or received. Alarm number 0901224 has been assigned to this incident. E52 was dispatched to 13435 Interurban ave s. for a flammable liquids spill. Upon arrival we found a 20' X 20' spill of diesel fuel. The fuel was traveling west into the oil separator. I made contact with witness Mari Tosthuma. She told me that the driver of the truck removed the gas nozzle from his fuel tank while still flowing product. The driver then replaced the nozzle into the fuel pump and drove away. Mrs. Tosthuma said that the fuel nozzle must have been defected because it wouldn't shut off. I directed the crew of E52 to contain and suppress the spill with Amerisorb while I contacted the Pacific Pride Hotline. I spoke with Mr. Rob Jett the lead manager and I informed him of the incident. Mr Jett then transferred me to Mr. Matt Hoyos, the Incident Response Technician. Mr. Hoyos said that they would send a team out to clean the spill. I then requested that if possible to retrieve the driver license plates from their video Surveillance. I was told that they would work on getting me that information that I had requested. Later the same day I spoke with Mr. Rob Jett regarding the Surveillance footage and possible card lock user identification. He did not have the information as of yet, but he said he would get back to me.
