Narrative Text E51 was dispatched to a hazardous materials investigation with L54 and B51.
E51 arrived on location at 13030 Military Rd S. and made a face to face with B51 and
L54. E51 was assigned recon, product testing and then over -pack the product. E51 was
staffed with two Haz-Mat technicians, Capt. Hayman and FF. Hickey. Prior to entry L54,
B51 and E51 had a safety briefing at which time L54 was designated the standby team.
E51 entered with the radiological meter, protein test kit, a chemical classifier strip, the
TIC, and a Hazmat IQ. E51 reconed the product that was in the men's bathroom on the
main floor. This plastic container was approximately a gallon in size and was filled'/< of
the way, with a white power. After examining the container and testing the product, E51
over -packed the product with a sealable bucket.
E51 found no heat release from the product, no radiological indicators, PH was neutral,
and negative was found for: oxidizers, fluoride, petroleum, organic solvents, iodine,
bromine, and chlorine. E51 received an identification of Zinc Chloride fume via the
Hazmat IQ. With the test completed and the container over -packed, E51 relayed this
information to B51 (Riverton Command) and exited the building.

was required of E51, so B51 released E51 from the call.

Narrative Text At 1937 hours on Wednesday April 29, 2009 we were dispatched to a hazardous condition.
Three units were assigned to this incident. Eight personnel responded. We arrived on scene
at 1943 hours and cleared at 2152 hours. The incident occurred at 13030 Military Rd S,
Tukwila in District 015. The local station is 54. The general description of this property is
doctor, dentist or oral surgeon's office. The primary task(s) performed at the scene by
responding personnel was investigation. No mutual/automatic aid was given or received.
Alarm number 0901428 has been assigned to this incident.
L54 dispatched to a Haz-Mat investigation. Upon our arrival we were met at the door by
the reporting party whom stated that there was an unidentified white powder in an
unmarked container in the men's restroom on the first floor. We conducted some brief
interviews gathering information. Nothing remarkable came from them. Initially we were
told that it had not been touched, however, we were told later that the cleaning lady had
handled the container. No one was complaining of any unusual symptoms.
We went into the men's room and looked at the container it was approximately 1 gallon in
size filled around 3/4. The container was not off gassing and was clearly not labeled. I
made a call to the on duty Haz-Mat team members working on E51 to come up with a plan.
E51 was added to the call and proceeded to the scene. B51 arrived and I tied in with what
we had found. B51 established Riverton Command and asked for a PD response. B51
interviewed the occupants gathering information. The building was evacuated by PD.
When E51 arrived B51 conducted a briefing. E51 was the entry team (2 members) and was
to attempt to identify the product. E51 driver was the apparatus operator. L54 officer
became Safety, 2 members were the back up team and decon if needed.
E51 entered and and conducted a Acid/Base Risk test, Oxidizer risk test, Fluoride risk
test, Petroleum Organic Solvent risk test, Iodine Bromine Chlorine risk test and the
Haz-Mat ID analyzer. All of the risk test were negative and the Haz-Mat ID came back
with a 93% accuracy of Zinc Chloride. This product was used for a variety of applications
medical uses being some of them. The entry team over packed the product and exited the
building.

It was determined that this was now Hazardous waste. B51 contacted the building owner
and advised him that we would be placing the over pack container back in the men's room
and placing scene tape across the door. The owner was also told he has the responsibility
on disposal and clean up.

B51 received a page from Valley Com asking me to call Fire One. Fire One advised me of
L54 being dispatched to a Haz Mat call at 13030 Military Rd S. I advised them that I was
preparing to respond with L54.
During my response L54 contacted E51, who had two Technicians, to gather HM testing
equipment and respond to the scene.
Upon arrival, I tied in with A/Capt Rodal. I established "Riverton Command" and called
for PD to respond due to the potential crime scene. I directed L54 to secure the building,
upon PD's arrival, I had them ensure that the building was evacuated.
I met with an employee form the Highline Women's Health Center, Anjoo Sood. She was
able to give me contact information for the buildings management personnel.
On L54's arrival, they found that the product was isolated to the mens restroom on the first
floor.
Upon E51's arrival, A/Capt Rodal, Capt Hayman and I met to discuss our action plan.
Capt Hayman and FF Hickey were assigned entry, FF McCone and FF Karwhite were
assigned as back up, A/Capt Rodal was assigned Safety and FF Rees and FF Codenys were
assigned to stand by.
E51 made entry in bunker gear and SCBA's with their testing equipment. See E51's
narrative for a detailed report regarding the product.. They were able to pack the jar in an
overpack container and found light dusting of the product on the shelf in the restroom. E51
exited the building with out incident.
After the product was secured and identified I contacted the Facilities Coordinator for
AGM Incorporated, Brandon Kuvara, to advise him of the situation and clean up
responsibilities. He agreed to assume responsibility for the product. I advised him that the
product would be in the restroom and that he needed to ensure proper disposal of the
product and to have the loose product properly cleaned in the restroom. He also agreed to
do this.
The entry team returned the overpack container to the restroom and secured the door with
fire scene tape.
