Narrative Text At 1101 hours on Wednesday July 22, 2009 we were dispatched to an oil or other
combustible liquid spill. One unit was assigned to this incident. Three personnel

responded. We arrived on scene at 1106 hours and cleared at 1130 hours. The incident
occurred at 3355 S 120 Pl, Tukwila in District 007. The local station is 53. The general
description of this property is professional supplies, services. The primary task(s)
performed at the scene by responding personnel was investigation. No mutual/automatic
aid was given or received.
Alarm number 0902604 has been assigned to this incident.
E53 dispatched to a hazardous investigation. While responding we were notified that a
unknown product had spilled and could be anywhere from 100 to 400 gallons. Upon our
arrival we could not find the spill. We finally found someone who told us to go to the back
of the building. When we arrived we found a Diesel spill approximately 20 gallons on the
driveway and into a storm drain. A small amount of fuel had spilled in the building as well.
There were several workers applying absorbent pads and booms on the spill to contain it.
One of the workers took me into the building to show me what the problem was. I entered
the generator room for Intemap and there was a service tech working on one of the
generators. He told me he was servicing the generator and had started it up to check that it
ran properly and when he shut it down it would not turn offproperly. The diesel pump did
not shut off from the day tank and the return pump failed to turn on pressurizing the system
and sending fuel out of the vent pipes down the side of the building onto the pavement.
John Sasser from Sabey came out and he advised me that he had already contacted a clean
up company (Clean Harbor) to clean up the spill. He had already contacted the Department
Of Ecology as well. The person from the DOE who would be coming out was Tammy
Sacayanan. DOE stated to John that they would contact the National Response Center.
John also had manually shut off the fuel pump and manually started the return fuel pump
which stopped the flow of fuel.
With the fuel spill stopped, contained and absorbent pads and booms in place I gathered as
much information as I could and we left them with the clean up.
Around an hour after the time of call I called the National Response Center to verify the
the spill had been reported and it had not. I called several hours later and still no report. I
then made the notification of the spill. The report number is 912408
