Eleven units were assigned to this incident. Eight personnel responded. We arrived on scene at 0839 hours and cleared at 1408 hours. The incident occurred at 7851 S 192 St, KENT in District KF 1218. The local station is 51. The general description of this property is manufacturing, processing. The primary task(s) performed at the scene by responding personnel were material detection, monitoring, sampling, and analysis. Other special haz mat actions were also taken. Mutual aid was given on this incident. E51, HM51 with Haz Mat Techs Aaron Johnson and Jim Evans, and B51 responded to a mutual aid haz mat call in Kent. Upon arrival, B51 became staging officer, E51 initially became standby team, and HM51 assisted Kent Haz Mat techs with set up for Haz mat monitoring and analysis. E51 eventually became responsible for gross decon in the event of a liquid contamination by ammonia. The operation consisted of two parts: First, verify that there had been a spill. If so, then where is it and is it contained. Second, to mitigate the spill and its cause. The first part went very smoothly. The spill was verified. It was in a compressor room and for the most part was contained. The second part (and second entry) consisted of ventilation to allow a repair technician to enter with a canister mask and repair the guage/valve. This was done by the second entry team and was successful. Subsequent monitoring showed that the leak was indeed stopped and the area properly ventilated. This operation was, in my opinion, very well thought out. Safety was paramount at all times and everything went according to the plan. There were no injuries. Johnson Narrative Type Company Narrative Date 18:46:33 Tuesday, July 20, 2010 Author TF 1106 - Johnson, Aaron Author Rank FFI Author Assignment 1 Narrative Text At approximately 0830 I received a phone call from Battalion Chief Ewing. I was instructed to staff HM51 for a zone 3 hazmat response, my assigned apparatus (L54) proceeded to Station 51 so that I may staff HM51 with FF Evans and respond to the call in Kent. Upon arrival, HM51 was assigned to the Hazmat group supervisor. We were directed to set up HM 51 on the southwest corner of the structure to support entry team operations for a leak of anhydrous ammonia. After setting up the entry link camera and weather station I was assigned to provide assistance to the entry team. My duties included preparing monitoring equipment such as the dragger with ammonia gastec tubes, PH paper and operating the entry link camera software from HM51. All operations went smoothly and safely. We were properly briefed prior to making entry by the Hazmat group supervisor, Entry team leader, Incident Safety officer, and research division. Entry was performed by a three hazmat tech's with a back up team ready and in place outside the structure. Baseline vitals were taken in accordance with policy before and after level A entry. The entry team identified the leak and performed monitoring of the affected area. The leak was secured remotely by a facility worker. The space was also ventilated in a controlled manner as to dilute the concentration of anhydrous ammonia in air to a permissible level. Once the leak was mitigated and controlled ventilation and monitoring of the space were complete, operations were terminated and clean up was initiated. All operations took place with a high regard to safety resulting in no injuries or exposures. All equipment was returned and HM51 restored back into service.
