Narrative Text At 1704 hours on Friday August 20, 2010 we were dispatched to a gas leak. One unit was
assigned to this incident. Three personnel responded. We arrived on scene at 1710 hours
and cleared at 1721 hours. The incident occurred at 14802 Tukwila Intl Blvd, Tukwila in
District 011. The local station is 54. The general description of this property is street or
road in commercial area. The primary task(s) performed at the scene by responding
personnel was investigation. No mutual/automatic aid was given or received.
Alarm number 1002991 has been assigned to this incident.
L54 was dispatched to a possible car fire. Upon arrival L54 was met by Tukwila PD. PD
advised that the involved pick-up truck was carrying numerous propane bottles in the bed
of the truck.
L54 and L54-3 found three five gallon and one twenty gallon propane truck in the bed,
secured with a single rope. The five gallon bottles were upright and the twenty gallon tank
was on its side. It appeared that the twenty gallon bottle had vented, causing the reports of
a possible car fire. Residue was found near the valve of the twenty gallon tank. The tank
was not venting on our arrival.
I advised the driver of the dangers in transporting the propane in such a manner. She stated
that she was not going to move the truck until her husband arrived to secure the tanks.
The scene was turned over to Tukwila PD.

Hello all. Please see the attached and following from Derek. While Derek did not have to submit a formal
request, the best way to handle this important and time sensitive issue is through use of the Public Records
Request process. The list of addresses and parcels are in the attached EXCEL file.
To meet the timeline, it would be helpful to have records no later than this Friday, September 22, so I can
compile them for FTP transfer. DCD, Public Works, and Fire staff, please put documents in your departmental
directory on Z:\Public Records Responsive Documents and let me know when they are there.
Nicole, Derek would like spill information for the locations attached..
Cindy and Barbara. He would like to know what businesses were at each of the locations for as far back as we
go. Barbara, that means 22 years for the addresses listed and for Cindy, those since 2011 when they took
over licensing. He is interested in businesses that may have hazardous materials: gas stations, paint store,
auto parts, etc. Let me know of questions. Thanks!

Christy,
The City is evaluating three sites for a potential city project and we are starting a phase one environmental
assessment. As part of that, we would like to know if there are any significant environmental issues related to the sites.
I've attached a list of the sites with parcel numbers and addresses. I've also attached maps since that may help with
context. Would you please ask the key staff in the following departments to gather any pertinent records?
Primarily, we are looking for the following:
(1) Any environmental contamination or chemical releases or related cleanup
(2) Any underground storage tanks or related permits
(3) Any code enforcement actions related to environmental contamination
(4) Any other environmental risks
We also would like to know if there were any businesses ever on the sites that are likely to have environmentally
hazardous materials such as a dry cleaners, gas station, paint supply store, etc.
Unfortunately, we are on a tight time frame. I need to pass this information along to our consultant by noon this coming
Monday, September 256. Please ask staff to place whatever they can find by that time into a folder on the Z
drive. Since there are three sites, it would help me if you can have a sub folder for each site.
I assume this would need to be sent to Public Works (Laurie), DCD (Kia and Jacklyn/Lori), Fire (Norita and Nicole),
Finance (Cindy), and your office (Barbara). Also, please ask Don to look this over since he has a great memory and may
just know something significant about the sites. People can call me if they have any questions.
