Narrative Text At 1406 hours on Monday March 28, 2011 we were dispatched to a gas leak. Four units
were assigned to this incident. Eight personnel responded. We arrived on scene at 1412
hours and cleared at 1537 hours. The incident occurred at 16425 Southcenter Parkway,
TUKWILA in District 004. The local station is 51. The general description of this property
is street. The primary task(s) performed at the scene by responding personnel was
investigation. Other special hoz mat actions were also taken. No mutual/automatic aid was
given or received.
Alarm number 1101026 has been assigned to this incident.
E51 responded from Station 54. We approached from the northeast on strander. Upon
arrival E52, Battalion 52, and FM51 were on location. E51 was directed to park south of
E52 to the south of the incident on Southcenter Parkway to assist in blocking traffic from
the south along Southcenter Parkway.
I observed a large commercial asphalt saw sitting in the road with a cut line in the road
with the sound of a compressed gas hissing out. I could smell the natural gas. The wind
was blowing approximately 10 to 15 miles from the south. There was an exposure, an
AT&T store directly to the west of the leak in the road.
E51 checked in with Southcenter Parkway command and was assigned to check the 16400
building for the presence of natural gas. FF Kim and I checked the 16400 building with
the BW Gas Alert and had zero presence of volatile organic compounds (w/ PID) and 0%
reading of LEL. Business on the 1st floor were notified that there was a leak across the
street and that if they began to smell natural gas to leave the building. With the wind
blowing north and no smell nor reading of natural gas, I determined that people working in
the 16400 building were in no danger and could stay in place. I reported my fmdings and
recommendations to Southcenter Parkway command face to face.
PSE arrived shortly after I cleared the 16400 building and began jack hammer work near
the leak to access the buried 5/8" pipe that had been cut under the street. E51 stood by
south of the repair appx 100' up wind while the repairs took place.

Once PSE was able to secure the line, E51 transitioned to the task of clearing the adjacent
building 16425 Southcenter parkway, the AT&T store. The occupants were evacuated by
E52 early in the incident and no representative was on location. I attempted to get into the
building to conduct atmospheric testing to ensure a safe atmosphere however the building
was locked. The key box had an old key which did not work. I contacted Fire 3 and they
had no subscriber information for the address and the manager, who's number we received
from calling the AT&T store located in the mall, did not answer his phone when we called
it.
I notified the construction supervisor that we were unable to clear the building and left my
card. They were going to be on location for a while and said they would call if the building
occupants returned. E51 cleared the incident and returned to quarters to attempt further
contact of the store supervisor. At Station 51 the construction manager called and said that
they had contacted a security employee for the store and that the staff was expected to
return to the building at 1630. I also talked to Adam who was still on scene with PSE. PSE
had a natural gas meter with them and agreed to check the interior of the store when the
employees returned at 1630. AJ

Narrative Text I added myself to this incident in the 16400 Block of Southcenter Parkway. Upon my
arrival, E-52 had just arrived and was blocking Southcenter Parkway just south of
Strander Blvd. I parked near them. E-51 was still enroute. When I stepped out of my
vehicle, I could smell natural gas in the area. We quickly discovered we were downwind
of the Natural Gas leak. E-52 moved upwind of the break and blocked traffic. I tied in
with the PSE rep on site and moved my vehicle upwind also.
A rep for Atkinson Construction informed me they cut through a natural gas line with a 9
inch circular saw being used to cut the asphalt. I confirmed with them that they did a
"locate" before they started cutting. They did, but the depth of the line was so shallow that
the 9" blade cut through it.
We had a strong wind out of the south, approx 10 mph. The leak was near the west
sidewalk of Southcenter Parkway, just north of the north entrance to McDonalds. There
were only four buildings in the area of the break, and only two of those were downwind of
the break. I had E52 crew check for any odor in the buildings on the west side of the
street, and E-51 checked the 16400 building on the East side of the leak. Reports back to
me were that no odor was detected by nose or monitors. We informed occupants of 16400
SC Prky, McDonalds and the DoubleTree Suites that they could remain open and continue
business. The A T & T store that was north and downwind of the leak was given direction
to evacuate until we could get the leak stopped. Employees left their buildings and were
not present when we had the leak stopped. E-51 addressed all the efforts they made to
contact them so they could re -open.
I directed E51 to park south (upwind) of the incident on their arrival.

The PSE rep informed me this was a service line, 5/8" in diameter, NOT the 4" line I was
told it might be by the construction crew that actually cut through the line.. Their (PSE)
plan was to jackhammer up the asphalt to get to the leak, pinch off the pipe to stop the leak
and then make the necessary repairs. I supported his plan and informed him I would put a
crew in place to protect his team as they dug up the ground. We moved Engine 51 so they
were in reach with an 1 3/4" line.
At the point that PSE had exposed the line, I terminated command and turned over the
scene to the E-51 officer. E-52 was released also.
I coordinated with PD and PW for traffic control in and around the area.
I cleared at 1530 hours, turning over the scene to E-51 officer.

Narrative Text E-51, E-52 and Battalion 51 responded for a natural gas leak outside.
E-52 responded while doing inspections in 51's area. E-52 was first to arrive, we met with
the Reporting party who stated that they cut a natural gas line approximately 9 inches
down with a concrete saw.
E-52 noticed we were down wind and repositioned to block traffic with E-51. Battalion 52
arrived and initiated command. He asked E-52 to evacuated McDonalds and two other
buisnesses that were downwind and close to the incident.
E-52 evacuated the buisness, checked the air and it was negative in all three buisnesses.
Later in the incident E-52 was assigned to let the personnel back into McDonalds and then
E-52 was released and returned.

Narrative Text At 1406 hours on Monday March 28, 2011 we were dispatched to a gas leak. Four units
were assigned to this incident. Eight personnel responded. We arrived on scene at 1412
hours and cleared at 1537 hours. The incident occurred at 16425 Southcenter Parkway,
TUKWILA in District 004. The local station is 51. The general description of this property
is street. The primary task(s) performed at the scene by responding personnel was
investigation. Other special haz mat actions were also taken. No mutual/automatic aid was
given or received.
Alarm number 1101026 has been assigned to this incident.
E51 responded from Station 54. We approached from the northeast on strander. Upon
arrival E52, Battalion 52, and FM51 were on location. E51 was directed to park south of
E52 to the south of the incident on Southcenter Parkway to assist in blocking traffic from
the south along Southcenter Parkway.
I observed a large commercial asphalt saw sitting in the road with a cut line in the road
with the sound of a compressed gas hissing out. I could smell the natural gas. The wind
was blowing approximately 10 to 15 miles from the south. There was an exposure, an
AT&T store directly to the west of the leak in the road.
E51 checked in with Southcenter Parkway command and was assigned to check the 16400
building for the presence of natural gas. FF Kim and I checked the 16400 building with
the BW Gas Alert and had zero presence of volatile organic compounds (w/ PID) and 0%
reading of LEL. Business on the 1st floor were notified that there was a leak across the
street and that if they began to smell natural gas to leave the building. With the wind
blowing north and no smell nor reading of natural gas, I determined that people working in
the 16400 building were in no danger and could stay in place. I reported my fmdings and
recommendations to Southcenter Parkway command face to face.
PSE arrived shortly after I cleared the 16400 building and began jack hammer work near
the leak to access the buried 5/8" pipe that had been cut under the street. E51 stood by
south of the repair appx 100' up wind while the repairs took place.

Once PSE was able to secure the line, E51 transitioned to the task of clearing the adjacent
building 16425 Southcenter parkway, the AT&T store. The occupants were evacuated by
E52 early in the incident and no representative was on location. I attempted to get into the
building to conduct atmospheric testing to ensure a safe atmosphere however the building
was locked. The key box had an old key which did not work. I contacted Fire 3 and they
had no subscriber information for the address and the manager, who's number we received
from calling the AT&T store located in the mall, did not answer his phone when we called
it.
I notified the construction supervisor that we were unable to clear the building and left my
card. They were going to be on location for a while and said they would call if the building
occupants returned. E51 cleared the incident and returned to quarters to attempt further
contact of the store supervisor. At Station 51 the construction manager called and said that
they had contacted a security employee for the store and that the staff was expected to
return to the building at 1630. I also talked to Adam who was still on scene with PSE. PSE
had a natural gas meter with them and agreed to check the interior of the store when the
employees returned at 1630. AJ

Narrative Text I added myself to this incident in the 16400 Block of Southcenter Parkway. Upon my
arrival, E-52 had just arrived and was blocking Southcenter Parkway just south of
Strander Blvd. I parked near them. E-51 was still enroute. When I stepped out of my
vehicle, I could smell natural gas in the area. We quickly discovered we were downwind
of the Natural Gas leak. E-52 moved upwind of the break and blocked traffic. I tied in
with the PSE rep on site and moved my vehicle upwind also.
A rep for Atkinson Construction informed me they cut through a natural gas line with a 9
inch circular saw being used to cut the asphalt. I confirmed with them that they did a
"locate" before they started cutting. They did, but the depth of the line was so shallow that
the 9" blade cut through it.
We had a strong wind out of the south, approx 10 mph. The leak was near the west
sidewalk of Southcenter Parkway, just north of the north entrance to McDonalds. There
were only four buildings in the area of the break, and only two of those were downwind of
the break. I had E52 crew check for any odor in the buildings on the west side of the
street, and E-51 checked the 16400 building on the East side of the leak. Reports back to
me were that no odor was detected by nose or monitors. We informed occupants of 16400
SC Pricy, McDonalds and the DoubleTree Suites that they could remain open and continue
business. The A T & T store that was north and downwind of the leak was given direction
to evacuate until we could get the leak stopped. Employees left their buildings and were
not present when we had the leak stopped. E-51 addressed all the efforts they made to
contact them so they could re -open.
I directed E51 to park south (upwind) of the incident on their arrival.

The PSE rep informed me this was a service line, 5/8" in diameter, NOT the 4" line I was
told it might be by the construction crew that actually cut through the line.. Their (PSE)
plan was to jackhammer up the asphalt to get to the leak, pinch off the pipe to stop the leak
and then make the necessary repairs. I supported his plan and informed him I would put a
crew in place to protect his team as they dug up the ground. We moved Engine 51 so they
were in reach with an 1 3/4" line.
At the point that PSE had exposed the line, I terminated command and turned over the
scene to the E-51 officer. E-52 was released also.
I coordinated with PD and PW for traffic control in and around the area.
I cleared at 1530 hours, turning over the scene to E-51 officer.

Narrative Text E-51, E-52 and Battalion 51 responded for a natural gas leak outside.
E-52 responded while doing inspections in 51's area. E-52 was first to arrive, we met with
the Reporting party who stated that they cut a natural gas line approximately 9 inches
down with a concrete saw.
E-52 noticed we were down wind and repositioned to block traffic with E-51. Battalion 52
arrived and initiated command. He asked E-52 to evacuated McDonalds and two other
buisnesses that were downwind and close to the incident.
E-52 evacuated the buisness, checked the air and it was negative in all three buisnesses.
Later in the incident E-52 was assigned to let the personnel back into McDonalds and then
E-52 was released and returned.
