Narrative Text On Wednesday, 21 March 2012, Engine 52 was investigating an Emergency Medical
Alarm at 18251 Cascade Av S (Case $TF1200972)
During that investigation we were informed that on the 20 March 2012, a fire/explosion
had occurred at this location, inside a ventilation hood unit.
One dry chemical fire extinguisher was used to extinguish the fire. The fire did not extend
outside the ventilation hood. There were no injuries associated with this event.
A worker had been heating a solution of oil and an unknown solvent in a laboratory flask
on a hot plate/heater unit when the explosion and fire occurred.
The fire was not large enough to activate the sprinkler system or transmit an automatic fire

ventilation hood)
A copy of this report (and a Photo CD) has been forwarded to the Fire Marshal's Office for
follow-up:
1. Examine, repair, reset Ventilation hood unit
2. Review site safety plans for reporting fire/explosion incidents
3. Verify that expended fire extinguisher has been recharge/replaced
4. Address any additional concerns by tenants or FMO staff
