Incident Report
2012-1201608 -000
Tukwila Fire Department
Narratives
Alarm number 1201608 has been assigned to this incident.
L54 was dispatched to possible unknown fire. Reporting party was in parking lot of his
residence which is just south of the fire building. Reporting party states he heard glass
breaking and saw residents running with a fire extinguisher.
L54 arrived on location with Tukwila Police. TPD was accessing hotel complex from the
east side of the structure. TPD cut chain with bolt cutters to access rear parking lot. L54
made entry from that entrance.
L54 arrived to fmd odor of something burning and odor of dry chemical extinguisher that
had been discharged. No audible alarms or strobes were activated. While approaching the
structure there was a large amount of shattered glass with the curtains down and hanging
out of the windows in unit 163. In the parking lot outside of unit 163 there were curtains
and the window screens. There was blood on the ground in and around the broken glass
and on the doors of units 163 and 164 and blood droplets between the same rooms in the
exterior walkway.
L54 looked through the windows and had a slight haze of extinguisher chemical. There
was no visible smoke or fire. L54 forced entry into unit 163 to further investigate the fire
odor and found no fire or smoke but that a fire extinguisher had been discharged inside the
kitchen of unit 163.
While L54 was exiting unit 163 a man who was naked with a towel around him came out
of the unit next door (164). This man had burns to both arms and face and several
lacerations. He stated he was cooking hash oil when there was an explosion and that his
arms and face were burning. He also stated that he dove through and broke the window
causing the lacerations. L54 started patient care and M4 was added to the call to evaluate
his injuries. The patient was later transported to Harbor View Medical Center by TriMed
ambulance.
L54 requested a fire investigator and FM71 (Fritz Wininger 253-740-1057) was added.
L54 assisted FM71 with his investigation and L54 went to front office to investigate the
fire alarm panel while FM71 and Tukwila Police continued their investigations.
When L54 arrived at the front office of the hotel I spoke to the manager on duty (Natalia
Lopez 206-244-6464). She stated the alarm panel went into supervisory alarm and the
panel stated she called into room 163. When the occupant of room 163 answered they
stated that there was no problem. The fire alarm had smoke detector activation's in rooms
163, 164, and 263.
When L54 arrived back to unit 163 from the front office L54 was asked by Tukwila Police
that the occupant in unit 263 requested we check his room prior to him re-entering. L54
investigated with our gas meter to fmd no readings and advised him that everything was ok
and he could return to his room.
L54 advised FM71 that the fire alarm panel showed detector activation's in room 163, 164,
and 263. FM71 was met by the Hotel manager (Natalia Lopez) who opened room 164 after

there was no answer to FM71 knocking on that door. FM71 continued his investigation
into 164 after the hotel manager opened the unit for him and until then room 164 had not
been observed.

location. TPD requested a copy of this fire report and was advised to contact Tukwila Fire
