Narrative Text At 0837 hours on Wednesday November 5, 2014 we were dispatched to an oil or other
combustible liquid spill. Three units were assigned to this incident. Six personnel
responded. We arrived on scene at 0843 hours and cleared at 0933 hours. The incident
occurred at 6545 S Glacier St, Tukwila in District 006. The local station is 51. The general

description of this property is vehicle parking area. The primary task(s) performed at the
scene by responding personnel was investigation. Automatic aid was received on this
incident.
Alarm number 1404385 has been assigned to this incident.
Fire and Hazmat units were dispatched to an incident that has a semi truck saddle fuel tank
being punctured and is now leaking into a PSE underground power vault. Upon arrival of
B-51 and E-51 it is found that a semi truck was just driving into the business parking area
where semis always travel to line up and back up to the loading docks. The semi was
driving over the lids of the power vault as is normal and the lid popped up behind the left
front wheel and into the front of the diesel fuel tank. The adjoining lid also popped up and
wedged up into the frame work of the semi. A 3/4" hole was punched into the front of the
fuel tank about 1/3 of the way up from the bottom of the tank. The driver states that he
had recently fueled the tank which holds 100 plus gallons of fuel. He has dual saddle tanks
but they do not have the bottom cross feed fuel lines and are top feed only. An employee
finds a piece of plywood that is placed across the top of the now open vault so that we may
place a prowler containment pool so catch most of the fuel that is flowing out of the tank.
Hazmat 76 arrives and gets their hand pump. Business employee also comes out with a 55
gallon drum. Pump the fuel from the pool into the drum and also pump fuel from the tank
into the drum. Hazmat 76 also uses plug and dike to get the puncture hole plugged. B-51
has been calling for PSE but they are not available at this time to check on their vault.
Todd Mattson wiht CBRE property management arrives to see what is happening. He has
called a company to bring a steel plate to place over the vault top when it is cleared. He is
also working with the trucking company to figure out a tow truck to get the truck released
from the vault lids that are jammed up under it. PSE is significantly out in response time
yet. B-51 releases Hazmat 76. The scene is turned over to Todd Mattson who is awaitng
PSE to see if they are going to handle the diesel cleanup in their vault.
