Page 4 of 5 Printed: 04/29/2015 16:56:29 Incident Report 2015-1501654 -000 Tukwila Fire Department Narratives E52 arrived first shortly following B51 and E51. B51 established Lowes command and assigned E52 and E51 to locate the leak and utilities to isolate the gas leak. Employees informed B51 that a forklift in receiving hit a heater in the overhear which severed the gas line and caused a large leak from the broken pipe. PSE was requested to the location. E52 located the main gas meter and isolated the supply of gas using a halogen bar. Once the main gas meter was isolated, the leak was significantly slowed and stopped. E51 performed monitoring inside and with the help of PSE located some pockets of gas in receiving on floor 2 that required ventilation. E51 then laddered the inside at a skylight and bent the fusible link to release it to allow the skylight to open and vent the gas. E51 continued to vent and monitor along with PSE representative until smells and gas levels were well within a safe levels. PSE used their meter walking the building confirming the gas was vented from the building. E51 then packed up our PPV fan and ladder and cleared the incident.
