Narratives
E 1 upgraded the call shortly after reading the short report to HAZLEAK (Zone 3 Hazmat
response) due to having multiple pts at a know facility with chemical hazards.
E5 1 arrived first on location and directed all units to set up on the South west side of the
building as the l'/inds were out of the SW. E51 proceeded to side A (East front entrance) to
investigate. The building did not appear to be evacuated.
E51 was met outside the front entrance and was informed that there were 3 patients in the
nurses office and there was a potential leak in the south side of the building where the
Nu ie,;ts wer: ,,-king. 276 arri•,ed on location and assumed 200th command and assigned
E5 1 to RECON. E51 relayed to command that we had not yet made pt contact and that
resources wouli oe needed to contact and begin pt treatment which was assigned to E46
who was arriving.
E51, lead by an employee performed a recon of the south side of the building where the
leak was suspected. Inside personnel were working with no protection and no complaint.
RECON (E51) directed management to evacuate all personnel from the affected section of
the building the 7 walked outside the building to the south.
While inside 1 received the following readings on our BW Gas Alert 02= 20.9 C0= 0
H2S= 0 LEL= uJ% and 0 reading on the PID.
Upor_ i .lentilying the effected area, isolating access and denying entry to the effected
portion of the building, E5 t tied in with Hazmat Group Supervisor and was assigned back
up -:earn for the ntry without incident.
E51 zttended bi ,th the pre and post entry briefings and was released once the building was
clear. No defini-ive source of the leak or irritant substance was identified.
