STEVENS H/M 52 . PETER WOJICK E51 TUKWILA FIRE DEPARTMENT INCIDENT REPORT NARRATIVE Page 1 Date: 06/06/99 Incident: 99-001814-000 E-51, E-52 and HM 52 Responded to a liquid spill at 803 S.C. Mall. The product spilled was a iquid coating for eyeglass lenses. The amount of the spilled product was around a liter. The business had a spill control plan and had placed corn cob granuals over the spill before evacuating. The cob had soaked up most of the product. Our entry was bunker gear with scba. We added some more cob to the spill and swept it up and sealed it in a 5 gallon bucket. The business was returned to the employee group. jds oC,p S In the Event of a Polycarbonate Spill: Attempt to clean up the spill within 10 to 15 minutes per the Polycarbonate Spill Procedures. If it is not possible to clean up within 10 to 15 minutes, or if fumes become too strong, LEAVE THE BUILDING IMMEDIATELY. EMERGENCY EVACUATION PROCEDURE 1. In the event of an evacuation, the Emergency Coordinator will CALMLY direct all associates and customers to leave the store by using the word: "EVACUATE," and will call 911 for the fire department. 2. If possible, turn off all machinery in operation. 3. Proceed to the nearest clear exit as shown on the store's EVACUATION MAP, and exit the area. • 4. The Emergency Coordinator will make sure that all customers and associates have left the store. 5. The Emergency Coordinator will take the Polycarbonate Spill Emergency Folder and the Chemical Safety Manual when leaving the store. 6. If a polycarbonate spill has occurred, the Lab Manager will obtain a copy of the Material Safety Data Sheet (MSDS) from the Polycarbonate Spill Emergency Folder, and accompany associates to the Mall Information Area or the following designated area D1c°.P Aizeitx IN rQGJiT Xb?- NOTE: Stand alone stores will assemble at a designated area in the parking lot, no closer than 300 feet from the building. For malls only: The lab manager will report the following information to the mall management: a. the occurrence of the fire or coating spill, b. the store has been evacuated, or closed, and c. the fire department has been called. 8. Report the emergency to your management. 9. Meet the Fire Department and/or other authorities outside the store. 10. Provide them with a copy of the appropriate MSDS's in the event of polycarbonate spill. 11. When the emergency is over, and "all clear" is given by theresponding fire department, verbally tell associates that it's "OKAY TO RETURN." 12. Resume normal operations in an orderly manner. 13. Refer any media personnel and questions to the Regional Director of Operations, RDO and Regional Quality Coordinator, RQC who in turn will notify the Corporate Communications Department. 14. EYE CONTACT: Causes Eye Irritation. The kidney, Mier and blood may be targeted by components of LTI-HT- 825. Fire and Explosion LTi -HT -825 is a flammable liquid with a flash point of 91 deg. F SETA dosed cup. Both the LEL and UEL have not been determined. Products of combustion include carbon dioxide and carbon mono:dde, nitrogen oxides, acrid fumes and volatile organic fractions. The exact composition of the combustion by-products will depend on the conditions of combustion. In a fire situation, dosed containers may rupture violently when exposed to heat Toxic and/or irrttatiing vapors may be released during a spill. Combustion by.products maybe hazardous. (See Reactivity Section} Signs and Symptoms of Overexposure Symptoms or signs of potential overexposure to LTI-HT-825 or its components may include headache, lethargy, skin rashes; hires, respiratory irritation resulting in coughing or wheezing, disoriented or drunkenness behavior, nausea, vomiting, eye reddening, conjunctivitis, itching, burning or other ktltation of the skin and burning or other irritation of the eye. Eyes: In case of contact, immediately flush eyes with plenty of water for at least 15 minutes. Get medical attention immediately. Skin: In case of contact, immediately wash exposed sldn with plenty of soap and water for at least 15 minutes. Seek medical advice if a rash or irritation persists. Wash doting before reuse. Destroy contaminated shoes and other leather goods or apparel. Inhalation: If affected by inhalation of vapor or spray mist, remove to fresh air. If breathing is difficult, give oxygen. If not breathing, give CPR. Get medical mention immediately. Keep upwind of fine or vapor clouds. 6: Accidental Release Measures SMALL SPILLS (a pint or titre) Cover with absorbent material (Expanded vermiculite or kitty titer). When liquid is Bully absorbed, sweep into a plastic bag. LARGE SPILLS Prevent build-up of vapors, by covering with absorbent material. Pay particular attention to confined areas or storm drains. TURN OFF POWER AT A REMOTE SWITCH PANEL. remember that tumirg equipment ON or OFF will muse an arc for possible ignition of vapor. Use spark -proof tools and conductive shoes to avoid sparking hazards. Dispose of In accordance with all applicable regulations. 7: Handling and Stoma* LTI-HT-825 is flammable and should be stored away from potential ignition sources. Keep away from heat, sparks and (tame. Vapors may ignite explosivNy vapors may spread over long dunces. Close container ager each use. Drums should be grounded while dispensing. Do not get in eyes, on skin or on clothing. Use only with adequate ventilation. 8: Exposure Preteetion In that each work area vanes, it is important that an Industrial Hygiene review be made of the process to establish exposure levels. Appropriate engineering controls should be designed and installed to present navels from exceeding the established TLV's. When engineering controls can not be put in place, ak purifying respiratory are required for exposures up to 10 times the TLV and positive pressure supplied for sir respirators for exposure above 10 tines the TLV. Due to the potential harmful characteristics of LTi -HT -825 cane needs to be taken to prevent skin exposure. Eye protection including safety glasses and /or face shields should be worm when ever the potential exists that LTi -HT -825 could attack the eye. This includes while dispensing or spraying if applicable. Suitable emergency eye wash facilities or other sufficient water supply is required 10 be available for emergency use. Good personal hygiene prate should be followed to prevent accidental consumption. This includes ensuring personnel wash after handling LTi-HT- 825and prior to smoking, eating or consuming tobacco items like chewing tobacco or applying cosmetics.
