Fill out all portions of the Agency Copy. 4. On the Site Copy; a. Fill out the incident number and b. Identify the fire watch frequency c. Circle the appropriate follow-up agency d. Identify which system is out -of -service. 5. Leave the Site copy with the responsible party. 6. Identify the person this form was left with. 1it 7. If the Fire Watch was declined, identify the name of the individual who declined the fire watch. 8. Was the building secure when you left? Yes ❑ No If not, please explain what actions were taken. 9. The system has been designed per NFPA 72, iss edition requirements as well as recently adopted IFC, WAC, UBC, NEC current and your additional requirements. The building is non -sprinkled. The system shall be fully addressable. The system shall monitor by zone activation as well as separate trouble signals. Monitoring shall be provided via UL listed Central Station, The Alarm Center LACEY, WA.,via AES Radio. Audibility shall be in compliance with NFPA standards with a 10 inch bell located on the outside of the building in the top 25% with a 117 candela strobe facing the first in -coming FIRE DEPARTMENT vehicles for easy building location. Interior DB shall be 75 DB minimum throughout. A full service and monitoring agreement is in force for the proper service and maintenance per NFPA requirements. All systems shall be UL certificated and placard. A full pre-test and NFPA form shall be conducted and completed prior to calling the AHJ for acceptance testing. Trouble and supervisory signals shall transmit to CS which will dispatch FPI runner service to site, (no notification circuits shall activate.) On site emergency contact personnel shall be notified. Wet Waterflow ALARMS shall be retarded by a period of not more than 45 seconds. Low air shall activate at 251bs, causing for a supervisory signal. A short on FACP smoke, Waterflow Switches, (wet and dry) and pull station shall cause alarm. Shorts on PIV, tamper and Ai -low air zones shall cause a Supervisory condition. The notification appliance shall also be listed with the California State Fire Marshal (CSFM) and the Bureau of Standards and Appeals (NYC). The notification appliance (combination audible/visible units only) shall produce a peak sound output of 92dBA or greater as measured in an anechoic chamber. The signaling appliance shall also have the capability to silence the audible signal while leav- ing the visible signal energized with the use of a single pair of power wires. Additionally, the user shall be able to select either continuous or temporal tone output with the temporal signal having the ability tobe synchronized. The audible/visible and visible signaling appliance shall also maintain a minimum flash rate of 1Hz or up to 2Hz regardless of power input voltage.
