5555 S 152ND ST ASSOCIATED PERMITS D06-262 D01-071 D02-319 PIM COPY DATE: September 24, 2016 TO: Ted Sitterley; Belfor Property Restoration FROM: Mike Bramhall RE: LaVista Apartments Vehicle Strike Site Visit Report 1404$ IREE FORENSICS Ted, at the September 23, 2016 request of Brian Jones with Belfor Property Restoration (Belfor), I arrived on site at the LaVista Apartments at 5555 S 152nd Street, Tukwila, Washington at about 8:00 pm on September 23, 2016 to observe the structural condition of the building following a reported vehicle strike. The building is a three story wood framed building built 1970. It is approximately 340 feet in the north -south direction and 40 feet in the east -west direction. The building is located on a site that slopes down from north to south. The first floor of the building is approximately 5 feet below the level of the adjacent parking lot to the east. The floor framing on the east side of the building is comprised of east -west oriented 2x10 joists at 16 inches on center. The east ends of the joists cantilever 5 feet past the east wall of the building to provide a walkway for accessing the units on the second and third floors. The walls are comprised of 2x4 studs at 16 inches on center with a stucco finish at the exterior. On September 23, 2016, a flatbed vehicle hauler carrying a passenger car left the parking area, proceeded down the embankment between the parking area and the building, and impacted the building at the entry to Units 34 and 35. In the process, the hauler struck three additional passenger vehicles, one of which was propelled into the bedroom of Unit 33. Observed damage to the building included the following: 1) Impact to the north wall of the bedroom in Unit 35. 2) Impact and displacement of the wall framing in the southeast corner of the bedroom in Unit 34. 3) Displacement of the wall framing at the south jamb of the window in the bedroom of Unit 34. 4) Displacement of the wall framing at the south jamb of the window in the bedroom of Unit 35. 5) Cracking to the nonstructural fascia of the second floor walkway above the Units 34. 6) Cracking and displacement of some of the railing of the second floor walkway above the Units 34. No evidence of damage to the structure was observed west of the east exterior wall. At the time of our September 23, 2016 site visit, shoring comprised of 4x6 and 4x4 elements was installed immediately adjacent to the east wall of the building for south of the entry to Unit 35 to north of the bedroom window in Unit 33. This shoring was then sheathed with''/] inch thick OSB panels. This shoring was designed and installed to reestablish the load path for dead and live loads in the floor framing bearing on the wall. On September 24, 2016, personnel from Belfor returned to the site to install 4x4 posts at damaged or missing king studs at the bedroom windows of Units 33 and 34 and the wall corner in the southeast corner of the bedroom in Unit 34. This shoring was designed and installed to reestablish the load path for dead and live loads from the jambs above. See attached plan and section of shoring location.
