By virtue of The Provisions of the International Fire Code adopted by City of Tukwila Ordinance,
Vascular Access Center of Seattle, LLC located at 14220 Interurban Avenue South, #110
having made application in due form, and as the conditions, surrounding, and arrangements are,
in my opinion, such that the intent of the Ordinance can be observed, authority is hereby given
and the PERMIT is granted *To store, transport on site, dispense, use or handle at normal
temperatures and pressures compressed gases in excess of the amounts listed in Table #105.6.9
per Chapter 30 of International Fire Code 105.6.9.
This PERMIT is issued and accepted on condition that all Ordinance provisions now adopted, or that
may hereafter be adopted, shall be complied with.
THIS PERMIT IS VALID FOR -Eighteen Calendar Months -
This permit does not take the place of any
License required by law and is not transferable.
Any change in use or occupancy of premises
shall require a new Hermit
