Box 3644 Seattle, WA 98124 Re: Industrial Park for Northwest Medical Supply Dear Mr. Shaw: The review of Sheet No. G -1 "The Grading Plan and Preliminary Hydraulics Concept for the proposed Industrial Park for Northwest Medical Supply at S. 134th Street and 44th Place South, and review of the Environmental Checklist form has been provided by the Public Works Department for the purpose of issuance of a grading permit. From this review, we have the following concerns and requests: 1) Environmental Checklist Form, Item Nos. 3b, c, f, g, (water) are indicated impacts by the proposal. To insure these potential impacts can be mitigated, you are requested to provide a hydraulic analysis of the site and receiving storm system by a qualified engineer. 2) Environmental Checklist Form, Item Nos. 13a, b (Transportation /Circulation) are indicated impacts by the proposal and, per this review, Item 13c may also create a significant impact by the proposal. You are requested to provide a traffic,study which includes: a)' additional peak traffic flows generated to north and southbound lanes of Interurban Avenue South b) number of stalls for each type of vehicle c) the study will address Items 13a, b, c and provide solutions to mitigate potential impacts. 3) Plan No. G -1, provide note on plans that all interim drainage on site due to grading activities (prior to installation of drain system) shall be contained within the property lines. Further, required ditching shall be provided which carries drainage to original point of exit on the site. South Seattle, Washington 98124 Dear Sir: RE: Northwest Medical Supply Drainage Permit (limited) As I stated in your phone conversation today, the Public Works Department has reviewed your site fill plan to date only for interim drainage for the Fill Permit issued by the Building Department. Per your request 10/9/80, the Public-:Works Department has reviewed the same site plan submittal for the diverted stream in an open ditch system as a permanent drainage facility. This review is limited to this drainage only. By this review Public Works comments are as follows: 1. A cross section of the ditch swale noting the final configuration of the swale and including materials to be used for the foundation of the swale shall be submitted for Public Works Department review prior to application of the drainage permit through Public Works. 2 3. The property owner shall be requred to provid4low calculations indicating capacity of ditch swale is sufficient to carry minimum 10 year storm flows from tributary drainage basin at full development upstream (current zoning). Property owner shall maintain the diverted stream ditch system in such a manner as to not silt downstream systems, which will include maintenance to the original approved ditch profile to be shown on the plans. Rockeries shall be provided to the final ditch on both sides of the stream throughout the property to protect adjacent properties, and provide a permanent channel. This entire ditch system, including rockeries, shall be in private property of the development. The toe of the rockeries shall be constructed into the existing undisturbed ground 1 foot minimum per the plan. The rock shall be of a hardness, and type of material acceptable to the City's inspector.
