By virtue of The Provisions of the International Fire Code adopted by City of Tukwila Ordinance,
Fatigue Technology, Inc. located at 401 Andover Park East
having made application in due form, and as the conditions, surrounding, and arrangements are, in my
opinion, such that the intent of the Ordinance can be observed, authority is hereby given and the
PERMIT is granted *To use a building or portion thereof as a high -piled storage area exceeding 500
square feet (46m2) per Chapter 32 of International Fire Code 105.6.22. *To conduct hot work per
Chapter 35 of International Fire Code 105.6.23. *For the use, storage or handling of flammable or
combustible liquids in containers and portable storage inside buildings per Chapter 57, sec. 5704.3 -sec.
5704.4 and in cabinets per Chapter 57, sec. 5704.3.2 thru 5704.3.2.2 of International Fire Code 105.6.16.
*For the storage and use of LP -gas and operation of cargo tankers that transport LP -gas per Chapter 61
of International Fire Code 105.6.27. *To produce, store or handle cryogenic fluids in excess of the
amounts listed in table #105.6.10 per Chapter 55 of International Fire Code 105.6.10. *To operate an
industrial baking or drying oven Chapter 30 of International Fire Code 105.6.24. *To conduct a spraying
or dipping operation utilizing flammable or combustible liquids or the application of combustible
powders per Chapter 24 of International Fire Code 105.6.41.
This PERMIT is issued and accepted on condition that all Ordinance provisions now adopted, or that
may hereafter be adopted, shall be complied with.
THIS PERMIT IS VALID FOR -Eighteen Calendar Months -

THIS PERMIT MUST BE POSTED ON THE PREMISES MENTIONED ABOVE.
