Decision on Substantive Permit: The City Community Development Department has determined that the application
for a Type 2 Wireless Communications Facility does comply with applicable City and state code requirements and has
approved that application with the following conditions:
1. Revise Note on Site Plan Sheet A3.0 to state that it is a requirement for antennas and ancillary
equipment to be painted to match the existing structure.
2. A business license is required as listed in TMC 18.58.070(2). Corresponding building permit shall not be
issued until business license is issued.
INFORMATIONAL ITEMS
1. The applicant is required to install emergency shut -down switch for Fire Department use. This can be a
condition of approval of the corresponding building permit, D15-0308. Contact the Tukwila Fire Marshal
at (206)575-4407 with any questions regarding this requirement.
III. YOUR APPEAL RIGHTS
The Decision on this Permit Application is a Type 2 decision pursuant to Tukwila Municipal Code §18.104.010.
Other land use applications related to this project may still be pending.
No administrative appeal of a DNS or an EIS is permitted. One administrative appeal to the City Hearing Examiner of
the Decision on the Permit itself is permitted. If an MDNS was issued, any person wishing to challenge either the
conditions which were imposed by the MDNS decision or the failure of the Department to impose additional
conditions in the MDNS must raise such issues as part of the appeal to the Hearing Examiner.
A party who is not satisfied with the outcome of the Hearing Examiner appeal process may file an appeal in King
County Superior Court from the Hearing Examiner's decision.
IV. PROCEDURES AND TIME FOR APPEALING
In order to appeal the Community Development Director decision on the Permit Application, a written notice of
appeal must be filed with the Department of Community Development within 14 days of the issuance of this
decision.
The requirements for such appeals are set forth in Tukwila Municipal Code ch. 18.116. All appeal materials shall
be submitted to the Department of Community Development. Appeal materials MUST include:
1. The name of the appealing party.
2. The address and phone number of the appealing party` and if the appealing party is a corporation, association
or other group, the address and phone number of a contact person authorized to receive notices on the
appealing party's behalf.

3. A statement identifying the decision being appealed and the alleged errors in the decision, including any
specific challenge to an MDNS.
4. The Notice of Appeal shall identify (a) the specific errors of fact or errors in application of the law in the
decision being appealed; (b) the harm suffered or anticipated by the appellant, and (c) the relief sought. The
scope of an appeal shall be limited to matters or issues raised in the Notice of Appeal.
5. Appeal fee of $602 plus retainer.
V. APPEAL HEARINGS PROCESS
Any administrative appeal regarding the Permit shall be conducted as a closed record hearing before the
Hearing Examiner based on the information presented to the Community Development Director, who made the
original decision. No new evidence or testimony will be permitted during the appeal hearing. Parties will be
allowed to present oral argument based on the information presented to the Community Development Director
before their decision was issued. The Hearing Examiner's decision on the appeal is the City's final decision.
Any party wishing to challenge the Hearing Examiner's decision on this application must file an appeal pursuant
to the procedures and time limitations set forth in RCW ch. 36.70C. An appeal challenging a DNS, an MDNS or
an EIS may be included in such an appeal. If no appeal of the Hearing Examiner's decision is properly filed in
Superior Court within such time limit, the Decision on this permit will be final. The City's decision to issue a DNS,
an MDNS or an EIS is final for this permit and any other pending permit applications for the development of the
subject property.
VI. INSPECTION OF INFORMATION ON THE APPLICATION
Project materials including the application, any staff reports, and other studies related to the permits are available for
inspection at the Tukwila Department of Community Development, 6300 Southcenter Blvd., Suite 100, Tukwila,
Washington 98188 from Monday through Friday between 8:30 a.m. and 5:00 p.m. The project planner is Laura

information.
Property owners affected by this decision may request a change in valuation for their property tax purposes. Contact

FINDINGS
Vicinity/Site Information
Project Description
The applicant is requesting approval of a Type II Wireless Telecommunication Facilities [WCF] Permit to replace
and add equipment on an existing non -concealed building mounted facility at 16400 Southcenter Parkway. The
project includes:
• Removal of 2 existing panel antennas and addition of three new panel antennas.
• Addition of 1 FRIG RRU radio.
• Addition of 1 FXFC RRU radio.
• Addition of cabinet equipment including 6 diplexers, 2 FSMF, 2 FBBC, and 1 ESMB.
The existing non -concealed building mounted cellular facility extends beyond the roofline. The proposed
modifications do not alter the existing height and is below the 85 foot maximum height without incentives in the
TUC -RC.
Existing Development
This non -concealed building mounted wireless facility is located on the Legacy Southcenter Place, an office
building located in the Southcenter (Tukwila Urban Center) area.
Surrounding Land Uses
The subject property is zoned TUC -RC. The surrounding parcels to the south and east are zoned Tukwila Urban
Center — Pond (TUC-P). The property fronts Southcenter Parkway. To the west of Southcenter Parkway, the
parcels are zoned Tukwila Urban Center — Commercial Corridor (TUC -CC). To the north, across Strander
Boulevard, the properties are also zoned TUC -RC.
BACKGROUND
In 2006, the City adopted a Wireless Facilities Chapter (TMC 18.71) to the Zoning Code. The chapter provides
specific location and design requirements for the placement of WCF within the City of Tukwila.
The WCF being modified was originally approved by the City in 1998 (L98-0016) through a conditional use
permit.
In 1998 the WCF was approved for modifications (L98-0023) through a conditional use permit. In 2008 the WCF
was approved for modifications (L08-047) through a Type 2 Wireless Facilities permit.
Location Selection
Tukwila Municipal Code (TMC) provides a hierarchy of appropriate locations within the City. Generally, co -
locations and utilizing existing structures are favored over the development of new standalone WCF within the
City.

located on an existing platform.

CONCLUSIONS
1. The proposed modifications to the existing WCF will not significantly alter the conditions that currently
exist and will not be materially detrimental to the public welfare or injurious to property or
improvements in the vicinity.
2. As demonstrated in Tables 1 and 2, the project meets all applicable development standards for a Type 2
WCF.
3. The proposed modifications will not alter the surrounding land uses in terms of traffic and pedestrian
circulation, building or site design.
4. The proposed project is in keeping with the goals and policies of the Comprehensive Land Use Policy
Plan.
RECOMMENDATIONS
The recommendation to the Director of Community Development is to approve this application for a Type I
Wireless Telecommunication Facility permit as applied for by Centerline Solutions for T-MOBILE with the
following conditions:
1. Revise Note on Site Plan Sheet A3.0 to state that it is a requirement for antennas and ancillary
equipment to be painted to match the existing structure.
2. A business license is required as listed in TMC 18.58.070(2). Corresponding building permit shall not be
issued until business license is issued.

This modification meets the Tukwila Municipal Code §18.58 requirements for a Type II Wireless
Modification. This modification is exempt from SEPA per WAC 197-11-800 (25) (a)(i). The site does not
include any sensitive areas per NWMaps.net and all equipment is on the roof. No generator is located
at the site and none is proposed. The site is unscreened so a screening board is not applicable. The site
is on a rooftop so a landscaping plan is not applicable. Antennas will be painted to match the building

18.58.070 General Requirements. The following shall apply to all wireless communication facilities
regardless of the type of facility:
1. Noise — Any facility that requires a generator or other device which will create noise must
demonstrate compliance with TMC Chapter 8.22, "Noise". A noise report, prepared by an acoustical
engineer, shall be submitted with any application to construct and operate a wireless communication
facility that will have a generator or similar device. The City may require that the report be reviewed by
a third party expert at the expense of the applicant.
Applicant Response: This site does not include a generator and none is proposed.
2. Business license requirement — Any person, corporation or entity that operates a wireless
communication facility within the City shall have a valid business license issued annually by the City. Any
person, corporation or other business entity which owns a tower also is required to obtain a business
license on an annual basis.
Applicant Response: A business license application is pending.
3. Signage — Only safety signs or those mandated by other government entities may be located on
wireless communication facilities. No other types of signs are permitted on wireless communication
facilities.
Applicant Response: Acknowledged. Only signs mandated by government entities will be located on
the WCF.
4. Parking — Any application must demonstrate that there is sufficient space for temporary parking for
regular maintenance of the proposed facility.
Applicant Response: The site is located on top of an office building surrounded by ample parking.
5. Finish — A tower shall either maintain a galvanized steel finish or, subject to any applicable standards
of the FAA or FCC, be painted a neutral color so as to reduce its visual obtrusiveness. TITLE 18 — ZONING

6. Design —The design of all buildings and ancillary structures shall use materials, colors, textures,
screening and landscaping that will blend the tower facilities with the natural setting and built
environment.

7. Color — All antennas and ancillary facilities located on buildings or structures other than towers shall
be of a neutral color that is identical to or closely compatible with the color of the supporting structure
so as to make the antenna and ancillary facilities as visually unobtrusive as possible.
Applicant Response: Antennas will be painted to match the building in a neutral color.
8. Lighting — Towers shall not be artificially lighted unless required by the FAA, FCC or other applicable
authority. If lighting is required, the reviewing authority shall review the lighting alternatives and
approve the design that would cause the least disturbance to the surrounding areas. No strobe lighting
of any type is permitted on any tower.
Applicant Response: No lighting is proposed or required by the FAA or FCC.
9. Advertising — No advertising is permitted at wireless communication facility sites or on any ancillary
structure or facilities equipment compound.
Applicant Response: There is no existing or proposed advertising.
10. Equipment Enclosure — Each applicant shall be limited to an equipment enclosure of 360 square feet
at each site. However, this restriction shall not apply to enclosures located within an existing
commercial, industrial, residential or institutional building.
Applicant Response: The equipment is on the roof of a building, there is no Equipment Enclosure.
18.58.110 Non -concealed Building Mounted Development Requirements. The following requirements
shall apply:
1. Height — The proposed facility must meet the height requirements of the applicable zoning category.
If the building where the facility is located is at or above the maximum height requiirements, the
antennas are permitted to extend a maximum of 3 feet above the existing roof line. Non -concealed
building mounted facilities shall not qualify as "Rooftop Appurtenances" under TMC 18.50.080.
Applicant Response: The existing facility meets the 85' TUC -RC height limit and no height increase is
proposed.
2. Antenna aesthetics — The first preference for any proposed facility is to utilize flush -mounted
antennas. Nonflushmounted antennas may be used when their visual impact will be negated by the
scale of the antennas to the building. "Shrouds" are not required unless they provide a better visual
appearance than exposed antennas. Antennas shall be painted to match the color scheme of the
building(s).

3. Feed lines and coaxial cables — Feed lines and cables should be located below the parapet of the
rooftop. If the feed lines and cables must be visible, they must be painted to match the color scheme of
the building(s).
Applicant Response: All cables will be located below the parapet of the rooftop.
4. Cabinet equipment — If cabinet equipment cannot be located within the building where the wireless
communication facilities will be located, then it must be located on the rooftop of the building. If the
equipment can be screened by placing the equipment below the parapet walls, no additional screening
is required. If screening is required, then the proposed screening must be consistent with the existing
building in terms of color, style, architectural style and material. If the cabinet equipment is to be
located on the ground, the equipment must be fenced with a 6-foot-tall fence and materials shall be
used to screen the equipment from view. Barbed wire may be used in the TVS, LI, HI, MIC/L, and MIC/H
zones. (Ord. 2135 §1 (part), 2006)

Executive Summary
This report provides the results of an RF power density analysis performed for T-Mobile
at Site SE04000B.
Current Site Configuration:
Three sector site operating with technology including GSM in PCS band (1900 MHz),
UMTS in PCS (1900 MHz band), UMTS in AWS (2100 MHz), LTE in AWS band (2100
MHz), and LTE in the 700 MHz band.
Proposed Site Configuration:
Modification in the current Gamma sector configuration and applying a sector split. Four
sector site operating with technology including GSM in PCS band (1900 MHz), UMTS in
PCS (1900 MHz band), UMTS in AWS (2100 MHz), LTE in AWS band (2100 MHz),
and LTE in the 700 MHz band.
Scope of Work:
The proposed physical modifications will result in replacing antennas and radio hardware
at this site.
Rooftop Equipment Modifications:
• Removal of (3) existing panel antennas
• Addition of (3) panel antennas
• Addition of (1) FRIG
• Addition of (1) FXFC
• Addition of (8) Diplexer
• Addition of (2) FSMF (2) FBBC, & (1) ESMB in (E) cabinet
Site Description:
• Site Type and Height- Rooftop, elevation of 73.0 ft. AGL
• Equipment Location- Adjacent to the antennas.
• Other Antennas- There are 8 Verizon antennas and a microwave dish. Also, there
are several satellite dishes.
• Access- The only access to the site is through the locked door leading to the
rooftop.
• Study Reference Location- This study examines the power density environment at
ground, rooftop, and penthouse levels from ALL sources of RF radiation, except
for those antennas specifically excluded as described in the Introduction section.
• Existing Signage- There are blue RF signs and a yellow RF guideline sign placed
at the interior side of the door that leads to the rooftop of the building. Also, there
are barriers with blue Notice signs around Verizon's antennas.

• RF Environment- For purposes of evaluating compliance with FCC OET Bulletin
65, the area on the ground level is considered to be an uncontrolled/public
environment. However, the areas on the rooftop and penthouse levels are
considered to be controlled/occupational environments.
Analysis:
The analysis of the RF power density at this site indicates that emissions on the ground
will not exceed the FCC's MPE limit for General Public environments. T-Mobile's
contribution to the rooftop level exposures is calculated to be less than 28.2% of the FCC
MPE limit for Occupational/Controlled environments. The rooftop level exposures near
Verizon's Beta sector antennas are calculated to be less than 122.9% of the FCC MPE
limit for Occupational/Controlled environments. However, Verizon has mitigated the
risk of accidental exposure by placing barriers and RF signs.
Conclusion and Recommendations:
• The results of the analysis indicate that the power density levels at the ground will not
exceed the FCC MPE General Population limit with the proposed antenna plan.
• The results of the analysis indicate that the power density levels on the main building
rooftop contributed by T-Mobile will not exceed the FCC MPE Occupational limit with
the proposed antenna plan.
• The results of the analysis indicate that the power density levels on the north penthouse
(Penthouse 2) may exceed the FCC MPE Occupational limit with the proposed antenna
plan. However, Penthouse 2 is not generally accessible.
• Centerline Solutions recommends placing blue RF signs on the wall of the south
penthouse (Penthouse 1), adjacent to the antennas of the Gamma sector. In
addition, Centerline Solutions recommends placing yellow RF signs near the top of
the roof and on each side of the north penthouse (Penthouse 2). See Exhibits 5 and 6
for recommended signage locations.
• By taking the above mentioned recommended actions, this site will operate in general
compliance with the FCC's OET Bulletin 65.

Introduction
The purpose of this analysis and report is to evaluate the cumulative power density levels
of all antennas located on the building rooftop and identify any areas of concern that
require mitigation. This report also assesses the rooftop's compliance with FCC OET
Bulletin 65; "Guidelines for Human Exposure to Radio -frequency Electromagnetic
Fields".
As part of this analysis, the site was visited on 11/17/2015 to perform a complete field
survey. The purpose of the survey was to verify antennas located on the building rooftop
and identify co -located wireless facilities.
The power density simulation performed for this site utilized RoofView® analysis
software. All antennas were assigned an operating frequency and transmit power and
were deemed to be operating at 100% of their rated output power. The microwave dish
antenna located on Verizon's Alpha sector and other satellite dishes were not included in
the analysis, as their beamwidth is very narrow, and very little of their radiated energy is
propagated to the rooftop level below the antenna.

Predictive Analysis Details
For purposes of this analysis, RoofView® was configured to provide an output based on
the appropriate MPE limit(s) published in the FCC's guidelines. The antenna
information was loaded into RoofView®, an MPE predictive analysis tool by Richard
Tell and Associates, Inc. The analysis was run for the ground, building rooftop, and
penthouse levels.
The following exhibits show the calculated RF emissions at these levels. Exhibit 4 shows
the RF emissions at ground level, Exhibit 5 shows the RF emissions at the rooftop level,
and Exhibit 6 shows the RF emissions at the penthouse level. The solid green shading
indicates that power density levels are expected to be less than 5% of the applicable MPE

Appendix A: FCC Compliance and RF Safety Policies
In August of 1997, the FCC published OET Bulletin 65 Edition 97-01 to regulate
methods for evaluating compliance with FCC guidelines for human exposure to
radiofrequency (RF) electromagnetic fields. The FCC guidelines for human exposure to
RF electromagnetic fields incorporate two categories of limits; namely "Controlled"
(a.k.a. Occupational) and "Uncontrolled" (a.k.a. General Public). The guidelines offer
suggested methods for evaluating fixed RF transmitters to insure that the controlled and
uncontrolled limits deemed safe by the FC for human exposure are not exceeded.
OET Bulletin 65 recommended guidelines are intended to allow an applicant to "make a
reasonably quick determination as to whether a proposed facility is in compliance with
the limits." In addition, the guidelines offer alternate supplementary considerations and
procedures such as field measurements and more detailed analysis that should be used for
multiple emitter situations.
These guidelines define RF as emissions in the frequency range of 300 kHz to 100 GHz.
The FCC define Maximum Permissible Exposure (MPE) limits within this frequency
range based on limits recommended by the National Council on Radiation Protection and
Measurement, the Institute of Electrical and Electronics Engineers (IEEE), and by the
American National Standards Institute (ANSI).

For General Population/Uncontrolled limits, the FCC states that "General Population/
Uncontrolled exposures apply in situations in which the general public may be exposed,
or in which persons that are exposed as a consequence of their employment may not fully
be aware of the potential for exposure or cannot exercise control over their expos ire."
For purposes of this analysis, all limits are evaluated against the Power Density limits.
Typical guidelines for determining whether Occupational/Controlled limits can be
applied include insuring the environment (such as a rooftop) as limited/controlled access
via locked doors or physical barrier that are preferably controlled by a landlord that is
aware of the situation and can inform anyone going through the locked door of the
existence of the RF emissions. Such notification/awareness is typically accomplished by
means of signage on the door, or other access to the area of concern, as well as signage

Appendix B: Overview of RoofView® Functions and
Assumptions
RoofView® is a tool developed and supported by Richard Tell Associates, Inc. to be
used for analysis of RF field levels at telecommunications sites produced by antennas of
the type commonly used in cellular, paging, SMR, PCS and two-way radio
communications services. Although its name suggests that the tool is only for use in
evaluating emissions for roof top applications, it can also be used to evaluate ground
level effects of tower facilities.
RoofView® allows the user to apply near field, far field, or a combination of near and far
field computational methods as desired by the user. For this analysis, near field
computations are used for areas within the near field, and far field computations are used
beyond the near field. Specific break points are dynamic based on the aperture of the
antenna being analyzed.
The near field methodology is based on a cylindrical model that assumes the power into
an antenna is distributed as a cylinder around the aperture of the antenna. Research by
Richard Tell Associates, Inc. found that using such a model, along with corrections for
height and antenna pattern, is very accurate, if not slightly conservative in estimating RF
exposure. FCC Bulletin 65 recognizes the use of the cylindrical model for near field
calculations. The following picture and corresponding equation summarizes the
computations used by RoofView® on a bin -by -bin basis when the near field method is
used:

Each bin's results are then also adjusted by spatially averaging the portion of a 6 foot tall
human that intercepts the aperture over 6 feet. Once the antenna is completely above (or
below) the height that corresponds to a 6 foot tall human, the cylindrical results are

reduced to 10% of their results and then dissipated inversely in proportion to the square
of the distance.
Once bins being analyzed fall outside of the near field (as determined by a method and
variable that is user -selectable; see below for method and variable used in this analysis), a
far -field spatial average is calculated. Spatially averaged power density in the far -field is
calculated by reducing the spatially averaged power density inversely, by the square of
the distance from the antenna(s).
There are several input variables to RoofView® that can impact the results produced
when evaluating specific cell sites. Those variables are summarized accordingly:

i) Is a microcell and is to be attached to an existing structure that is not a residence or school and

ii) Includes personal wireless antennas, other than a microcell, and is to be attached to an existing
structure (that may be an existing tower) that is not a residence or school and does not contain a
residence or school, and the existing structure to which it is to be attached is located in a
commercial, industrial, manufacturing, forest or agriculture zone; or
iii) Involves constructing a personal wireless tower less than sixty feet in height that is located in a
commercial, industrial, manufacturing, forest, or agricultural zone.

7. A letter from the applicant describing the proposed project and describing compliance with the
City's Code requirements.
8. Sensitive Area studies and proposed mitigation (if required).
9. If an outdoor generator is proposed, a report prepared by an acoustical engineer demonstrating
compliance with TMC Chapter 8.22, "Noise".
If the application requires a Type 2 review these additional items are needed:

11. Materials board for the screening material;

13. Letter from a radio frequency engineer that demonstrates that the facility meets Federal
requirements for allowed emissions;

explaining the need for the proposed wireless communication facility at that location. Additionally, the
applicant shall provide detailed discussion on why the wireless communication facility cannot be located
within a commercial or industrial zone.

15. A narrative responding to the criteria at TMC Section 18.58.060.
16. The radio frequency engineer report shall include a discussion of the information required under
TMC Section 18.58.070. The report shall also explain why a tower must be used instead of any of the
other location options outlined in Table A.
17. Engineering Plans for the proposed tower.
18. A vicinity map depicting the proposed extent of the service area.
19. A graphic simulation showing the appearance of the proposed tower and ancillary structures and
ancillary facilities from five points within the impacted vicinity. Such points are to be mutually agreed
upon by the Director of DCD and applicant. All plans and photo simulations shall include the maximum
build -out of the proposed facility.
20. Evidence of compliance with Federal Aviation Administration (FAA) standards for height and
lighting and certificates of compliance from all affected agencies.
21. Evidence that the tower has been designed to meet the minimum structural standards for wireless
communication facilities for a minimum of three providers of voice, video or data transmission services,
including the applicant, and including a description of the number and types of antennas the tower can
accommodate.
PUBLIC NOTICE MATERIALS:
22. Payment of notice board fee (see Land Use Fee schedule) to FastSigns Tukwila OR provide a 4' x 4'
public notice board on site within 14 days of the Department determining that a complete application has
been received (see Public Notice Sign Specifications Handout).
23. Pay the fee as established by the Land Use Fee Schedule for generating mailing labels; OR provide an
excel spreadsheet of mailing labels for all property owners and tenants (residents and businesses) within
500 feet of the subject property. Each unit in multiple family buildings e.g. apartments, condos, trailer
parks --must be included.
Once your project is assigned to a planner, you will be required to provide an electronic copy of the mailing

GENERAL NOTES
1. THE FACILITY IS AN UNOCCUPIED SPECIALIZED MOBILE RADIO FACILITY.
2. PLANS ARE NOT TO BE SCALED AND ARE INTENDED TO BE A GRAPHIC REPRESENTATION OF THE FINAL
INSTALLATION. THE WORK SHALL INCLUDE FURNISHING MATERIALS, EQUIPMENT, APPURTENANCES AND
LABOR NECESSARY TO COMPLETE ALL INSTALLATIONS AS INDICATED ON THE DRAWINGS.
3. PRIOR TO THE SUBMISSION OF BIDS, THE CONTRACTOR SHALL VISIT THE JOB SITE AND BE RESPONSIBLE
FOR ALL CONTRACT DOCUMENTS, FIELD CONDITIONS, AND DIMENSIONS AND CONFIRMING THAT THE
WORK MAY BE ACCOMPLISHED AS SHOWN PRIOR TO PROCEEDING WITH CONSTRUCTION. ANY
DISCREPANCIES ARE TO BE BROUGHT TO THE ATTENTION OF THE PROJECT MANAGER.
4. THE CONTRACTOR SHALL RECEIVE, IN WRITING, AUTHORIZATION TO PROCEED BEFORE STARTING WORK.
5. THE CONTRACTOR SHALL INSTALL ALL EQUIPMENT AND MATERIALS IN ACCORDANCE WITH
MANUFACTURERS RECOMMENDATIONS UNLESS SPECIFICALLY OTHERWISE INDICATED OR WHERE
LOCAL CODES OR REGULATIONS TAKE PRECEDENCE.
6. ALL WORK PERFORMED AND MATERIALS INSTALLED SHALL BE IN STRICT ACCORDANCE WITH
APPLICABLE CODES, REGULATIONS, AND ORDINANCES. CONTRACTOR SHALL GIVE ALL NOTICES AND
COMPLY WITH ALL LAWS, ORDINANCES, RULES, REGULATIONS, AND LAWFUL ORDERS OF ANY PUBLIC
AUTHORITY BEARING ON THE PERFORMANCE OF THE WORK. MECHANICAL AND ELECTRICAL SYSTEMS
SHALL BE INSTALLED IN ACCORDANCE WITH ALL APPLICABLE MUNICIPAL AND UTILITY COMPANY
SPECIFICATIONS AND LOCAL AND STATE JURISDICTIONAL CODES, ORDINANCES, AND APPLICABLE
REGULATIONS.
7. THE CONTRACTOR SHALL SUPERVISE AND DIRECT THE WORK USING BEST SKILLS AND ATTENTION. THE
CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR ALL CONSTRUCTION MEANS, METHODS,
TECHNIQUES, SEQUENCES, AND PROCEDURES AND FOR COORDINATING ALL PORTIONS OF THE WORK
UNDER THE CONTRACT INCLUDING CONTACT AND COORDINATION WITH THE PROJECT MANAGER AND
WITH LANDLORD'S AUTHORIZED REPRESENTATIVE.
8. PROVIDE A PORTABLE FIRE EXTINGUISHER WITH A RATING OF NOT LESS THAN 2-A OR 2-A10BC WITHIN 5
FEET OF TRAVEL DISTANCE TO ALL PORTIONS OF THE PROJECT AREA DURING CONSTRUCTION.
9. THE CONTRACTOR SHALL PROVIDE SITE FOREMAN WITH A CELLULAR PHONE, AND KEEP SAME ON SITE
WHENEVER ANY PERSONNEL ARE ON SITE.
10. DETAILS ARE INTENDED TO SHOW END RESULT OF DESIGN. MINOR MODIFICATIONS MAY BE REQUIRED
TO SUIT JOB DIMENSIONS OR CONDITIONS, AND SUCH MODIFICATIONS SHALL BE INCLUDED AS PART OF
THE WORK.
11. THE CONTRACTOR SHALL MAKE NECESSARY PROVISIONS TO PROTECT EXISTING IMPROVEMENTS,
PAVING, CURBS, GALVANIZED SURFACES, ETC., AND UPON COMPLETION OF WORK, REPAIR ANY DAMAGE
THAT OCCURRED DURING CONSTRUCTION TO THE SATISFACTION OF THE PROJECT MANAGER AND/OR
LANDLORD.
12. ON A DAILY BASIS: KEEP GENERAL AREA CLEAN, HAZARD FREE, AND DISPOSE OF ALL DIRT, DEBRIS,
RUBBISH, AND REMOVE EQUIPMENT NOT SPECIFIED AS REMAINING ON THE PROPERTY. LEAVE PREMISES
IN CLEAN CONDITION AND FREE FROM PAINT SPOTS, DUST OR SMUDGES OF ANY NATURE.
13. CONTRACTOR TO PROVIDE COMPLETE SET OF AS -BUILT DRAWINGS WITHIN 10 WORKING DAYS OF
PROJECT COMPLETION.
14 WHERE A CONSTRUCTION DETAIL IS NOT SHOWN OR NOTED, THE DETAIL SHALL BE THE SAME AS FOR
OTHER SIMILAR WORK.
15. ASTM SPECIFICATIONS NOTED ON THE DRAWINGS SHALL BE OF THE LATEST REVISION.
16 IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE ALL EXISTING UTILITIES WHETHER
SHOWN HEREON OR NOT AND TO PROTECT THEM FROM DAMAGE. THE CONTRACTOR SHALL BEAR ALL
EXPENSE OF REPAIR OR REPLACEMENT IN CONJUNCTION WITH THE EXECUTION OF THIS WORK.
17. ALL ITEMS REMOVED OR DAMAGED DURING CONSTRUCTION WORK WILL BE REPLACED OR REPAIRED TO
MATCH EXISTING.
18. ALL ELEMENTS OF EXISITNG STRUCTURE TO REMAIN INDISTURBED, UNO. EXISTING STRUCTURE IS
ASSUMED TO BE IN GOOD CONDITION, FREE OF DAMAGE OR DETEREORATION. CONTRACTOR TO VERIFY
ALL ELEMENTS OF EXISTING STRUCTURE AFFECTED BY THIS MODIFICATION AND NOTIFY FOR IF ANY
DAMAGE, DETEREORATION, OR DISCREPANCIES BETWEEN THE EXISTING CONDITIONS AND THOSE
DEPICTED ON THESE CONSTRUCTION DRAWINGS ARE FOUND.
SPECIAL INSPECTION
1. IF REQUIRED, SPECIAL INSPECTIONS SHALL BE PERFORMED BY AN INDEPENDENT SPECIAL
INSPECTOR.
2. THE SPECIAL INSPECTOR SHALL PROVIDE A COPY OF THEIR REPORT TO THE OWNER, ARCHITECT,
STRUCTURAL ENGINEER, CONTRACTOR, AND BUILDING OFFICIAL AS EACH TEST IS COMPLETED.
3. ANY MATERIAL WHICH FAILS TO MEET THE PROJECT SPECIFICATIONS SHALL IMMEDIATELY BE
BROUGHT TO THE ATTENTION OF THE ARCHITECT AND THE STRUCTURAL ENGINEER. SPECIAL
INSPECTION TESTING REQUIREMENTS APPLY EQUALLY TO ALL BIDDER DESIGNED COMPONENTS.
4. INSPECTION FOR PREFABRICATED CONSTRUCTION SHALL BE THE SAME AS FOR THE MATERIAL USED
IF THE CONSTRUCTION TOOK PLACE ON SITE. CONTINUOUS INSPECTION WILL NOT BE REQUIRED
DURING PREFABRICATION IF THE APPROVED AGENCY CERTIFIES THE CONSTRUCTION AND
FURNISHES EVIDENCE OF COMPLIANCE.
5. THE SPECIAL INSPECTOR SHALL SUBMIT A FINAL REPORT CERTIFYING THAT ALL WORK REQUIRING
SPECIAL INSPECTION WAS, TO THE BEST OF THE SPECIAL INSPECTOR'S KNOWLEDGE IN
CONFORMANCE WITH THE APPROVED PERMIT PLANS AND SPECIFICATIONS AND THE APPLICABLE
WORKMANSHIP PROVISIONS.
STRUCTURAL NOTES
GENERAL STRUCTURAL NOTES:
1. WHERE A CONSTRUCTION DETAIL IS NOT SHOWN OR NOTED, THE DETAIL SHALL BE THE SAME AS
FOR OTHER SIMILAR WORK.
2. NOTES AND DETAILS ON DRAWINGS SHALL TAKE PRECEDENCE OVER GENERAL NOTES.
3. NO PIPES, DUCTS, SLEEVES, CHASES, ETC., SHALL BE PLACED IN SLABS, BEAMS, OR WALLS UNLESS
SPECIFICALLY SHOWN OR NOTED, NOR SHALL ANY STRUCTURAL MEMBER BE CUT FOR PIPES,
DUCTS, ETC., UNLESS OTHERWISE NOTED. CONTRACTOR SHALL OBTAIN PRIOR APPROVAL FOR
INSTALLATION OF ANY ADDITIONAL PIPES, DUCTS, ETC.
4. CONTRACTOR AGREES THAT HE SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY FOR JOB
SITE CONDITIONS DURING THE COURSE OF CONSTRUCTION OF THIS PROJECT INCLUDING SAFETY
OF ALL PERSONS AND PROPERTY; THAT THIS REQUIREMENT SHALL APPLY CONTINUOUSLY AND
NOT BE LIMITED TO NORMAL WORKING HOURS; AND THAT THE CONTRACTOR SHALL DEFEND,
INDEMNIFY AND HOLD T-MOBILE AND THE ARCHITECT/ENGINEER HARMLESS FROM ANY AND ALL
LIABILITY, REAL OR ALLEGED, IN CONNECTION WITH THE PERFORMANCE OF WORK ON THIS
PROJECT, EXCEPTING FOR LIABILITY ARISING FROM THE SOLE NEGLIGENCE OF T-MOBILE OR THE
ARCHITECT/ENGINEER.
5. THE CONTRACT DRAWINGS AND SPECIFICATIONS REPRESENT THE FINISHED STRUCTURE. THEY DO
NOT INDICATE THE METHOD OF CONSTRUCTION. THE CONTRACTOR SHALL PROVIDE ALL MEASURES
NECESSARY TO PROTECT THE STRUCTURE, WORKERS, AND PEDESTRIANS DURING
CONSTRUCTION. SUCH MEASURES SHALL INCLUDE, BUT NOT BE LIMITED TO BRACING, SHORING
FOR LOADS DUE TO CONSTRUCTION EQUIPMENT, TEMPORARY STRUCTURES, AND PARTIALLY
COMPLETED WQRK, ETC. OBSERVATION VISITS TO THE SITE BY THE ARCHITECT/ENGINEER SHALL
NOT INCLUDE INSPECTION OF SUCH ITEMS.
6. ASTM SPECIFICATIONS NOTED ON THE DRAWINGS SHALL BE OF THE LATEST REVISION.
7. CONSTRUCTION MATERIALS SHALL BE SPREAD OUT IF PLACED ON FRAMED FLOOR OR ROOF. LOAD
SHALL NOT EXCEED THE DESIGN LIVE LOAD PER SQUARE FOOT. PROVIDE ADEQUATE
SHORING/BRACING WHERE STRUCTURE HAS NOT ATTAINED DESIGN STRENGTH.
8. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE ALL EXISTING UTILITIES
WHETHER SHOWN HEREON OR NOT AND TO PROTECT THEM FROM DAMAGE. THE CONTRACTOR
SHALL BEAR ALL EXPENSE OF REPAIR OR REPLACEMENT IN CONJUNCTION WITH THE PROSECUTION
OF THIS WORK.
9. DIMENSIONS SHALL TAKE PRECEDENCE OVER SCALES SHOWN ON DRAWINGS.
10. THESE NOTES SHALL BE CONSIDERED A PART OF THE WRITTEN SPECIFICATIONS.
11. ALL ITEMS REMOVED DURING CONSTRUCTION WORK (I.E., DRYWALL, PLYWOOD, CEILING PANELS,
ETC.) SHALL BE REPLACED TO MATCH EXISTING.
THE FOLLOWING REQUIREMENTS SHALL BE MET FOR SPECIAL INSPECTION:
A. THE SPECIAL INSPECTOR SHALL BE UNDER THE SUPERVISION OF A REGISTERED PROFESSIONAL
ENGINEER.
B. THE SPECIAL INSPECTOR SHALL FURNISH INSPECTION REPORTS TO THE ARCHITECT/ENGINEER,
AND OTHER DESIGNATED PERSONS. ALL DISCREPANCIES SHALL BE BROUGHT TO THE IMMEDIATE
ATTENTION OF THE CONTRACTOR FOR CORRECTION; THEN, IF UNCORRECTED, TO THE PROPER
DESIGN AUTHORITY AND THE BUILDING OFFICIAL.
C. THE SPECIAL INSPECTOR SHALL SUBMIT A FINAL REPORT SIGNED BY BOTH HE AND HIS
SUPERVISOR STATING WHETHER THE WORK REQUIRING SPECIAL INSPECTION WAS IN
CONFORMANCE WITH THE APPROVED PLANS AND SPECIFICATIONS AND THE APPLICABLE
WORKMANSHIP PROVISIONS OF THE CODE.
D. COMPLY WITH ALL CITY SPECIAL INSPECTION REQUIREMENTS.

4. COAXIAL CABLE SPECIFICATIONS REQUIRE CABLE SUPPORT EVERY
3'-0" ON CENTER. CONTRACTOR SHALL SUPPLY SUPPORTS AS
REQUIRED TO MEET THIS REQUIREMENT.
5. VERTICAL CONNECTIONS SHALL BE TAPED FROM THE BOTTOM UP SO
OVERLAP MOVES WATER AWAY FROM CONNECTION (SEE STEP 4).
6. PROVIDE HEAT SHRINK IN PLACE OF TAPE FOR QUAD POLES & TMAS.
7. ALL ANTENNAS & COAXIAL ANTENNA CABLE TO BE FURNISHED BY
T-MOBILE AND INSTALLED BY CONTRACTOR.
8. COAX SHALL BE TAGGED WITH COLOR CODING AT (2) PLACES USING 1"
WIDE WEATHERPROOF COLORED VINYL TAPE AT THE FOLLOWING
LOCATIONS:
#1 - AT ANTENNA CONNECTION
#2 -AT ENTRY TO EQUIPMENT CABINET
9. RUN COAXIAL CABLE WITH MINIMUM 12" SLACK & 12" FROM EDGE OF
EQUIPMENT CABINETS, ACROSS WAVE GUIDE BRIDGE (IF APPLICABLE),
UP TO TOWER LEG (IF APPLICABLE), AND DISTRIBUTE TO EACH
ANTENNA DEVICE. FURNISH AND INSTALL A MINIMUM OF (3) GROUND
KITS PER COAXIAL CABLE. VERIFY NUMBER OF ANTENNAS, CABLE, AND
CABLE DIAMETER WITH T-MOBILE CONSTRUCTION MANAGER.
