Application and plans must be complete in order to be accepted for plan review. VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is subject to possible revision by the Building Division to comply with current fee schedules. BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the permit will be required as part of this submittal. EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition). Appendix A W -6774 Page 1 W -6774 APPENDIX A SUBSURFACE EXPLORATION PROCEDURES AND LOGS SUBSURFACE EXPLORATION The field exploration program conducted for this study consisted of advancing 16 hollow stem auger borings (B -1 through B -16) and installing 7 monitoring wells (MW -1 through MW -7) at the approximate locations shown on the Site and Exploration Plan, Figure 1. These locations were obtained in the field by taping and pacing from existing structures. ( The hollow stem auger borings completed for Chevron were accomplished and continuously logged by a representative from our firm on 7 and 8 June 1990 and 26 July 1990. 1 . SOIL SAMPLING PROCEDURES �. Soil samples were obtained using the Standard Penetration Test (SPT) procedure. The SPT procedure involves driving a 2 -inch O.D. steel split spoon sampler approximately 18 inches into the undisturbed soil below the auger bit. Blow counts are recorded for every 6 -inch drive interval to allow determination of relative soil density. The sampler is considered to have met refusal if the blow count exceeds 50 blows for any 6 -inch interval. The soil samples collected from the hollow stem auger borings were obtained using P g 9 g procedures designed to minimize the risk of cross contamination. Prior to each boring and between each sample interval, the drilling equipment was cleansed by steam cleaning and between each soil sample interval the sampling tools were cleaned with • 1 _ distilled water and a scrub brush. The soil samples were collected from the split spoon sampler, logged in the field and immediately transferred to laboratory prepared glass containers fitted with Teflon lined lids and an aluminum foil seal. All soil samples were stored and transported in a chilled cooler in accordance with standard RZA chain of custody procedures. • , 1., • • �. • . Prior to being placed in the chilled cooler, the sample jars containing the soil were allowed to equilibrate to ambient temperature and subjected to field headspace measurements. Field headspace measurements involve piercing the aluminum foil seal with the probe of a portable photoionization meter to obtain a measurement of the organic vapor concentration in the head space of the sample jar. This method is used . t as a field screening technique for the presence of petroleum hydrocarbons but cannot 1 . be used to identify specific compounds. The boring logs presented in this Appendix are based on drilling action, ease of excavation and visual inspection of the samples. The various types of soil encountered are indicated, as well as interpreted depths where the soils or characteristics of the soils I changed. It should be noted that these changes may be gradual. Hart Crowser J -3076 APPENDIX A FIELD AND SAMPLING METHODS Groundwater Sampling Prior the collection of a groundwater sample, standing water is removed from the well bore and the surrounding sandpack. This is referred to as well purging and is important to collect a representative sample of the water - bearing zone. We purged the well by bailing the water using a PVC bailer. In this site we dedicated one PVC bailer to each well, this minimized the potential for cross contamination between wells. The water - bearing zone on this site had a relatively low permeability; therefore, we were able to bail the water from the well faster than the water was moving into the well from the surrounding formation. Purging was completed once a well was bailed dry (EPA, 1986). Groundwater sample were collected as the water began to recharge the well as per recommendations for sampling presented in the RCRA Ground Water Monitoring Technical Enforcement Guidance Document (EPA, 1986). The reason for sampling the water as it recharges the well is to minimize the loss of volatile organics. Groundwater samples for volatile organics (EPA method 8020) were collected first. These samples were placed in VOA jars supplied by the contract laboratory. Care was taken during sampling not to agitate the 1 water during transfer from the bailer to the VOA jars and to fill the jars to the top removing trapped air bubbles within the sample jar. Samples for petroleum hydrocarbons (EPA method 8015 modified) were collected in 250 ml plastic bottles supplied by the contract laboratory. These jars were also filled to the top and checked for trapped air bubbles. The sample jars were placed in a cooler for transport to the contract laboratory for analysis. Chain of Custody forms were kept for each set of samples submitted for analysis. These forms accompany the Laboratory Certificates in Appendix B. Page A -1 =r • �._ Hart Crowser J -3076 Test Pit Explorations The test pits were excavated on August 27, and November 1, 1990, using a rubber tired backhoe under the direction of a Hart Crowser field representative. Soil logs were prepared for each exploration in general accordance with the visual- manual description procedure (ASTM D 2488) as depicted on Figure A -1. Summary logs are presented on Figures A -2 through A -6. 1 Soil Sampling A small stainless steel spoon was used to remove soil from the center of the backhoe bucket. Samples collected for chemical analysis were placed in a glass jar with a teflon -lined lid, supplied by the contract laboratory, and placed in a cooler for transport to the laboratory. Chain of Custody forms are presented with the laboratory certificate in Appendix B. Excavations I The excavations to remove soil containing petroleum hydrocarbons were conducted from October 30, to November 1, 1990, under the direction of a Hart Crowser representative. The soil was excavated using a rubber tired backhoe from ESE Corporation under subcontract to Chevron. A small stainless steel spoon was used to remove soil from the center of the backhoe bucket for soils collected for chemical analysis and organic vapor headspace measurements. Composite samples collected from the side wall of the excavation were composite samples from the areas outlined on Figure 3. The composite samples were collected by placing small amounts of soil from the backhoe bucket into one soil jar. The composite samples consisted of soil collected approximately every two feet along the side wall. Samples collected for chemical analysis were placed in a glass jar with a teflon -lined lid, supplied by the contract laboratory, and placed in a cooler for transport to the laboratory. Hart Crowser J -3076 Excavated Soil Piles Soil with otentiall containing petroleum hydrocarbons were placed on P Y gP Y P plastic sheeting and covered with plastic sheeting. Composite soil samples were collected to characterize the soils for disposal. The composite samples were collected at 10 locations on the soil pile. The samples were collected at least six inches below the pile surface using a stainless steel spoon. Samples collected for chemical analysis were placed in a glass jar with a teflon -lined lid, supplied by the contract laboratory, and placed in a cooler for transport to the laboratory. Chain of Custody forms are presented with the laboratory certificate in Appendix B. Organic Headspace Vapor Measurements Sample jar headspace organic vapor measurements were made using an H -Nu photoionization detector to assess the possible presence of petroleum product in soils. Soil samples from the split -spoon sampler were collected in glass jars (filled half full), covered with aluminum foil �.. prior to capping, and then allowed to sit for 10 minutes. The H -Nu measurements were made using a 10.2 eV probe. The H -Nu was calibrated using a manufacturer - supplied standard gas (isobutylene, equivalent to 34 ppm benzene) prior to making the measurements. 1 Equipment Decontamination Soil sampling and groundwater sampling equipment was decontaminated between each sampling. Decontamination consisted of washing the equipment with an Alconox solution followed by rinses in successive baths of tap water and deionized water. Well Abandonment Procedure Monitoring well MW -2 was abandoned using bentonite- cement and bentonite chips in accordance with WAC 173 -160 -425. The total number of fire extinguishers required for your establishment is calculated at one extinguisher for each 3000 sq. ft. of area. The extinguisher(s) should be of the "All Purpose" (2A, 10B:C) dry chemical type. Travel distance to any fire extinguisher must be 75' or less. (NFPA 10, 3 -1.1) Extinguishers shall be installed on the hangers or in the brackets supplied, mounted in cabinets, or set on shelves (NFPA 10, 1 -6.9), and shall be installed so that the top of the extinguisher is not more than 5 feet above the floor. (NFPA 10, 1 -6.9) Extinguishers shall be located so as to be in plain view (if at all possible), or if not in plain view, they shall be identified with a sign stating, "Fire Extinguisher ", with an arrow pointing to the unit. (NFPA 10, 1 -6.3) (UFC Standard 10 -1) Clear access to fire extinguishers is required at all times. They may not be hidden or obstructed. (NFPA 10, 1 -6.5) 2. Exit doors shall be openable from the inside without the use of a key or any special knowledge or effort. (UFC 1207.3) Obstructions, including storage, shall not be placed in the required width of an exit, except projections as permitted by the Building Code. Exits shall not be obstructed in any manner and shall remain free of any material or matter where its presence would obstruct or render the exit hazardous. (UFC 1203) Dead bolts are not allowed on auxiliary exit doors unless the dead bolt is automatically retracted when the door handle is engaged from inside the tenant space. Exit hardware and marking shall meet the requirements of the Uniform Fire Code. (UFC 1207.1-1212.8) 3. All electrical work and equipment shall conform strictly to the standards of The National Electrical Code. (NFPA 70) 4. Required fire resistive construction, including occupancy separations, area separation walls, exterior walls due to location on property, fire resistive requirements based on type of construction, draft stop partitions and roof coverings shall be maintained as specified in the Building Code and Fire Code and shall be properly repaired, restored or replaced when damaged, altered, breached, penetrated, removed or improperly installed. The following usage chart illustrates the substantial reductions in water usage reclaim units offer as opposed to 100% fresh water operations. The chart was prepared using actual wash package cycles, with speeds of the individual cycles adjusted accordingly. Summary of results: Total water consumption of reclaim systems averages 47.3 gallons per vehicle, of which 12.1 gallons is fresh. (This figure obtained from the "Averages Of All Packages section under the Cond. Water Column on the usage / costs sheet.,) Currently the highest volume Laserwash in our records processes an average of 85 vehicles per day. Two separate scenarios will follow: • Using the average fresh water consumption from above and the 85 vehicle per day . average, fresh water usage would be 1,028 per day. Limiting the operating, hours of the Laserwash to 12 hours per day, the hourly fresh water consumption would be 85 gallons. • The maximum hourly production for the Laserwash is 20 vehicles.
