Handouts are available at the Building counter which provide more detailed information on application and plan submittal requirements. Application and plans must be complete in order to be accepted for plan review. VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is subject to possible revision by the Building Division to comply with current fee schedules. BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the permit will be required as part of this submittal. EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition). No application shall be extended more than once. Dear Ms. Freiheit. This report presents a summary of our monitoring services for the installation of auger -cast piling for the Southcenter Golf Driving Range. The two story driving range building is supported on auger -cast piling in accordance with the recommendations provided by Geospectrum Consultants Inc., in their report dated June 3. 1994. The scope of our services for this project included the following: (i) Review of Geotechnical Report and addendum prepared by Geospectrum Consultants Inc. (ii) Review of Plans and Specifications (iii) Monitor installation of auger -cast piling (iv) Monitor installation of grade beams on auger -cast piling (v) Prepare report that summarize the results of our observations. OBSERVATIONS A total of 74 auger -cast piles of 18 -inch diameter were installed from October 9, 1995 to October 11, 1995 by the McDowell Northwest Pile Driving Company. The pile locations were laid out by Ferguson Contracting. The piles were all installed to a 35 foot depth below the site grade. A field engineer from PacRim Geotechnical Inc. was on site during the drilling and installation of the piles. Pertinent data for each pile was recorded in the field, including drilling time, casting time, cast length, volume of grout used, pump pressures, and hydrostatic head of grout during casting. The data collected were recorded for each pile on individual pile installation record forms, and a summary of each day's activity is attached to this report. The summary includes the field engineer's daily memos as well as the summarized pile installation data. The individual pile records are in our files and are available for k.". Grout samples were taken by a representative of A.A.R Testing Laboratory Inc., who was present on site during all 3 days of installation. The pile identification noted on the summary sheet is the same as that used in the foundation plans of the Architect. On October 25, 1995, the undersigned inspected the pouring of the grade beams over the west half of the Club House. A field memo of that inspection is provided at the end of the report. CONCLUSIONS Based on results of our observations during construction, it is the opinion of PacRim Geotechnical Inc. that all of the auger -cast piles are founded in competent soil and were constructed in accordance with the project plans and specifications, and the recommendations provided in the Geospectrum's Report dated June 3, 1994. During installation the grout pressure, grout volume, and speed of drilling and withdrawal of auger were noted for each pile. The pile installation summary is attached. Based on the result of the observation we conclude that the piles have been installed in accordance with the recommendations presented in the soils report. During installation of 2nd pile, the grout hose was plugged up and had to be cleaned before casting the pile (F -4). Mike King arrived at the site @9:15 and took over the monitoring of the rest of the piles. Monitored up to pile #20. Mike King: Observed drilling and construction of augercast piles. At the time of inspection, the contractor was finalizing the tying of steel reinforcement for the beams. All the pile tops had been jet cleaned and roughened such that a good bond between the pile tops and the grade beams could be acheived. Based on my observations and review of site plans, it is our opinion that the foundation has been well prepared for the design loads.
