Thomas REPORT NO.: FR26507 REMARKS: A representative of PSI arrived at the above referenced project, by request, for special inspections of fiber- reinforced epoxy composite material. The installation was at the mall level grid line 21 at lines K and L and 21 at lines G and F. The surface walls were previously prepared to the specifications (1/16" amplitude) and the cracks were injected with epoxy. The thickened Tyfo S material was used to prime the surfaces and fill the voids. Tyfo SEH 51A system was utilized. The lot numbers utilized on this date include S01622 and S06122 and there were 20 gallons used on this date. He observed the mixing of the materials, the saturation of fiberglass fabric, the installation and the pinning of the fabric with fiberglass anchors. He requested that the Contech representatives fabricate (4) 12" x 12" sample panels to represent the day's installation. The samples are to be retrieved after 48 hours and the lot numbers for the fabric style SEH -51A which were used include lot 2002- 02 -01, roll 57 and 169. The methods and materials on this date are in conformance to the project plans and the specifications to the best of the inspector's knowledge.
