2: No changes shall be made to the approved plans unless approved by the design professional in responsible charge and the
Building Official.
3: All permits, inspection records, and approved plans shall be at the job site and available to the inspectors prior to
start of any construction. These documents shall be maintained and made available until final inspection approval is
granted.
4: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap the sanitary
sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks, wells, and other
excavations. Final inspection approval will be determined by the building inspector based on satisfactory completion of
this requirement.
5: All plumbing and gas piping work shall be inspected and approved under a separate permit issued by the Cityof Tukwila
Permit Center.
6: All electrical work shall be inspected and approved under a separate permit issued by the City of Tukwila Building
Department (206 - 431 - 3670).
7: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an approval of,
any violation of any of the provisions of the building code or of any other ordinances of the City of Tukwila. Permits
presuming to give authority to violate or cancel the provisions of the code or other ordinances of the City of Tukwila
shall not be valid. The issuance of a permit based on construction documents and other data shall not prevent the
Building Official from requiring the correction of errors in the construction documents and other data.
9: Contractor shall notify Public Works Project Inspector Mr. Greg Villanueva at (206)433 -0179 of commencement and
completion of work at least 24 hours in advance.
10: Any material spilled onto any street shall be cleaned up immediately.
11: The site shall have permanent erosion control measures in place as soon as possible after final grading has been
completed and prior to the Final Inspection.
12: From October 1 through April30, cover any slopes and stockpiles that are 3H: 1V or steeper and have a vertical rise of
10 feet or more and will be unworked for greater than 12 hours. During this time period, cover or mulch other disturbed
areas, if they will be unworked more than 2 days. Covered material must be stockpiled on site at the beginning of this
period. Inspect and maintain this stabilization weekly and immediately before, during and following storms.
13: Any septic tanks in the area shall be pumped empty and removed or filled with sand. A copy of the documentation from
the business that performed the pumping shall be provided to the Public Works Project Inspector.
14: Water service line, if used again in the near future, shall be capped at the water meter.

Environmental Associates, Inc. (EAI), has completed an Asbestos Survey for the unoccupied
residence with two sheds at the address given above. It is our understanding that demolition of the
structures may occur in the future. This report, prepared in accordance with the terms of our proposal
dated March 14, 2007, summarizes our approach to the project along with results and conclusions.
The contents of this report are confidential and are intended solely for your use and the use of your
representatives. Four (4) copies of this survey report are being distributed to you. No other
distribution or discussion of this document will take place without your prior approval in writing.
Additional copies are available for a small fee.
Environmental Associates, Inc., (EAI) staff visited the unoccupied residence with two sheds on
March 26, 2007. We submitted a total of 26 samples to the project laboratory. Project laboratory staff
divided 17 samples into two or more layers each. Consequently, we received 53 reportable results
from the laboratory. One sample of beige vinyl floor tile returned a positive result for the presence
of asbestos above the regulatory limit of 1% asbestos content. The approximate extent of the beige
vinyl floor tile material is approximately 150 square feet. Two samples of drywall joint compound
also contained asbestos, however, the laboratory analyst reports that the "Composite result for the
whole [drywall] sample is less than 1% asbestos."

METHODOLOGY /SCOPE OF WORK
The scope of work for this project included the following tasks:

Task 1 - Asbestos Survey /Sampling
An AHERA/EPA certified asbestos inspector from Environmental Associates, Inc., (EAI) reviewed
accessible interior spaces and certain exterior locations, in an effort to identify potential asbestos -
containing material (ACM). The buildings were unoccupied at the time of our site visit. During the
inspection, our personnel obtained samples of potential ACM for later laboratory "confirmation"
analysis. Suspected friable asbestos containing materials were grouped according to functional areas
and homogenous areas. Three or more samples of each homogenous area were tested.
Additional notes were kept regarding specific observations during the survey along with
photographic documentation.
Task 2 - Laboratory Analysis
Asbestos screening analysis consisted of polarized light microscopy (PLM) with dispersion staining
in accordance with U.S. EPA/600/R- 93/116 test method. This method permits resolution of bulk
percentage of asbestos content to 1 percent which coincides with EPA's lower end value for
definition of a material as ACM.
Task 3 - Report Preparation
At the conclusion of the inspection and laboratory analysis activity, Environmental Associates, Inc.,
— prepared a survey report documenting methods used during the various stages of the project along
with an organized discussion of findings and recommendations. With respect to quantities of
asbestos, this effort resulted in a report which provides estimated quantities for ACM which may be
encountered.

GENERAL DISCUSSION
An EPA - Certified Asbestos Inspector from Environmental Associates, Inc. visited the subject
property on March 26, 2007 in order to obtain representative samples of construction materials
suspected to contain asbestos. Our staff documented the sampled materials condition, extent, and

LABORATORY TESTING SUSPECT ASBESTOS CONTAINING MATERIALS
As previously discussed, we visited the occupied building on March 26, 2007. We submitted a total
of 26 samples to the project laboratory. Project laboratory staffdivided 17 samples into two or more
layers each. Consequently, we received 53 reportable results from the laboratory. One sample of
beige vinyl floor tile material returned a positive result for the presence of asbestos above the
regulatory limit of 1% asbestos content.
The results of the asbestos analysis, reported by the project laboratory on March 28, 2007, are
provided in the following table. Sample numbers correspond to localities depicted on the Sampling
Plan, Plate 1.

CONCLUSIONS /RECOMMENDATIONS
ASBESTOS- CONTAINING MATERIALS
Based upon this inspection, it appears that asbestos is present above the regulatory level of 1%
asbestos content in the beige vinyl floor tile in the kitchen ( sample #19). We estimate approximately
150 square feet of this material is present in the residence.

We recommend that this report be maintained in the files of the owner and at the facility to verify
that sample analysis was conducted and document management protocol implemented in the course
of this work. It is our further recommendation that the findings of this report be disclosed to all

asbestos - containing materials described herein so that appropriate personal protective measures may
be implemented.

In reviewing our current records the above noted permit has not received a final inspection by the City of Tukwila Building Division.
Per the International Building Code and/or the International Mechanical Code, every permit issued by the Building Division under the
provisions of this code shall expire by limitation and become null and void if the building or work authorized by such permit is not
commenced within 180 days from the date of such permit, or if the building or work authorized by such permit is suspended or
abandoned at any time after the work is commenced for a period of 180 days.
Based on the above, you are hereby advised to:
Call the City of Tukwila Inspection Request Line at 206 -431 -2451 to schedule for the next or final inspection.
This inspection is intended to determine if substantial work has been accomplished since issuance of the permit or last inspection; or if
the project should be considered abandoned.
If such determination is made, the Building Code does allow the Building Official to approve one or more extensions of time for
additional periods not exceeding 90 days each. Extension requests must be in writing and provide satisfactory reasons why
circumstances beyond the applicants control have prevented action from being taken.
In the event you do not call for the above inspection and receive an extension prior to 01/01/2008, your permit will become null and
void and any further work on the project will require a new permit and associated fees.
