3. Parking Analysis of existing and proposed capacity; proposed stalls with dimensions

5. Recycle collection location and area calculations (change of use only)
6. Location and screening of outdoor storage (change of use only)
7. Limits of clearing /grading with existing and proposed topography at 2' intervals extending 5' beyond property's
boundaries
8. Identify location of sensitive area slopes 20% or greater, wetlands, watercourses and their buffers (change of
use only)
9. Identify location and size of existing trees that are located in sensitive areas and buffer (TMC 18.45.040), of
those, identify by size and species which are to be removed and saved
10. Landscape plan with irrigation and existing trees to be saved by size and species (exterior changes or change
of use only)
11. Location and gross floor area of existing structure with dimensions and setback
12. Lowest finished floor elevation (if in flood control zone)
13. See Public Works Checklist for detailed civil /site plan information required for Public Works Review (Form H-
9).

❑ ❑ Sprinkler details - details of sprinkler hangers, specifically penetrations in structure, i.e., roof; size of

exceed sprinkler system design criteria as identified by the Fire Department.
❑ ❑ Washington State Non - Residential Energy Code Data shall be noted on the construction drawings.
❑ ❑ SEPA Checklist - if intensification of use (check with Planning Department for thresholds).
❑ ❑ Attach plans, reports or other documentation required to comply with Sensitive Area Ordinance or other
land use or SEPA decisions.

Department of Public Health prior to submitting for building permit application. The Department of
Public Health is located at 201 Smith Tower, Seattle, WA or call (206) 296 -4787. (Form H -5)
❑ ❑ Copy of Washington State Department of Labor and Industries Valid Contractor's License. If no
contractor has been selected at time of application a copy of this license will be required before the
permit is issued OR submit Form H -4, "Affidavit in Lieu of Contractor Registration ".
Building Owner /Authorized Agent If the applicant is other than the owner, registered architect/engineer, or contractor licensed
by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and
obtain the permit will be required as part of this submittal

The attached set of building plans have been reviewed by
The Fire Prevention Bureau and are acceptable with the
following concerns:
1. The total number of fire extinguishers required for
your establishment is calculated at one extinguisher for
each 3000 sq. ft. of area. The extinguisher(s) should be
of the "All Purpose" (2A, 10B:C) dry chemical type. Travel
distance to any fire extinguisher must be 75' or less.
(NFPA 10, 3 -1.1)
Portable fire extinguishers shall be securely
installed on the hanger or in the bracket supplied,
placed in cabinets or wall recesses. The hanger or
bracket shall be securely and properly anchored to the
mounting surface in accordance with the manufacturer's
instructions. The extinguisher shall be installed so
that the top of the extinguisher is not more than 5
feet above the floor and the clearance between the
bottom of the extinguisher and the floor shall not be
less than 4 inches.
Extinguishers shall be located so as to be in plain
view (if at all possible), or if not in plain view,
they shall be identified with a sign stating, "Fire
Extinguisher ", with an arrow pointing to the unit.

Clear access to fire extinguishers is required at all
times. They may not be hidden or obstructed. (NFPA
10, 1 -6.5)

2. Commercial -type food heat- processing equipment from
which grease -laden vapors emanate in normal cooking
application shall be protected by an approved automatic
extinguishing system. The extinguisher system shall be
interconnected to the fuel and current supply so that the
fuel or current is automatically shut off to all equipment
under the hood, when the system is actuated. (UFC 1006)
A sodium bicarbonate or potassium bicarbonate
dry - chemical -type portable fire extinguisher having a
minimum rating of 40 -B shall be installed within 30
feet (9144 mm) of commercial food heat - processing
equipment, as measured along an unobstructed path of
travel. (UFC 1006.2.7)
3. No point in an unsprinklered building may be more than
150 feet from an exit, measured along the path of travel.
(UBC 1003.4)
Exit doors shall swing in the direction of exit travel
when serving any hazardous area or when serving an
occupant load of 50 or more. (UBC 1004.2)
Exit doors shall be openable from the inside without
the use of a key or any special knowledge or effort.
Exit doors shall not be locked, chained, bolted,

Fire extinguishers require monthly and yearly t
inspections. They must have a tag or label securely,.
attached that indicates the month and year that the
inspection was performed and shall identify the
;company or person performing the service. (NFPA 10,
4- 3,.4 -4 and 4 -4.3) Every six years, dry chemical and
halon type fire extinguishers shall be emptied and
subjected to the applicable recharge procedures. (NFPA
10, 4-4.1) If the required monthly and yearly .
inspections of the fire extinguisher(s) are not
accomplished or the inspection tag is not completed, a
reputable fire extinguisher service company will be
required to conduct these required surveys. (NFPA
10A -4 -4)

unless the dead bolt is automatically retracted when
the door handle is engaged from inside the tenant
space.
Exit hardware and marking shall meet the requirements
of the Uniform Fire Code. (UFC 1207.1 - 1212.8)
Internally illuminated exit signs shall have both
bulbs working at all times. (UBC 1013.3)
Aisles leading to required exits shall be provided
from all portions of buildings. Aisles located within
an accessible route of travel shall also comply with.
the Building Code requirements for accessibility.

Any room having an occupant load of more than 50 where
fixed seats are not installed, and which is used for
classroom, assembly or similar purpose, shall have the
capacity of the room posted in a conspicuous place on
an approved sign near the main exit from the room.
Such sign shall be maintained legible by the owner or
the owner's authorized agent and shall indicate the
number of occupants permitted for each room use. (UFC
2501.16,1)
Exits shall not pass through kitchens, storerooms,
restrooms, closets or spaces used for similar
purposes. (UBC 1003.5)

maintain, subject to reasonable wear and tear, all landscaped areas, sidewalks, paved
areas, heating, and air conditioning items.
5. REPAIRS AND MAINTENANCE. The outside walls, roof and foundations
shall promptly be repaired and kept in good condition by Lessor at Lessor's sole
expense unless the need for such repairs is caused by negligent acts of Lessee or its
agents. If either party fails to make the repairs and perform the maintenance required of
it, the other party may, at its option, twenty (20) days after having given written notice,
complete such maintenance and /or repairs; and the amount paid by such party for such
repair and maintenance, with interest thereon at twelve percent (12 %) from date of
payment, shall become immediately due and payable by the defaulting party. Payment
by either party or any such amounts shall not be deemed a waiver of any default by the
other with respect to such repair and /or maintenance or of the right of either to institute
such action as would be available in the case of other defaults under this lease
agreement.
6. UTILITIES. Lessee hereby covenants and agrees to pay all charges for heat,
light, water and sewer charges, and for all other public utilities which shall be used in or
charged against the leased premises during the full term of this lease.
7. PERSONAL PROPERTY. All personal property on said leased premises shall
be at the risk of Lessee. Lessee shall also pay when due all taxes upon said personal
property.
8. NON - LIABILITY OF LESSOR. Lessor or Lessor's agents shall not be liable
for any damage, either to person or property, sustained by Lessee or others caused by
any defects now in said premises or hereafter occurring therein, or due to any part or
appurtenance thereof becoming out of repair, or caused by the bursting or leaking of
water, gas, sewer or steam pipes, or from any act or neglect of employees, co- tenants
or other occupants of said building, or any other persons, or due to the happening of
any accident from whatsoever cause in and about said building. Lessee agrees to
defend and hold Lessor harmless from any and all claims or damages suffered in or
about the leased premises by any person, firm or corporation, unless caused by
Lessor's sole negligence.
9. USE AND MAINTENANCE OF PREMISES. Lessor shall not be called upon
to make any improvement or repair of any kind upon said premises, except those noted
in paragraph 5 and said premises shall at all times be kept and used in accordance with
the laws of the state of Washington and ordinances and rules and regulations of the
applicable municipality, and in accordance with all directions, rules and regulations of
the health officer, fire marshal, building inspector, or other proper officer of the
applicable government entity, at the sole cost and expense of said Lessee, and Lessee
will permit no waste, damage or injury to the premises, except that incurred by
reasonable wear and tear, and, at Lessee's own cost and expense, will keep all
drainage pipes free and open and will protect water, heating and other pipes so that
they will not freeze or become clogged, and will repair all Teaks, and will also repair all

1. I have made application for a building permit from the City of Tukwila, Washington.
2. I understand that state law requires that all building construction contractors be registered with the
State of Washington. The exceptions to this requirement are stated under Section 18.27.090 of the
Revised Code Washington, a copy of which is printed on the reverse side of this Affidavit. I have
read or am familiar with RCW 18.27.090.
I understand that prior to issuance of a building permit for work which is to be done by any
contractor, the City of Tukwila must verify either that the contractor is registered by the State of
Washington, or that one of the exemptions stated under RCW 18.27.090 applies.
4. In order to provide verification to the City of Tukwila of my compliance with this requirement, I hereby
attest that after reading the exemptions from the registration requirement of RCW 18.27.090, I
consider the work authorized under this building permit to be exempt under No. , and
will therefore not be performed by a registered contractor.
I understand that I may be waiving certain rights that I might otherwise have under state law in any decision to
engage an unregistered contractor to perform construction work.

18.27.090 Exemptions. This chapter si�ll not apply to:

1. An authorized representative of the United States
Government, the State of Washington, or any
incorporated city, town, county, township, irrigation
district, reclamation district, or other municipal or
political corporation or subdivision of this state;
2. Officers of a court when they are acting within the
scope of their office;
3. Public utilities operating under the regulations of the
utilities and transportation commission in
construction, maintenance, or development work
incidental to their own business;
4. Any construction, repair, or operation incidental to the
discovering or producing of petroleum or gas, or the
drilling, testing, abandoning, or other operation of any
petroleum or gas well or any surface or underground
mine or mineral deposit when performed by an owner
or lessee;
5. The sale or installation of any finished products,
materials, or articles of merchandise which are not
actually fabricated into and do not become a
permanent fixed part of a structure;
6. Any construction, alteration, improvement, or repair
of personal property, except this chapter shall apply
to all mobile /manufactured housing. A
mobile /manufactured home may be installed, set up,
or repaired by the registered or legal owner, by a
contractor licensed under this chapter, or by a
mobile /manufactured home retail dealer or
manufacturer licensed under chapter 46.70 RCW;
7. Any construction, alteration, improvement, or repair
carried on within the limits and boundaries of any site
or reservation under the legal jurisdiction of the
federal government;
8. Any person who only furnished materials, supplies, or
equipment without fabricating them into, or
Consuming them in the performance of, the work of
the contractor;
9. Any work or operation on one undertaking or project
by one or more contracts, the aggregate contract
price of which for labor and materials and all other
items is less than $500, such work, or operations
being considered as of a casual, minor, or
inconsequential nature. The exemption prescribed in
this subsection does not apply in any instance
wherein the work or construction is only a part of a
larger or major operation, whether undertaken by the
same or a different contractor, or in which a division

of the operation is made into contracts of amounts
Tess than $500 for the purpose of evasion of this
chapter or otherwise. The exemption prescribed in
this subsection does not apply to a person who
advertises or puts out any sign or card or other
device which might indicate to the public that he is a
contractor, or that he is qualified to engage in the
business of contractor;
10. Any construction or operation incidental to the
construction and repair of irrigation and drainage
ditches of regularly constituted irrigation districts or
reclamation districts; or to farming, dairying,
agriculture, viticulture, horticulture, or stock or poultry
raising; or to clearing or other work upon land in rural
districts for fire prevention purposes; except when
any of the above work is performed by a registered
contractor;
11. An owner who contracts for a project with a
registered contractor;
12. Any person working on his own property, whether
occupied by him or not, and any person working on
his residence, whether owned by him or not but this
exemption shall not apply to any person otherwise
covered by this chapter who constructs an
improvement on his own property with the intention
and for the purpose of selling the improved property;
13. Owners of commercial properties who use their own
employees to do maintenance, repair, and alteration
work in or upon their own properties;
14. A licensed architect or civil or professional engineer
acting solely in his professional capacity, an
electrician licensed under the laws of the state of
Washington, or a plumber licensed under the laws of
the state of Washington while operating within the
boundaries of such political subdivision. The
exemption provided in this subsection is applicable
only when the licensee is operating within the scope
of his license;
15. Any person who engages in the activities herein
regulated as an employee of a registered contractor
with wages as his sole compensation;
16. Contractors on highway projects who have been
prequalified as required by. chapter 13 of the Laws of
1961,,IRC.W 47.28.070 with the department of
transportation tto, perforrn highway construction,
reconstruction, or maintenance work.
