9. Identify location and size of existing trees that are located in sensitive areas and buffer (TMC 18.45.040), of
those, identify by size and species which are to be removed and saved
10. Landscape plan with irrigation and existing trees to be saved by size and species (exterior changes or change
of use only)
11. Location and gross floor area of existing structure with dimensions and setback
12. Lowest finished floor elevation (if in flood control zone)

contractor has been selected at time of application a copy of this license will be required before the
permit is issued OR submit Form H -4, "Affidavit in Lieu of Contractor Registration ".

Tube Entries provide a single port through a building wall or roof. If used
as a roof Entry, they are sealed with B193 Rdofing Compound. They can be
used for thin walls, such as for a fiberglass equipment building, or for walls
up to 20" thick. A Boot assembly is used on the exterior to weatherproof
the Entry. Another Boot assembly may be used on the interior to provide a
more finished look. This Entry is ideal for adding runs to existing systems.
B184 Sealing Tape is recommended for weather proofing the outside

inspection services for the referenced project. Our services were performed as requested and
scheduled by U.S. WEST Wireless. Daily reports of our inspection were prepared at the time
our services were provided. Our daily field reports, which are attached, form the basis for this
final summary letter.
Performed inspection services for structural steel bolting. A -325N high- strength bolts were
found to conform with approved plans and specifications.
Performed inspection services for welding. All welds were found to conform with approved
plans and specifications.
Performed inspection services for expansion anchor bolts. All expansion anchor bolts were
found to conform with approved plans and specifications.
