EEI was instructed that if Option 1 loading is not acceptable to analyze Option 2 loading. This monopole was designed and supplied by Engineered Endeavors, Inc. and depicted in drawing GS46856. The objective of this analysis is to determine if the monopole and foundation can structurally support the desired configuration and meet the requirements of the TIA/EIA 222 -F code and Manual of ,; Steel Construction Allowable Stress Design. For further information on the structural loading, refer to the EEI analysis cover sheets and calculations. Under both Option 1 and Option 2 proposed loading, the 100 ft monopole is not ;'. adequate to support the proposed antennas and ancillary equipment and meet the criteria specified in the above referenced codes. A maximum principal stress of 36.9 ksi occurs at the bottom of the middle section of this monopole. The allowable stress at this point is 51.6 ksi, based on 65 ksi steel. Therefore, the monopole shaft is adequate and approximately 72% stressed. Refer to the anchor bolt and baseplate calculations. Note that the baseplate is approximately 13% overstressed. In addition, the supplied anchor bolts provide an embedment of 5ft where the necessary Option 2 anchor bolt embedment in 4000 psi concrete is 6ft. With the monopole not being adequate to support the proposed loading, the foundation was not analyzed. Also, methods for reinforcing the baseplate or adding anchor bolts has not been considered. It is the responsibility of U.S. West to verify that the monopole modeled and analyzed is the correct structure that exist. This report is intended for use with regard to the specific monopole discussed in general herein. The analysis reported that with the Option 1 loading, the monopole shaft is adequate while the baseplate would be over stressed and the anchor bolt embedment was not adequate. Engineered Endeavors, Inc. agreed to provide a foundation analysis and engineering to modify the baseplate and add anchor bolts. 1) Foundation The foundation was designed by Engineering & Consulting Services, Inc. and is depicted in drawing ALLEN. The soil report was provide by Rittenhouse, Zeman & Associates, Inc. Under project P -3329. EEI assumes that the foundation is in good condition and was installed as specified in the foundation design. A comparison of tower design base loads are shown in Table I for the original design and Option 1 loading. As shown, the new foundation loads are approximately 50% greater than the original loads. The foundation overturning stability calculations reveal that the overturning factor of safety is 1.51 and equal to the industry minimum accepted value of 1.5. A maximum deflection of 1.1 in is also acceptable. In addition, the foundation concrete reinforcement is adequate. Therefore, EEI has determined that the foundation is adequate to support the monopole Option 1 loading. ENGINEERED ENDEAVORS, INC. Note that the addition of four anchor bolts and the baseplate adaptors provide the additional capacity needed for this monopole to be structurally adequate to support the proposed Option 1 loading (with the exception of operational rotation for the microwave at 80 ft). The necessary embedment for all twelve anchor bolts in 4,000 psi concrete is 5 ff. The applied anchor bolt axial stress is 25.6 ksi and less than the allowable stress of 60 ksi. An actual baseplate stress of 38.8 ksi is acceptable compared to the allowable bending stress of 60 ksi. Instructions for the baseplate modification and anchor bolt addition are outlined in the Quality Assurance and Procedural Manual for Field Installation of Anchor Bolts and Baseplate Adaptors provided within this report. The manual provided is for use as a guide for the field engineer and /or construction supervisor and the process and order of installation is ultimately their decision. It is the responsibility of US West to verify that the monopole modeled and analyzed is the correct structure that exist and to inspect the final field modifications. Specific standards and procedures are emphasized to assure the integrity of the steel monopole and foundation. Based on the recommendations of reference [13, four additional anchor bolts ( #18J A615 GR. 75 steel) must be installed on a 100 ft monopole located in Allentown, WA which has a cell site name of SEA 241. The additional anchor bolts must be embedded 5.0 ft into the 4,000 psi concrete and have a i ft projection. A 2 34 " 43 hole is provided through the baseplate adaptor and required in the foundation concrete to a depth of 7.5 ft. The location of the anchor bolts is shown in the Anchor Bolt Installation drawing 4318 -AB. The foundation must be field drilled to accomplish this task. The anchor bolts are to be secured in place by Sikadur® 32, Hi Mod adhesive. Materials Required Located in Table 1 is a brief summary of the hardware and materials required to complete this task. Table 1: Materials Required for Anchor Bolt Installation Quality Assurance and Procedural Manual ENGINEERED ENDEAVORS INC, for Field Installation of Anchor Bolts ; Field Installation of Baseplate Adaptors 2 Install the baseplate adaptors as specified in EEI drawing GS51225 and the following items. The adaptors may be installed before or after the additional anchor bolts are installed as specified by the construction supervisor. O Grind galvanized areas of the existing baseplate to be welded. Clean the existing baseplate and baseplate adaptors as necessary before welding. . OO Install the '4" x 2" x 2' -8 1/8" LG. Back -Up Bars (A871 -GR60) as shown. OO Install the 2" x 11" x 2' -6 1/8" LG. Baseplate Adaptors (A871 -GR60) as shown with a full penetration weld. ® Install the 1" x 12" x 12" Gusset Plates (A871 -GR60) as shown. Field Drilling for Anchor Bolts Refer to EEI Anchor Bolt Installation drawing 4318 -AB for a graphical summary of the following items. O Location of the four anchor bolt holes are clearly shown in the accompanying drawings. The bolt holes are to be drilled on a 52.25 in bolt circle to match the bolt pattern after the baseplate adaptors are installed. OO Any non - shrink grout must be removed from the surrounding location of the new anchor bolts. Surface locations shall be thoroughly cleaned of grout, dirt, and foreign matter. Oa The diameter of the holes is 2 3/ in. Holes should be drilled straight and vertical. The baseplate adaptors will act as a template secured in place to guarantee the proper bolt hole location. ® A depth of 7.5 ft into the concrete is required. OO At the completion of the field drilling and before the anchor bolts are installed, the surface of the baseplate must be thoroughly cleaned and then coated with a "zinc rich" substance to avoid rusting of the steel material. Two coats of an organic zinc rich paint, e.g. ZRC, should be applied to areas of the modified baseplate affected by the drilling. Quality Assurance and Procedural Manual ENGINEERED ENDEAVORS, INC. for Field Installation of Anchor Bolts Field Installation of Anchor Bolts Refer to EEI Anchor Bolt Installation drawing 4318 -AB for a graphical interpretation of the following items. Refer to the Sika Corporation technical data sheets located in Appendix A. Inspection 3 O The holes should be clean from dust, debris, and water. A damp surface is allowed. 20 The Sikadur® 32, Hi -Mod mixture should be prepared according to the specifications. The pot -life time of the prepared mixture is about 30 minutes. O Epoxy grout should be placed through a long funnel, from the bottom up, taking extreme care not to introduce any air into the epoxy in the hole. A quantity should be placed in the hole such that when the anchor bolt is positioned in the hole. the epoxy grout will rise to approximately 1 ft below the top of the concrete. ® Lower the bolt into the hole, turning it and working it up and down to coat the bar thoroughly with the epoxy, but do not work it so vigorously as to get air into the epoxy grout. O Lower the bolt below the baseplate adaptor (if already installed) and temporarily secure the bolt, in order to place the bottom hex nut on the bolt. Lower the bottom hex nut into place such that the anchor bolt can project through the base plate hole sufficiently to place the top hex nut on the bolt. Ultimately, the anchor bolt should project 1 ft above the concrete foundation. © Finish filling the hole with the epoxy grout until the adhesive oozes out the top of the hole. Again, the anchor bolt should be turned and worked (moved up and down) to thoroughly coat the bar. O Repair or replace the non - shrink grout between the base plate and concrete foundation. Although inspection is not required by any specific code, EEI suggests that an inspection be performed by a qualified inspector. ENGINEERED ENDEAVORS, INC. Quality Assurance and Procedural Manual for Field Installation of Anchor Bolts Remarks 1. This report has been prepared in order to aid in the field installation of anchor bolts on an existing monopole and to insure the integrity of the monopole and foundation. 2. It is the responsibility of US West Wireless to inspect the final field installation of the anchor bolts and baseplate adaptors in order to verify that the proper procedures and practices were followed. This document is intended for use with regard to this specific monopole and anchor bolts discussed in general herein and any substantial changes to the anchor bolts or monopole should be brought to the attention of EEI so that EEI may determine how this may affect our conclusions. 4. In the event any claim or action is commenced in which the City is named a party, and which suit or action is based on a claim or demand which gives rise to Permitee's obligation pursuant to this Permit, the City shall promptly notify Permitee thereof, and Permitee shall, at its sole cost and expense, defend such suit or action by attorneys of its own election. In defense of such suit or action, Permitee may, at its election and at its sole cost and expense, settle or compromise such suit or action. Coordination - All work by Permitee within the Permit area shall be coordinated with the Engineering Division of the Public Works Department to ensure consistency with future City of Tukwila Capital Improvement Projects and shall not commence without 24 hours advance notice to the City's Utility Inspector, Greg Villanueva (206) 433 -0179. 4. Construction Standards - Any construction, installation, maintenance, and restoration activities performed by or for Permitee within the Permit area shall be conducted and located so as to produce the least amount of interference with the free passage of pedestrian and vehicular traffic. All construction, installation, maintenance, and restoration activities shall be conducted such that they conform to the most current City of Tukwila standards in effect at the time that such activities take place. 5. Underground Installation Required - All telecommunications cables and junction boxes or other vaulted system components shall be installed underground in accordance with the utilities undergrounding ordinance requirements (Tukwila Municipal Code - Chapter 13 -08) which are in effect at the time of the construction authorized under the attached Permit. In the event that the undergrounding requirements are deferred for any new or upgrade construction or installation of wire or associated facilities, Permittee agrees by their signature and acceptance of this permit, to participate in the proportionate share of the underground trenching costs and underground conversion costs for any portion of the route taken by the work authorized in this Permit at such time in the future that the City constructs street improvements which include conversion of existing overhead utilities to underground. 6. Relocation - Whenever the City determines that it is necessary for any of Permitee's facilities or other system components to be moved or relocated to accommodate the maintenance, construction, or enhancement of any public amenity in the Permit area, the City shall notify Permitee in writing of such determination, and Permitee shall promptly submit plans for such relocation. Within ninety (90) days of the approval by the City of the plans for relocation, Permitee shall relocate those facilities or structures designated by the City. All costs of moving or relocating such facilities or structures, including but not limited to costs for design, engineering, and construction, shall be the sole responsibility of Permitee. 7. Removal or Abandonment - Upon the removal from service of any transmission lines or other associated structures, facilities and amenities, Permitee shall comply with all applicable standards and requirements prescribed by the City of Tukwila Public Works Department for the removal or abandonment of said structures and transmission facilities. • 8. Performance Bond - The Public Works Director for the City of Tukwila reserves the right to require Permitee to obtain a performance bond for any work which the Director deems to pose a threat to any property or to public safety. Such bond shall be issued by a company licensed by proper authority of the State of Washington and shall be filed with the City Clerk's Office until release is authorized by the Public Works Director. 9. "One -Call" Location & Liability - Permitee shall subscribe to and maintain membership in the regional "One -Call" utility location service and shall promptly locate all of its lines upon request. The City shall not be liable for any damages to Permitee system components or for interruptions in service to Permitee customers which are a direct result of work 'performed for any City project for which Permitee has failed to properly locate its lines and facilities within the prescribed time limits and guidelines established by One Call. The City shall also not be liable for any damages to Permitee's system components or for interruptions in service to Permitee's customers resulting from work performed under any other Permit issued by the City. 10. City - provided Conduit - In those areas of the City where the City has installed its own conduit, Permitee shall utilize such conduit under separate lease agreement with the City. Such agreement shall be negotiated as to the specific terms and the lease shall be provided at a reasonable rate. 11. Traffic Control - Detours within the City's right -of -way shall be in accordance with the current edition of the Manual on Uniform Traffic Control Devices. A traffic control, plan shall be submitted for prior approval if requested by the City's Utility Inspector. 12. Open Cuts - For work within an improved public street, no open cuts shall be allowed unless specifically approved. 13. As Built Records - Permitee shall provide the City with As -Built drawings depicting line and grade for all structures and facilities installed under this permit. Such As- Builts shall be in a form acceptable to the City and approved by the City prior to closure and sign -off of this Permit. 14. Permit Compliance - The failure by Permitee to comply fully with any of the provisions of this Permit may result in a written notice from the City which describes the violations of the Permit and requests remedial action within thirty (30) days of receipt of such notice. If Permitee has not attained full compliance at the end of the thirty (30) day period following receipt of the violation notification, the City may declare an immediate termination of all Permit rights and privileges. The demonstration of due diligence on the part of the Permitee may be grounds for the grant of an extension in the period during which compliance is to be attained; provided that, Permitee continues to pursue correction of any violations of the Permit noted by the City. 15. Emergency Actions - If any of Permitee's actions or any failure by Permitee to act to correct a situation is deemed by the City to create a threat to life or property, the City may order Permitee to immediately correct said situation or, at the City's discretion, the City may undertake measures to correct said situation itself. Permitee shall be liable for all costs, expenses, and damages attributable to the correction of such an emergency situation as undertaken by the City and shall further be liable for all costs, expenses, and damages resulting from such situation. Any reimbursement of such costs to the City shall be made within thirty (30) days of the completion of such action or determination of damages by the City. The failure by Permitee to correct a situation identified by the City as a threat to public or private safety or property shall be considered a violation of Permit terms and each day that such a situation continues to exist shall be regarded as a separate violation. 16.
