Our work has been conducted in accordance with the Proposal for Services letter. dated 14 March 1995 which was authorized by Joe Neuenschwander of Group Health on 20 March 1995. The opinions made in this letter are based primarily upon review of our Phase I /11 Environmental Site Assessment report for the property dated 10 November 1994 (AEE Project No. 11- 096655 -00) , supplemented by building plans furnished us by Group Health and other information available in our office library. PHASE 1 /11 ASSESSMENTS AND UST REMOVAL The results of AEEs Phase I Assessment of the property indicated the following issues of concern: • Results of a limited asbestos survey indicated a relatively small amount of flooring materials in the building contained asbestos; however, these materials were in generally good non - friable condition; • Three (3) steel fuel underground storage tanks (USTs) (diesel fuel: two 8,000 - gallon; one 2,000 - gallon) were located on the subject parcel. The 2,000 gallon UST was installed in 1976 and the 8,000 gallon USTs were installed in 1986; Engineering & Environmental Services C09191 411 CCICICI Group Health Cooperation 29 March 1995 11- 09655 -02 Page 2 • Several chemical containers were observed in the warehouse and generator rooms. No significant spillage or staining was observed in these areas; • At least five confirmed contaminated sites are located within a %: -mile radius of the site, including the METRO south -base site, located at 12100 East Marginal Way South, which is adjacent to the north side of the subject parcel. The results of AEEs observation of UST removal and Phase II Assessment indicated the following results. Well and UST Locations area depicted on Figures 4 and 4A and analytical test results of soil and groundwater are included on Tables 1, 2, and 4. • Soil samples collected from the excavation cavity after removal of the 2,000 - gallon diesel fuel UST and the associated product line did not contain Total Petroleum Hydrocarbons (TPH) concentrations in excess of Washington State Department of Ecology (Ecology) Model Toxics Control Act (MTCA) Method A cleanup levels (See Table 1); • During backfilling of the 2,000 gallon diesel fuel excavation, a shallow 4 -inch I.D. PVC well was installed in the excavation cavity. A groundwater sample collected from the UST well initially contained a diesel -range TPH concentration of 1.9 parts per million (ppm), which is in excess of the MTCA Method A cleanup level of 1.0 ppm. Four hundred gallons of water were pumped from the well and it was subsequently resampled two additional times. The results of the preceding sampling events indicated diesel -range TPH concentrations which were in excess of the MTCA Method A cleanup level (See Table 4); • AEE performed a Phase II Assessment on the site which included nine soil borings, six of which were completed as temporary 2 -inch I.D. wells (MW -1 through MW -6), and three of which were completed as typical 2 -inch I.D. monitoring wells (MW-1A through MW -3A). Soil samples collected from the borings either did not contain concentrations or contained concentrations of TPH that were below MTCA Method A cleanup levels (See Table 2). Groundwater samples collected from temporary well MW -5, located to the north of the two existing 8,000 gallon USTs, contained a diesel -range TPH concentration that was in excess of the MTCA Method A cleanup level. Water samples collected from five of the eight other temporary and installed wells contained detectable diesel and motor oil -range TPH concentrations which cumulatively (diesel + motor oil -range concentrations) are slightly in excess of the MTCA Method A cleanup level of 1.0 ppm (See Table 4). Groundwater °AGRA Earth & Environmental Group Health Cooperation 29 March 1995 11-09655-02 Page 3 depths in the vicinity of the former and existing onsite USTs ranged from 4.33 feet to 6.04 feet below the tops of casing at the time of sampling, with an inferred migration direction to the northwest (See Figure 4A); • The cathodic protection for the two existing 8,000 gallon USTs was found to be in compliance with current Washington Department of Ecology requirements. The results of water samples collected from the well installed in the cavity of the former 2,000 gallon diesel fuel UST indicate a TPH concentration in excess of the MTCA Method A cleanup level. However, given the relatively low concentration, it is highly unlikely that Ecology would demand that a remedial action be performed.at the site. The results of soil and groundwater samples collected from monitoring wells MW -1 A and MW -2A, installed in the vicinity of the existing 8,000 gallon diesel fuel USTs, would not indicate that a significant release had occurred in the vicinity of these tanks at the time of sampling. Additionally, to our knowledge there are no other environmental hazards that would warrant mandating of a large scale cleanup action encompassing the site area due to known contamination sources on the subject site or immediate vicinity. Liquefaction and Floodplain Considerations In addition to the environmental considerations, we have been asked to review available site soil information and foundation plans for the building. We have been furnished the foundation plans for the original building construction (revised 9- 23 -76) from the plan set prepared by Lance Mueller & Associates, Architects. We have also been furnished the foundation plans (dated 4- 29 -85) prepared by Wright Forssen Associates for the new Wing /Micropgraphics/Warehouse, as well a the foundation plans for the Computer Center Expansion (record drawings issue dated 10- 10 -89) prepared by Andersen, Bjornstad, Kane, Jacobs, Inc. Based on the foundation plans, the structure is supported on conventional spread and continuous footings with a slab -on -grade floor. The footings were designed on the basis of an allowable soil bearing pressure of 3,000 pounds per square foot. A total of eight borings /monitoring wells were installed at the site for our Phase II Environmental Site Assessment. The results of which were submitted in our 10 November 1994 report, the borings /monitoring wells were concentrated along the north property line to address potential for off -site impacts, and just south /southwest of the building to address potential impacts from the two existing 8,000 - gallon UST's and the removed 2,000 - gallon UST. On the basis of our borings /monitoring wells, soil conditions consisting primarily of medium dense to very dense silty gravelly sands were encountered with groundwater levels generally in the 10 to 15 foot range with extremes of 5 to 25 feet. According to the Geologic Map of Southwestern °AGRA Earth & Environmental pi ti Group Health Cooperation 29 March 1995. 11- 09655.02 Page 4 King County, Washington, the site area is mapped as . being underlain by the geologic unit Qvt glacial till. On the basis of this available information, it is our opinion that risk of damage to . the building as a result of seismically induced settlement or soil liquefaction is remote. In order to evaluate the risk of flooding from the Duwamish River, we reviewed the King County Sensitive Areas Map Folio (dated December 1990) with respect to 100 -year floodplains as well as the USGS topographical survey for the Des Moines Quadrangle. According to this information, the site is situated at an approximate elevation of 100 feet roughly 1,000 feet from the west bank of the Duwamish River.. The King County Sensitive Areas Map Folio shows the site area to be well outside any 100 -year floodplains. Therefore, it is our opinion that the risk from flooding of the Duwamish River impacting the subject site is remote." -. The total number of fire extinguishers required for your establishment is calculated at one extinguisher for each 3000 sq. ft. of area. The extinguisher(s) should be of the "All Purpose" (2A, 10B:C) dry chemical type. Travel distance to any fire extinguisher must be 75' or less. (NFPA 10, 3 -1.1) Portable fire extinguishers shall be securely installed on the hanger or in the bracket supplied, placed in cabinets or wall recesses. The hanger or bracket shall be securely and properly anchored to the mounting surface in accordance with the manufacturer's instructions. The extinguisher shall be installed so that the top of the extinguisher is not more than 5 feet above the floor and the clearance between the bottom of the extinguisher and the floor shall not be less than 4 inches. Extinguishers shall be located so as to be in plain view (if at all possible), or if not in plain view, they shall be identified with a sign stating, "Fire Extinguisher ", with an arrow pointing to the unit. (NFPA 10, 4 -4.1) If the required monthly and yearly inspections of the fire extinguisher(s) are not accomplished or the inspection tag is not completed, reputable fire extinguisher service company will be required to conduct these required surveys. (UBC 1004.2.2) When two or more exits from a story are required and when two or more exits froma room or an area are required, exit signs shall be illuminated.• (UBC 1003.2.8.4) Manually operated edge- or surface - mounted flush bolts and surface bolts are prohibited. When exit doors are used in pairs and approved automatic flush bolts are used, the door leaf having the automatic flush bolts shall have no door knob or surface - mounted hardware. The unlatching of any leaf shall not require more than one operation. (UFC 1207.3) All exit signs shall be illuminated at all times.
