Appendix G <br />Medical Screening <br />Medical Screening of all authorized members is required annually by DHS FEMA US &R program <br />directive. <br />Departments with an existing annual medical screening program; <br />WATF -1 will reimburse Task Force member's agency for any required additional medical <br />testing that is not currently provided under the agency's existing program. The <br />Department may bill WATF -1 with a single billing for all of its authorized members for the <br />additional testing required by FEMA. <br />WATF -1 will reimburse agencies for expenses incurred up to but no more than the current <br />contracted prices from the Sponsoring Agencies Contracted Medical Service Provider. <br />Individual Task Force members may not submit medical bills for reimbursement only <br />participating agency bills will be accepted. <br />Departments without an existing annual medical screening program <br />Authorized members without existing health screening from their Participating Agency will <br />be required to use the Sponsoring Agency's contracted medical service provider for health <br />screening. <br />Required testing includes; <br />Vital signs and standard physical examination <br />Laboratory <br />o CBC, Differential platelets <br />o Chemistry panel to include liver renal function tests <br />Aspartate Aminotransferase (AST) <br />Alanine Aminotransferase (ALT) <br />Alkaline Phosphatase (AlkPhos) <br />Gamma Glutamyl Transpeptidase (GGTP) <br />Total and direct Bilirubin <br />creatinine <br />blood Urea Nitrogen (BUN) <br />Glucose <br />electrolytes, (Na, K, CI, Co2) <br />Total protein <br />Albumin <br />Calcium. <br />o Urinalysis (urine dip, microscopic if indicated) <br />o Urine heavy metal screen, (qualitative, spot urine). HAZMAT task Force <br />members only <br />o Spirometry <br />o Chest x -ray (PA and Lateral) To establish baseline. Does not need to be repeated <br />annually unless medically indicated <br />o Resting 12 -lead ECG <br />24 <br />
