6. Determination of the degree of hazard and risk from the proposal both on <br />the site and on adjacent properties. <br />7. An assessment of the probable cumulative impacts to critical areas, their <br />buffers and other properties resulting from the proposal. <br />8. A description of reasonable efforts made to apply mitigation sequencing to <br />avoid, minimize and mitigate impacts to critical areas. <br />9. Plans for adequate mitigation to offset any impacts. <br />10. Recommendations for maintenance, short-term and long-term monitoring, <br />contingency plans and bonding measures. <br />11. Any technical information required by the Director to assist in determining <br />compliance with this chapter. <br />C. Geotechnical Report. <br />1. A geotechnical report appropriate both to the site conditions and the <br />proposed development shall be required for development in Class 2, Class 3, Class 4 <br />areas, and any areas identified as Coal Mine Hazard Areas. <br />2. Geotechnical reports for Class 2 areas shall include at a minimum a site <br />evaluation review of available information regarding the site and a surface <br />reconnaissance of the site and adjacent areas potentially impacted by the proposed <br />project. Subsurface exploration of site conditions is at the discretion of the geotechnical <br />consultant. <br />3. Geotechnical reports for Class 3, Class 4 and Coal Mine Hazard Areas <br />shall include a site evaluation review of available information about the site, a surface <br />reconnaissance of the site and adjacent areas potentially impacted by the proposed <br />project, a feasibility analysis for the use of infiltration on -site and a subsurface <br />exploration of soils and hydrology conditions. Detailed slope stability analysis shall be <br />done if the geotechnical engineer recommends it in Class 3 or Coal Mine Hazard Areas, <br />and must be done in Class 4 areas. <br />4. Applicants shall retain a geotechnical engineer to prepare the reports and <br />evaluations required in this subsection. The geotechnical report and completed site <br />evaluation checklist shall be prepared in accordance with the generally accepted <br />geotechnical practices, under the supervision of and signed and stamped by the <br />geotechnical engineer. The report shall be prepared in consultation with the Community <br />Development and Public Works Departments. <br />5. The opinions and recommendations contained in the report shall be <br />supported by field observations and, where appropriate or applicable, by literature <br />review conducted by the geotechnical engineer, which shall include appropriate <br />explorations, such as borings or test pits, and an analysis of soil characteristics <br />conducted by or under the supervision of the engineer in accordance with standards of <br />the American Society of Testing and Materials or other applicable standards.
