SECTION 14.3 Optical Plan. Examination and eye glasses/contact lenses for <br />all employees and dependents covered under this Agreement will be paid for <br />by the Employer as set forth herein. The City shall provide coverage for eye <br />examinations, vision and optical care, contacts, and eye glasses, to regular <br />full-time police officers and their dependents in the amount of of $250 per <br />person, to a maximum of $500 per family unit, each year. <br />SECTION 14.4 Life Insurance. The Employer shall pay one hundred percent <br />(100%) of the total premiums for life & accidental death and dismemberment <br />insurance benefits for the employees covered under this Agreement. The <br />face value of said insurance policy shall be $25,000 and shall include an up <br />to $25,000 dismemberment clause. <br />SECTION 14.5 LEOFF II Disability Insurance. With respect to LEOFF II <br />officers, the Association has elected to forego City coverage and has selected <br />an alternative long-term disability (LTD) plan. The City shall require LEOFF II <br />Officers to participate in the long-term disability insurance program selected <br />by the Association as a condition of employment. The City shall provide for a <br />mandatory payroll deduction to accomplish this purpose. <br />SECTION 14.6 Insurance Carrier. The City retains the right to select all <br />insurance carriers or to self -insure coverage as provided herein. <br />SECTION 14.7 Re -opening Clause. During the term of this Agreement, the <br />City and the Association each reserve the right to open negotiations in the <br />event healthcare reform legislation, including the Affordable Care Act (ACA), <br />mandates changes unanticipated by the parties. The purpose of such <br />negotiations shall be to reach agreement on a mutually acceptable alternative <br />medical option(s). <br />SECTION 14.8 Retirement Health Savings Plan. The City will establish and <br />maintain a mutually agreed upon, IRS qualified retirement health savings plan <br />as soon as possible after the implementation of the collective bargaining <br />agreement. In the event no agreement is reached, there will be no retirement <br />health savings plan for bargaining unit employees. <br />31 <br />205 <br />
